A Resolution authorizing the City Manager to permit Team Footworks Educational
and Fitness Corporation to host a Twilight SK Trilogy on multiple dates during the
2023 calendar year.
WHEREAS, Team Footworks Educational and Fitness Corp. ("the applicant") has submitted a
special event application to host a Twilight SK Run/Walk Triolgy event series, followed by a post-race
event from 6:00 PM to 10:00 PM on multiple dates-Sunday: January 15, 2023; May 14, 2023; and,
September 24, 2023; and
WHEREAS, in the event of inclement weather on September 24 th, the alternate date
requested is October 15, 2023; and
WHEREAS, the race starts on SW 58 th Avenue and SW 74 th Street and ends on SW 57 th Court
between SW 73rd and 74th Street. The applicant is requesting for the necessary street closures that
correspond with the race route to ensure a safe and fun event for both participants and the
surrounding area. Barricades and police vehicles will block traffic at affected intersections. Refer to
the course map attached; and
WHEREAS, following the race the applicant plans to host a block party event in the South
Miami First National Bank parking lot, which is located directly behind the bank. Sponsors will have
tents, providing food and goods samples. An inflatable bounce house and face painting activities will
be available for children; and
WHEREAS, the applicant has submitted the Notification/Petition form from businesses in the
area as well as the Location Approval form signed by the property owner of First National Bank of
South Miami. Additionally, the applicant is required to submit the necessary Certificate of Insurance
that complies with the City's standard insurance requirements, including naming the City of South
Miami as an additional insured prior to the event; and
WHEREAS, total event fees amount to $39,876.52; and

NOW THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSIONERS OF THE CITY OF
SOUTH MIAMI, FLORIDA:
Section 1. The foregoing recitals are hereby ratified and confirmed as being true and they
are incorporated into this resolution by reference as if set forth in full herein.
Section 2. The City Manager is hereby authorized to permit Team Footworks Educational and
Fitness Corp. ("the applicant") to host a Twilight SK Run/Walk Triolgy event series, followed by a post-
race event from 6:00 PM to 10:00 PM on multiple dates-Sunday: January 15, 2023; May 14, 2023;
and, September 24, 2023. The race starts on SW 58th Avenue and SW 74th Street and ends on SW 57 th
Court between SW 73 rd and 74th Street. The City Manager is authorized to permit all necessary street
closures that correspond with the race route to ensure a safe and fun event for both participants and
the surrounding area. Refer to the course map attached.
Section 3. In the event of inclement weather or cancellation on September 24 th, the City
Manager is hereby authorized to permit Team Footworks Educational and Fitness Corp. to host a
race followed by a post-race event on October 15, 2023 from 6:00 PM to 10:00 PM.
Section 4. The City Manager is also hereby authorized to waive the following event-related
service fees in the amount of $20,544.52.

Section 5 . Corrections. Conforming language or technical scrivener-type corrections may be
made by the City Attorney for any conforming amendments to be incorporated into the final
reso lution for signature.
Section 6 . Severability. If any section clause, sentence, or phrase of this re so lution is for any
reason held inva li d or unconstitutional by a court of competent jurisdiction, the holding shall not
affect the validity of the remaining portions of this resolution.
Section 7. Effective Date. This resolution shall become effective immediately upon adoption.

BACKGROUND:Team FootWorks Educational and Fitness Corp. (“the applicant”) has
submitted a special event application to host a Twilight 5K Run/Walk
Triolgy event series, followed by a post-race event from 6:00 p.m. to 10:00
p.m. on multiple dates—Sunday: January 15, 2023; May 14, 2023; and,
September 24, 2023. In the event of inclement weather on September
24th, the alternate date requested is October 15, 2023.
The race starts onSW 58th Avenue and SW 74th Street and ends on SW 57th
Court between SW 73rd and 74th Street. Theapplicant is requestingfor the
necessary street closures that correspond with the race route to ensure a
safe and fun event for both participants and the surrounding area.
Barricades and police vehicles will block traffic at affected intersections.
Refer to the course map attached.
Public sidewalks will remain open to the public. Streets, sidewalks, gutters,
etc. will be monitored and cleaned by Public Works staff.
Following the race the applicant plans to host a block party event in the
South Miami First National Bank parking lot, which is located directly
behind the bank. Sponsors will have tents, providing food and goods
samples (e.g. ON shoes will be providing demos for the runners to test on
their 5K run). South Miami Hospital and Ussery Automotive Group are
event sponsors and will have tents. There will be a couple of Mercedes-
Benz automobiles on the lot for display purposes. An array of local
restaurants as in-kind sponsors will provide sample bites for the
participants. Wynwood Brewery will participate as an in-kind sponsor
providing samples to the participants (only participants over 21 with a beer
band will be granted a sample). An inflatable bounce house and face

