Aah's Overall philosophy on compensation arrangements: We will work with you to determine the fee structure and /or commission offset arrangement that best meets your needs. We are flexible in that these alternatives can range from "time and charges' to commissions only or a combination of both. We do not want the compensation structure to be a barrier to continuing our partnership with City of South Miami. Aon is always 100% transparent in regards to all compensation we receive. We will confirm on several occasions during the plan year the exact amount of commissions received by our office as requested. Aon Risk Solutions I Employee Benefits Consulting Group I Aon Risk Services. Inc. of Florida Proprietary & Confidential M Regardless of the arrangement, we provide full disclosure to City of South Miami of compensation received as commissions. We will guarantee "not -to- exceed" fee arrangements, and we will provide an accounting to you. Should, at any time, commissions grow to exceed the payment for services performed; we will provide additional services or reduce the commission amount. In general, we recommend an approach that frees City of South Miami from "watching the clock" so that unforeseen issues can be addressed promptly. Aon's fees for any additional work outside our standard scope of services would be determined after further consultation with City of South Miami regarding the specific service desired. Serving our clients in a transparent manner has been a longstanding practice at Aon. We have spent many years working to build a strong reputation based on two of our core values: client focus and integrity. Even the appearance of a conflict of interest is unacceptable. AON'S MANDATED DISCLOSURE TO CLIENTS REGARDING COMPENSATION IS AS FOLLOWS: In connection with placing, renewing, consulting on or servicing any insurance policy, prior to binding, Aon shall disclose to each client in writing: - All quotes and indications sought and all quotes and indications received by Aon in connection with the coverage of the client's risk with all terms, including but not limited to any Aon interest in or contractual agreements with any of the prospective insurers; and - All compensation to be received by Aon for each quote, in dollars if known at that time or as a percent of premium if the dollar amount is not known at that time, from any insurer or third party in connection with the placement, renewal, consultation on or servicing of insurance for that client. Our objective is to guarantee that the manner in which we get paid is transparent, understandable and acceptable to all relevant parties. Accordingly, Aon Risk Solutions clients receive full disclosure of all compensation earned for Aon's work on your behalf. Such disclosures will be provided prior to the time we bind your coverage and again at the end of the year. We will continue, as always, to seek the very best results to meet your business objectives. We will also: • Document our agreement with you on program design and the carriers and intermediaries to be approached. • Provide specific broker and intermediary compensation for each quote received. • Disclose contractual agreements with and ownership interests in insurers and intermediaries. • Report to you all Aon revenue annually. Aon Compensation Standards: There are no hidden fees or other income as it relates to your account. Our protocol, as it relates to client compensation, is the following: • In cases where commissions must be received, we must disclose to you in writing through a PTE -77 Commission Disclosure Form prior to binding the coverage. • Aon will provide the City of South Miami team with annual statements of compensation received. • Aon will not bill the City of South Miami for any services outside the scope without prior mutual agreement and a signed engagement letter for that project. Provide specific examples of a significant savings in the cost of benefits to the client that can be directly attributed to your past services. Jennifer Retamar was engaged in the renewal process for a client. The plan designs offered were very rich, low out of pocket plan designs. Due to low cost sharing by the employees and an increase in plan utilization, the costs continued to escalate on the fully insured program. Jennifer was able to review plan design alternatives and presented options for a consumer driven health plan (CDHP) approach in a dual choice program. The management team expressed concerns over the communication challenge they faced since many of the employees worked in the field and were not able to attend a group meeting. Aon developed a communication campaign to educate the employees on the CDHP designs. An enrollment firm was brought in to enroll the employees on a 1:1 basis. Much of this was done in a webinar / co- browsing format so all employees had access to an enroller. The cost savings to change the plan designs and enroll a high percentage of the employees in the CDHP program yielded a 21 % savings, allowing the client to remain within their budget parameters. 18, List any companies you are affiliated With or have contractual arrangement with including insurance companies, third party administrators (claims or other administrative /record keeping services), provider networks, HR or benefits, software vendors, etc, Aon has a partnership with the following companies: a. This guide highlights your options for 2010; please review it and share the information with your family. Full -time employees of ABC Company are eligible for medical and dental benefits on the first day of the month following 90 days from hire date. You may also enroll eligible family members for medical and /or dental coverage. Voluntary Vision coverage is also offered. Eligible family members may include: • Your legal spouse • Dependents up to 26 years of age, regardless of fill time student status Eligible unmarried children are defined as natural children, legally adopted children, stepchildren, and children for whom you have legal guardianship. See each plan's summary plan description for the specific definition of an eligible dependent or if you have questions about your benefits. Medical Benefits • In accordance with Health Care Reform, dependent children are now covered to age 26 and pre- existing conditions are waived for children under age 19. • You can no longer use funds from your Flexible Spending Account or Health Savings Account to purchase over- the - counter medications without a