09-184(3)-002Facility will be storing the following unused hazardous materials for their
operations and these Materials witl be 'placedJn- Lhe specially designed
cabinet located with —in the. lab - room (Le.�' Synthetic Alipathic
Hydrocarbon, Reagent Alcohol, Eosin'" -in Al I cohol,' Hematoxylin, and
Lithium Carbonate in Alkalized Water). All of these chemicals will be
stored in one (1) gallon containers with no more than four (4)
containers at any time.
c) Photo room will be equipped with an HP Officejet Pro L7000 Series All in
one Printer, Fax, Scanner & Copier that utilizes ink cartridges only.
7) The facilities lab room where all of the hazardous materials will be
stored does not have any floor drains or sinks, as required by the EQCB
Order.
f) Facility will install high efficiency dual flush toilets delivering 0.8 or 11
gaillons per flush or high efficiency toilets labeled "'Water Sense"
EPA.
• Plans show the location of a ground water monitoring well to be located
downstream of the septic tank's drainfield in the direction of the
groundwater flow and a maximum of ten feet away from drainfield for
the Laboratory area.
h) Facility will be required to sample and analyze the groundwater from the
monitoring well sixty (60) days after installation of the groundwater
monitoring well and submit said results to the DERM. The sampling
parameters are EPA it (Lithium, Aluminum), HPLC03/HPLC-UV
(Acetic Acid), EPA 8015 (Ethylene Glycol), EPA 8015 (Alcohols (ethanol,
methanol, isopropanol) and EPA 8315 (Formaldehyde).
3. The only waste water that can be discharged to the septic tanks systems is
domestic sewage.
4. Operations at this facility shall be conducted without any discharges to oper,
ground or to the storm sewer system.
5. Plans show the future use of portable emergency generators, please be advised
that any deviation on the type of emergency generator systems will require
review and approval from the DERM.
