RESOLUTION:
A RESOLUTION OF THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI,
FLORIDA, AUTHORIZING THE CITY MANAGER TO EXECUTE AN ADDITIONAL WORK
ORDER (CHANGE ORDER) WITH T. J. PAVEMENT CORP. FOR THE CHURCH STREET
IMPROVEMENTS -PHASE 1 CONSTRUCTION IN AN AMOUNT OF $19,975.00 TO BE CHARGED
TO MIAMI -DADE COUNTY OFFICE OF COMMUNITY AND ECONOMIC DEVELOPMENT
(OCED) GRANT ACCOUNT NUMBER 001 - 0000 -115 -7044; PROVIDING FOR AN EFFECTIVE

BACKGROUND & ANALYSIS:
With proper management of the Church Street Improvements — Phase 1 project, the Public
Works Department has been able to secure administrative fees in an amount of $17,200.00 for
the project and have awarded a construction contract for $126,732.50 with installation of
electrical conduits for new Florida Power & Lights in an amount of $6,188.00. All expenses to-
date total $150,120.50 for an un- encumbered balance of $21,879.50. To avoid losing the un-
encumbered grant amount, it is recommended to award additional construction work to the
contractor (T.J. Pavement Corp.) out of the scope of work for Phase 2.
The additional scope of work includes: supply and installation of two tapping sleeves to loop and
increase water pressure along Church Street, pressure testing of each system and the resurfacing
of the construction areas. This project will be funded from the fiscal year 2003 Community
Development Block Grant through Miami -Dade County Community & Economic Development.
The balance for the grant account is $21,879.50.
RECOMMENDATION:
It is recommended that the City Commission approve funding for the work.

WHEREAS, the Mayor and City Commission wishes to provide a much needed water main
extension along Church Street, and
WHEREAS, the Mayor and City Commission authorize the City Manager to execute a change
order for additional construction under the contract with T.J. Pavement Corp. for construction of
an additional water main extension along Church Street for an amount of $19,975.00.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF
THE CITY OF SOUTH MIAMI, FLORIDA THAT:
Section 1: The remaining funds from this OCED grant account number 001- 0000 -115 -7044 are
being used to construct additional water main extension.
Section 2: The water main extension will be constructed within the Church Street Improvements
corridor.
Section 3: The project is funded through the Miami -Dade County Office of Community &
Economic Development, fiscal year 2003 Community Development Block Grant.
Section 4: The Mayor and City Commission authorize the City Manager to execute a change
order with T.J. Paving, Corp. for the construction of an additional water main extension within
the Church Street Improvements corridor for an amount of $19,975.00 to be charged to account
number 001 - 0000 -115 -7044.