prescription from your physician. Voluntary Supplemental Life Insurance • Voluntary Supplemental Life Insurance will be offered through UnitedHealthcare beginning in 2011. Supplemental Life Insurance allows you to purchase more life insurance in addition to basic life insurance policy provided to you by ABC Company. The ABC Company medical plans are offered through UnitedHealthcare®. For the highest benefit levels, we encourage you to select a- provider from Unitedilealthcare's® extensive network. You may pay less out of your pocket and receive increased benefit coverage. ABC Company gives you two options for medical coverage: UnitedM lthccare" RthilriaGstithGrtmq Axxgw y 4 I. Traditional PPO - UnitedHealthcare® PPO plan is a traditional PPO plan with a network of doctors, hospitals and other health care providers that offer you health care at discounted prices. You can use any provider you want, but you will pay less when you use an in- network provider. Once you enroll in the PPO plan, you make co- payments for things like office visits, ER visits and prescriptions. You will also satisfy a Deductible before the plan pays benefits. After you've paid the Deductible, the plan shares the cost of your medical expenses with you. Generally, the plan paid 80% and you pay 20 %. If your out -of- pocket expenses totals the out -of- pocket maximum, the plan pays 100% of your eligible medical expenses for the rest of the calendar year (Deductible counts towards the out -of- pocket maximum). 2. UHC's Definity High Deductible Health Plan / Health Savings Account (HDHP/HSA) -The high deductible health plan has a much higher deductible than the PPO. You will be responsible for all of your out -of- pocket medical costs until you reach your deductible. Once you reach your deductible, the plan will pay 100% of your covered expenses for the rest of the year. Additionally, the plan offers a wellness provision that covers preventive care at 100 %, or at no out -of- pocket cost to you. To help you pay for your expenses until you meet your deductible, you may open an HSA. HSA's will be explained in greater detail on page 8. As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means your plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for the provision of preventive health services without any costs sharing. However, grandfathered health plans must comply with other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits. Questions regarding which protections apply and which protections do not apply to a grandfathered health plan and what might cause a plan to change h-om grandfathered health plan status can be directed to the plan administrator or your Employer. You may also contact the Employee Benefits Severity Administration. U.S. Department of Labor at 1- 866 - 444 -3272 or wuryv.dol.govlebsedhealthrefomt. If you think saving money on health care is hard, think again! Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) give you a valuable tax break. They allow you to pay eligible expenses with before tax pay - helping to make your income go farther. Your choice of FSA or HSA is dictated by your choice of medical plan. If you choose the Traditional PPO, you may also have an FSA. If you choose the Definity HDFIP/HSA, then you may open an HSA. While there are similarities in these plans, there are also some distinct differences. How Does a Flexible Spending Account (FSA) Work? Each year during the FSA Open Enrollment period, you can choose to contribute a specific dollar amount for the plan year, up to $3,050. Then, each pay period money is deducted (before taxes) from your paycheck and placed in your FSA. You use this money to pay your out -of- pocket medical /dental /vision expenses by using your debit card issued to you for your FSA. Remember to keep all your receipts! You will be asked to verify your purchases. Remember that you may choose an FSA with the traditional PPO plan. The plan year for 2010 has been extended to March 15, 2011. This means that submitted claims incurred between January 1, 2011 and March 15, 2012 will be applied to your 2010 FSA election. Funds allocated to your FSA in 2011 must be used by March 15, 2012. Employees who are or have been in the military may qualify for special FSA claims submittal benefits, allowable under Section 125(h) of the IRC code. A qualified reservist is an employee who is a member of a service component ordered or called to active duty for a period of 180 days or more. This benefit is governed by the IRS and has several technical provisions and rulings associated with it. Please see Human Resources for more details should this benefit apply to you. How does a Health Savings Account (HSA) work? A Health Savings Account (HSA) provides a tax -free way to help you build savings for current and future qualified medical expenses. HSA's offer several advantages. Unused funds and interest in calendar year are carried over, without limit, from year to year. The accounts are potable, which means you retain the balance even if you change jobs or health plans. Your money will be accessible through a variety of options, including a debit card, checks or ATM withdrawals. You are responsible for using the money for qualified expenses and keeping any documentation, such as receipts, to substantiate your purchases. Does a Health Savings Account make "cents" for you? To help you decide if the High Deductible Health Plan (HDHP) with Health Savings Account (HSA) option is right for you, review this list of HSA features: • An HSA is available only if you are enrolled in a HDHP (High Deductible Health Plan) Your HSA contributions are pre -tax through payroll deduction • Earnings on your account are also tax -free • Withdrawals from your account are tax -free, as long as used for qualified medical expenses • Unused funds "carry over" from year to year • A "catch -up" provision allows employees over age 55 to make additional contributions to an HSA account ($1,000 limit for 2010) • You own the HSA and it is yours to keep - even when you change plans or retire or move employment • Contributions can start or stop at anytime during the year HSA Contributions For 2011, if you have a single coverage HDHP, you may contribute up to $3,050 to your HSA. If you have a family coverage HDHP, you may contribute up to $6,150 to your HSA. Mothers' Health Protection Act Disclosure Group health plans and health insurance issuers generally may not, under Federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 homy following a vaginal delivery, or less than 96 hours following a cesarean section. However, Federal law generally does not prohibit the mother's or newborn's attending provider, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, plans and issuers may not, under Federal law, require that a provider obtain authorization from the plan or the issuer for prescribing a length of stay not in excess of 48 hours (or 96 hours). Michelle's Law (lIR 285 1) Signed by President Bush on October 8, 2008 and taking effect on October 9, 2009, Michelle's Law requires group health plans to provide the same benefits as if a dependent child continued to be a full -time student when he or she takes a medical leave of absence from college due to serious injury or illness for up to one year. Certification of the dependent child's illness by a physician must be provided to the health plan. 