Request: To enter an agreement with CCDH Inc's All Children Together (ACT) Resource Network for the provision of inclusion training at no cost to the City of South Miami. Background: CCDH, Inc. has been Miami -Dade County's lead agency in partnering with people with disabilities and their families to plan, provide, and coordinate services. The Children's Trust has funded CCDH, Inc. to develop the .ACT Resource Network to increase the participation of children with disabilities and their families in all aspects of community life through capacity building, advocacy, and promoting best practices. As a funded agency of the Children's Trust, we are granted the opportunity to work with this agency at no cost. As part of CCDH's agreement with the Children's Trust, the ACT Resource Network will be working with children's service providers, funded by the Children's Trust, as we are, to improve our capacity to include children with disabilities. We are committed to assuring that all children are included in community life through summer camps, after school programs, the arts, athletics and all the activities that help enrich and protect our children. APPROVED: MAYOR COMMISSION VOTE: Mayor Feliu: Vice Mayor Beasley: Commissioner Palmer: Commissioner Newman: Commissioner Sellars: MEMORANDUM OF AGREEMENT Between CCDH, Inc.'s All Children Together (ACT) Resource Network and City of South Miami THIS MEMORANDUM OF AGREEMENT is entered into this day of 2009 by and between CCDH Inc 's All Children Together (ACT) Resource Network, located at 9555 North Kendall Drive, Suite 206 - Miami Florida 33176, hereinafter 6130 Sunset Drive, South Miami, Miami Florida 33143, hereinafter referred to as "City of South Miami WHEREAS, CCDH, Inc. has been Miami -Dade County's lead agency in partnering with people with disabilities and their families to plan, provide, and coordinating services; WHEREAS, The Children's Trust has funded CCDH, Inc. to develop the ACT Resource Network to increase the participation of children with disabilities and their families in all aspects of community life through capacity building, advocacy, and promoting best practices, WHEREAS, the ACT Resource Network will be working with children's service providers, funded by the Children's Trust, to improve their capacity to include children with disabilities, WHEREAS, both parties are committed to assuring that all children are included in community life through summer camps, after school programs, the arts, athletics and all the activities that help enrich and protect our children, WHEREAS, both parties agree to use the "Pathways to Inclusion ", as a best practice guide to improving capacity to include children with disabilities, NOW THEREFORE, CCDH /ACT and (City of South Miami), have agreed to diligently work together on the following pathway domains, and to collaboratively develop IPIP goals. It is understood that ACT project staff are consultants and advisors and may not serve as additional staff to the programs with which they are working. As part of its commitment to increase its capacity to become more inclusive of children with disabilities, The City of South Miami agrees to consult with CCDH /ACT staff prior to determining whether or not it is able to serve a particular child who has a disability. Both Agencies agree that any and all client, financial, proprietary or other sensitive information about the program will be treated confidentially, and not be shared without express consent. EFFECTIVE TERM The Parties agree that the effective term of this Contract shall be from the date of signature through March 30, 2010, subject to funding availability and Provider's performance..
