Request: To authorize the City Manager to execute a Memorandum of Agreement with
Thelma Gibson Health Initiative.
Reason/Need: During the second budget hearing on September 22, 2009, the City
Commission allocated funding to TGHI for Social Work services for the
seniors in the Senior Citizen Center. This funding is intended to fund
much needed social work services to the seniors, at its original level of
service before funding was withdrawn by the County due to budget
constraints.
Cost: $45,000.00

RESOLUTION NO.:
A RESOLUTION OF THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI,
FLORIDA, AUTHORIZING THE CITY MANAGER TO EXECUTE A MEMORANDUM OF

been undertaken, it is important that these activities continue; and
WHEREAS, this $45,000.00 funding from the City will return this worthwhile program to its original level of
service to our seniors; and
WHEREAS, TGHI has agreed to assume responsibility for implementing and continuing this vital program to
enhance sense of community and social benefits within the neighborhoods of the City of South Miami.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF THE CITY
OF SOUTH MIAMI, FLORIDA THAT:
Section 1: The Mayor and City Commission authorize the City Manager to execute a Memorandum of
Agreement with Thelma Gibson Health Initiative for social work services to the senior residents at the Senior Citizen
Center in an amount of $45,000.
Section 2: The attached exhibit is incorporated by reference into this resolution.

MEMORANDUM OF AGREEMENT WITH THELMA GIBSON HEALTH
INITIATIVE (TGHI) FOR FUNDING OF SOCIAL SERVICES FOR AT -RISK
MINORITY RESIDENTS OF SOUTH MIAMI
This AGREEMENT entered into this day of October, 2009, by and
between The City of South Miami, a Florida Municipal Corporation (the "City ") and The
Thelma Gibson Health Initiative (the "TGHI ").
WHEREAS, the City, has in the past benefited from a program managed by
TGHI, which directly benefited at -risk minority residents of South Miami by providing
social services, and
WHEREAS, the funding from the County and other sources previously received
by TGHI has now been withdrawn due to budget constraints in Miami -Dade County, and
WHEREAS, the City of South Miami, through its City Commission, believes
that providing $45,000.00 towards revitalizing and continuing this worthwhile program is

Section 1. Scope of Services
The scope of services to be provided by TGHI is attached hereto as Exhibit A,
which document (Exhibit A) is attached to and made a part of this Memorandum of
Agreement as though fully set forth herein.
Section 2. Compensation
The total compensation to be paid for the above services described in Section 1
will be an allocation by the City Commission of the sum of $45,000.00 to TGHI.
Section 3. Time of Performance
The time of performance of this Agreement shall commence as of the 1 s` day of
October, 2009 and shall continue to and through the 30'' day of September, 2010 as
described in the attached Exhibit A.
Section 4. Personnel
TGHI represents that it has presently employed or will subsequently engage all
the necessary personnel required to perform the services under this Memorandum of
Agreement.

Communications and details concerning this Agreement shall be directed to the

The time of performance for services to be performed under this Agreement may
be extended, prior to its expiration, for a period not to exceed one year, by written
administrative approval from the City Manager. All such notices relating to performance
shall be provided as provided for in Section 5, above of this Memorandum of Agreement.

