Miami Dade County has prepared tentative design plans for drainage and traffic improvements
for the above referenced location (see attached plans). I spoke with Ms. Teresa Novakovic,
P.E. from Miami -Dade County Traffic, who informed me that traffic and drainage improvements
are being done to alleviate the traffic backup (bottleneck) along SW 67th Ave. (due to limited
storage lane capacity) and ponding near the bus stop areas at the intersection of S.W. 56th
St/SW 67th Ave. Ms. Novakovic also indicated that the grass swale near the bus stops is
deteriorated due to drivers who cut thru the grass swale to avoid the traffic backup in this area.
Ms. Novakovic also informed me that the county received public complaints about the ponding
happening near the bus stop area as well as the traffic backup. As a result of this, the county
has gone forth and performed traffic counts and percolation tests for the design improvements.
Ms. Novakovic indicated that the design should be finalized in a month, but that availability of
funds for construction was yet to be confirmed. I also spoke with Mr. David Hayes from Miami -
Dade County Traffic who indicated that the site was inspected with staff (Mr. Ruben Arias) from
the office of Miami Dade County Commissioner Gimenez (District 7) to make this matter a
priority.

19. EXISTING ABOVE GROUND FEATURES ARE SHOWN ACCORDING TO THE BEST AVAILABLE
DATA AND MAY NOT ACCURATELY REFLECT PRESENT CONDITIONS. CONTRACTOR SHALL BE

DEPARTMENT WITH IMMEDIATE NOTIFICATION OF ANY CONFLICT WITH PROPOSED
CONSTRUCTION. THIS NOTIFICATION SHALL PROVIDE SURVEY INFORMATION ABOUT EXISTING
UTILITY ALIGNMENT, GRADE AND POSSIBLE CONFLICTS. PAYMENT FOR EXPLORATORY OR

41. ALL DITCH EXCAVATIONS SHALL BE PERFORMED IN FULL COMPLIANCE WITH THE PROVISIONS
OF THE TRENCH SAFETY ACT.
42. THE CONTRACTOR SHALL PREPARE AND SUBMIT SHOP DRAWINGS FOR ALL ITEMS USED IN

45. EXISTING MANHOLES AND INLETS SCHEDULED TO REMAIN SHALL BE THOROUGHLY CLEANED
BY REMOVING ALL DEBRIS AND SEDIMENTS, AND THE INTERIOR SHALL BE SEALED WITH AN
APPROVED NON -TOXIC BITUMSTIC SEALANT.
46. PRIOR TO CONSTRUCTION THE CONTRACTOR WILL INSPECT ALL EXISTING STRUCTURES WHICH
ARE TO REMAIN AND NOTIFY THE ENGINEER OF ANY OBVIOUS STRUCTURAL DEFICIENCIES.
47. PRIOR TO CONSTRUCTION THE CONTRACTOR WILL VERIFY INVERT ELEVATIONS OF ALL PIPES
WHICH ARE TO REMAIN AND NOTIFY THE ENGINEER OF ANY ELEVATION DEVIATIONS.
48. THERE SHALL BE NO MORE THAN THREE LATERAL DRAINAGE INSTALLATIONS WITHOUT
BACKFILLING. BACKFILLING OF LATERAL DRAINAGE SHALL NOT LAG MORE THAN. 72 HOURS
BEHIND THE START OF EXCAVATION.
49. SPECIAL ATTENTION IS DIRECTED TO THE FACT THAT PORTIONS OF SOME DRAINAGE
STRUCTURES EXTED INTO THE STABILIZED PORTION OF THE ROADBED AND EXTREME
CAUTION SHOULD BE USED IN THE STABILIZING OPERATIONS AT THESE LOCATIONS.
50. COMPLETE AS -BUILT INFORMATION RELATIVE TO LOCATION AND DEPTH OF PIPES,
MANHOLES, ETC. SHALL BE ACCURATELY RECORDED BY THE CONTRACTOR AND SUBMITTED
TO THE ENGINEER PRIOR TO FINAL ACCEPTANCE OF THE WORK. ALL ELEVATIONS SHALL
BE TAKEN BY A FLORIDA REGISTERED SURVEYOR AND SHOWN ON THE RECORD DRAWINGS.
51. ALL STATIONS AND OFFSETS REFER TO (CENTERLINE)/[BASELINE] OF CONSTRUCTION,
UNLESS OTHERWISE STATED.
52. ALL GRADES SHOWN IN PLAN ARE EXISTING GRADES FROM SURVEY.
53. THE CONTRACTOR SHALL PAINT ALL STATIONS WITH STENCILED NUMBERS ON THE FACE OF
CURB:
A. FROM THE BIGINNING OF THE PROJECT WHERE THE CURB IS TO REMAIN.
B. AT NEW CURB NO LATER THAN 72 HOURS AFTER BEING POURED.
C: WHERE CURB DOES NOT EXIST AND SHALL NOT BE CONSTRUCTED.
THE CONTRACTOR SHALL MAINTAIN STATIONING WITH SURVEYING STAKES.
CONSTRACTOR SHALL MAINTAIN THE STATION MARKS VISIBLE UNTIL FINAL
INSPECTION.
54. CLEARING AND GRUBBING, GRADING, AND OTHER INCIDENTAL WORK NECESSARY FOR
HARMONIZING OUTSIDE R /W.SHALL BE INCLUDED IN RELATED BID ITEMS.
55. ANY KNOWN OR SUSPECTED HAZARDOUS MATERIAL FOUND ON THE PROJECT BY THE
CONTRACTOR SHALL BE IMMEDIATELY REPORTED TO THE PROJECT ENGINEER, WHO SHALL
DIRECT THE CONTRACTOR TO PROTECT THE AREA OF KNOWN OR SUSPECTED
CONTAMINATION FROM FURTHER ACCESS. THE ENGINEER WILL ARRANGE FOR
INVESTIGATION, IDENTIFICATION, AND REMEDIATION OF THE HAZARDOUS MATERIAL. THE
CONTRACTOR SHALL NOT RETURN TO THE AREA OF CONTAMINATION UNTIL APPROVAL IS
PROVIDED BY THE PROJECT ENGINEER.
56. WHEN DISSIMILAR MATERIAL CONNECTIONS ARE MADE, SUCH AS CONCRETE TO METAL, THE
DISSIMILAR MATERIAL SHALL BE SEPARATED BY COATING THE CONTACT SURFACE_ WITH
BITUMASTIC MATERIAL.
57. CONTRACTOR SHALL ADJUST ALL EXISTING CATCH BASINS, GRATES, AND STORM MANHOLE
COVERS TO MEET NEW GRADES WHERE APPLICABLE.