'rhe dependent child must be on the plan as a full -time student at a post - secondary educational institution prior to the first day of the leave. Coverage is required to extend for one year, but the health plan is not required to extend coverage beyond the period when coverage would otherwise terminate under the terms of the plan. 13 1 Your right to continued participation in the Plan during leaves of absence for active military duty is protected by the Uniformed Services Employment and Reemployment Rights Act (USERRA). Accordingly, if you are absent from work due to a period of active duty in the military for less than 31 days, your Plan participation will not be interrupted. If the absence is for more than 31 days and not more than 12 weeks, you may continue to maintain your coverage under the Plan by paying premiums. If you do not elect to continue to participate in the Plan during an absence for military duty that is more than 31 days, or if you revoke a prior election to continue to participate for up to 12 weeks after your military leave began, you and your covered family members will have the opportunity to elect COBRA Continuation Coverage only under the medical insurance policy for the 24 -month period (18 -month period if you elected coverage prior to December 10, 2004) that begins on the first day of your leave of absence. You must pay the premiums for Continuation Coverage with after -tax funds, subject to the rules that are set out in that plan. Wellness Incentive as part of a Group Health Plan If it is unreasonably difficult due to a medical condition for you to achieve the standards for the reduced contributions under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, please contact your Human Resources Department at 123- 456 -7890 and we will work with you to develop another way to qualify for the reward. HIPAA. Preexisting Condition Exclusions This plan imposes a preexisting condition exclusion. This means that if you have a medical condition before coming to our plan, you might have to wait a certain period of time before the plan will provide coverage for that condition. This exclusion applies only to conditions for which medical advice, diagnosis, care, or treatment was recommended or received within a 6 -month period. Generally, this 6 -month period ends the day before your coverage becomes effective. However, if you were in a waiting period for coverage, the 6 -month period ends on the day before the waiting period begins. The preexisting condition exclusion does not apply to pregnancy nor to a child who is enrolled in the plan within 30 days after birth, adoption, or placement for adoption. This exclusion may last up to 12 months (18 months if you are a late enrollee) from your first day of coverage, or, if you were in a waiting period, from the first day of your waiting period. However, you can reduce the length of this exclusion period by the number of days of your prior "creditable coverage." Most prior health coverage is creditable coverage and can be used to reduce the preexisting condition exclusion if you have not experienced a break in coverage of at least 63 days. To reduce the 12 -month (or 18- month) exclusion period by your creditable coverage, you should give us a copy of any certificates of creditable coverage you have. If you do not have a certificate, but you do have prior health coverage, we will help you obtain one from your prior plan or issuer. There are also other ways that you can show you have creditable coverage. Please contact us if you need help demonstrating creditable coverage. All questions about the preexisting condition exclusion and creditable coverage should be directed to your Human Resources Department at 123- 456 -7890. 14 The saying "knowledge is power" applies to your health plan benefits. You'll find a little knowledge can go a long way in helping you take charge of your health and saving money on your health care. Knowing how your health plan works is a great starting point. This glossary can help. Annual Deductible- The amount you must pay each plan year before the medical plan pays benefits. Coinsurance - The percentage of covered costs that you pay and that the medical plan pays. For example, an 80% coinsurance means you pay 20% of your covered expenses and the plan pays 80 %. Co- payment- The amount you pay per occurrence out -of- pocket for prescriptions and other health care services. FSA- A Flexible Spending Account (FSA) allows employees to have money withheld from their paychecks on a pretax basis to use to pay for unreimbursed medical expenses, such as co- payments and coinsurance. An FSA is used in conjunction with a PPO. HDHP- A High Deductible Health Plan (HDHP) is a health insurance plan that features a higher annual deductible with lower premiums than a traditional health plan. When you enroll, you may also open a Health Savings Account (HSA). HSA- A Health Savings Account is a savings medium that allows employees to pay for their current health expenses and save for future qualified medical and retiree health expenses on a tax -free basis. You must be covered by a High Deductible Health Plan (HDHP) in order to contribute to an HSA. Network- Care received from a network provider. Network care is usually covered at a higher benefit level than care received from a Non - Network service. Non - Network- Care received from a non- participating provider. Generally, you pay a larger percentage of the cost for non - network provider. Out -of- Pocket Maximum-The maximum amount you pay for eligible out -of- pocket expenses during the plan year.