SCOPE OF SERVICES
ORGANIZATION INFORMATION
Mission Statement
The mission of the Theodore R. Gibson Memorial Fund, Inc (TRGMF) which was
incorporated in 1983 is to "unify and bridge gaps between diverse groups within the local
Coconut Grove and wider Miami and Miami -Dade communities ". This organization was
founded in memory of the late Reverend Theodore R. Gibson, an outstanding civil rights
leader of the 1960's as well as a City of Miami Commissioner for nine years.
The Thelma Gibson Health Initiative (TGHI) was established in December 2001,
under the aegis of the 501 (C) (3) tax - exempt status of its parent agency, TRGMF.
The mission of TGHI is to "building healthy minds and bodies" by addressing disparities
in health risk, conditions, vulnerabilities and access to health care systems.
This is accomplished through the offering of culturally competent and sensitive
comprehensive social services for minority, poor, fragile and marginalized citizens
residing in Miami -Dade County. This Agency has a nine year history of assisting
residents of West Coconut Grove, South Miami and adjoining Coral Gables with an array
of health care- related and social service needs prompted and compounded by the
following socio demographic characteristics: poverty, minority status, higher than
average rates of illiteracy, increased rates of chronic and disabling conditions with
co morbidity for earlier mortality diminished access to existing systems of health care
and elevated incidence of domestic violence and other forms of violence.
Oreanizational Structure
• Thelma V A Gibson, Volunteer, President Emeritus of TRGMF. Mrs. Gibson is a
former interim City of Miami Commissioner and retired Registered Nurse with over
twenty -five years of nursing and eight years of social work experience in addition to
starting and serving on the boards of many social service organizations.
• Merline J Barton, Certified HIV Tester and Executive Director, has worked in the
Coconut Grove and adjacent communities for more than twenty years with contributions
in community development, organizational management and interagency collaboration.
Ms. Barton is responsible for all Agency operations, fiscal and personnel management as
well as grant writing, management and compliance.
• Maritza Eienbaum, MED, LCSW, Program Director /Clinical Supervisor has thirty
years experience of working in social services with a focus on serving at -risk populations
of all ages, emphasizing empowerment and advocacy as key ingredients in this work.
Ms.Ejenbaum is responsible for program design and oversight, staff and intern training,
clinical supervision, grant writing and implementation.
• Patricia Taboas, MS, Youth Program Manager has significant experience in working
with at -risk youth in a wide variety of settings. Ms. Taboas is responsible for the
• day to day management of the Thelma Gibson Health Initiative Youth Prevention and
Intervention Program funded by The Children's Trust.
• Mark McCray, MSW and Certified HIV Tester, experienced with serving at -risk
individuals, youth, families and seniors. Mr. McCray provides intensive case
management and comprehensive social services to clients of all ages in the Agency and
community settings.
• Tahiese Beckford, Resource Coordinator has extensive experience in procuring a wide
array of resources, working with media, coordinating, designing and executing special
events. Ms. Beckford is responsible for resource identification and procurement, media
relations and exposure, design and execution of special Agency and community events
and technical assistance.
• Rosemary Peralta, Certified HIV Tester, Administrative Assistant, serves as support
staff for the Executive Director and the Program Director. Ms. Peralta is responsible for
sustaining and assisting the administrative staff with the day to day operations of the
Agency.
• Donna Clarit, Outreach Specialist, has long.history and strong ties with the community.
Ms. Clarit serves in street outreach and is the TGHI's liaison and contact for residents
when crises arise outside of the Agency's normal operating schedule.
Social Worker, BSW, TBA ( For the South Miami Community).
This innovative At -Risk Seniors Program serves vulnerable poor minority older persons
(ages fifty and above) residing in public housing and the community at -large in South
Miami. This program has been successfully providing these services to vulnerable
seniors in the City of South Miami for five years.
The main goal of this program is to maximize "Aging in Place" for each program
participant. "Aging in Place" is a principle related to an individual's choice to remain in
his/her own home as long as possible through the provision of comprehensive social
services thereby avoiding premature institutionalization or unnecessary death or
disability. (The American Association of Homes and Services for the Aging 2006)
This has been accomplished with a model of service delivery that addresses disparities in
health care access due to the combined factors of poverty, age, illness, illiteracy, lack of
health literacy and minority status. This is achieved with specialized outreach,
assessment of need and prioritization, intensive targeted case management, education,
counseling, crisis intervention and advocacy. The ultimate. desired outcome is enhanced
quality of life through increased access to health care benefits and services and other
resources that enable members of this target population to remain in their own homes as
long as possible.
The cost effectiveness of this model and its outcomes is significant. The human benefit
of avoiding premature institutionalization is indisputable.
PROJECT DESCRIPTION
Statement of Need
The project known as "Social Services for At -Risk Minority Seniors" who reside in the
City of South Miami has been severely impacted by cuts in funding. The service needs in
this target population of vulnerable minority residents ages 50 and older continue to be
affected by the following current socioeconomic variables:
• Lack of sufficient financial resources
• Deficiencies in adequate nutrition
• Increasing scarcity in safe, decent and affordable housing
• Illiteracy and/or lack of health literacy
• Limited or nonexistent social support
• Decreasing resources such as cutbacks in hospitals and related services
The current economic climate has affected many in this community and as always, those
that are historically more compromised by risk factors are impacted disproportionately
more so by adverse socioeconomic conditions.
The provision of a social service program that addresses the needs of poor, minority
elders is even more imperative than before because of the following reasons:
• The needs of seniors that have been served by this program are more intensive as
they age because they require more supportive services.
• There are additional elders that need to be served both in public housing and in
the community at large in the City of South Miami
• The existing systems of health care have become increasingly difficult to navigate
due to funding cuts that affect access and availability of services. This requires
more intensive and targeted case management which in turn has become more
demanding in terms of staff time, expertise and advocacy.
Proeram Approach and Desizn
The Thelma Gibson Health Initiative proposes to provide comprehensive health - related
social services with a primary directive of facilitating necessary access to healthcare,
supportive and preventive care for a minimum of two hundred (200) at -risk older
persons who reside in the City of South Miami, both in public housing and the
community at large. Additionally, this program will provide Education on matters
of Health, Safety and Well Being to a minimum of 400 program participants duriniz
the proiect year.
The target population is composed of vulnerable Black, Hispanic and other minorities
residing either in public housing or the community at large in the City of South Miami.
These individuals are in critical need of this program due to the following
socio demographic characteristics: poverty, minority, higher prevalence of chronic and
disabling illnesses, elevated incidence of complications and/or morbidity from those
conditions, illiteracy /low literacy (both general and health- related) , lack of access to
health care benefits /services and a generalized common lack of trust of the medical
system.
The ultimate goal of this project is to maximize each participant's quality of life by
facilitating their ability to "Age in Place ", by preventing unnecessary or premature
institutionalization, disability or death.
This program design is one that seeks to reduce personal socioeconomic, socio cultural
and systemic. barriers.
This will be accomplished by offering the following services and activities:

places, churches, parks and the community at- large.