painting activities will be available for children. The post-race block party
will end by 9:30 p.m.
As a sponsoring parter of the event, the applicant is requesting to addand
attach the City’s logo on its marketing and advertisement materials, in the
local newspaper, social media, and award certificates for all three (3) races.
The applicant has submitted the Notification/Petition form from

13INDEMINIFATION AGREEMENT THIS AGREEMENT ("Agreement") is entered into by and between _T_e_a_m_F_o_o_tW_o_r_k_s _______ _ ("Applicant''), and the City of South Miami ("City") on the date on which the last of the Parties executes this Agreement. RECITALS WHEREAS, the Applicant has submitted a Special Event Permit Application to the City for (event title) Sunset SK Run/Walk• Twilight SK Run/Walk-Dusk 5K Run/Walk at SW73rd Street & 57th Court (location) on Sunday 01/15123, 05/14/23 & 9/24/23 (10/15/23 Alternate! {date(s)) ______________ . "Special Event"); and WHERAS, pursuant to of the City's Code and the City's Special Event Regulations, the Applicant must execute an indemnification and hold harmless agreement protecting the City from claims which may arise out of the Special Event. Now, THEREFORE, in consideration of the matters recited above, the mutual covenants set forth herein, and other good consideration the receipt and sufficiency of which is hereby acknowledged, the Parties hereby agree as follows: 1. The above recitals are true and correct. 2. Applicant agrees to indemnify, defend and hold the City, its officers, affiliates, employees, successors and assigns ( collectively "lndemnitees") harmless from and against any and all such claims, suits, actions, damages, or causes of action arising as result of the Special Event, or of the condition of the site on which the Special Event is held including any personal injury or loss of life, or damage to or loss of property, and from and against any costs, attorney's fees, expenses or liabilities included in and about the defense or settlement of any clams, and the investigation thereof, except to the extent caused by indemnitees negligence of willful misconduct. 3. No Street shall be closed without approval from the City Commission. 4. Traffic shall be maintained in accordance with Florida Department of Transportation (FDOT), Miami-Dade County standards and any additional requirements by the City of South Miami Public Works Department and Police Department. 5. Event areas shall be restored to equal or better condition than they were before the event started. 6. Any damage to private property shall be restored to its original condition or better and as accepted by the Owner caused by applicant or any of its agents, servants or employees, invitees and onlookers. 7. If all restoration work is not performed within 30 days of work completion, the City of South Miami Public Works Department may restore the event area and charge the applicant for the cost of restoration and additional incidental fees. 8. Applicant must pay all fees prior to the date of their event(s). 9. Applkants shall notify the City of South Miami Special Event Coordinator at least forty-eight (48) hours prior to start of their event of any cancellations to be considered for any refunds. Walk will be held on June 5, 2022. This is a Sunday
evening run/walk that starts at SW 58 Ave and SW 74

The SK run/walk event begins at 6:00pm and will take about one hour to
complete. We are expecting approximately 1,200 participants. The lead
runners will run just over/under 5-minute miles -meaning that they will
get to mile one at 6:05pm, mile two at 6:10pm and the finish (3.1 miles) at
6:16pm. Walkers will finish at about6:40 to6:55pm.

FINAL CHECK LIST
This list is to ensure that all necessary documentation is included and that all required procedures are
followed. The Special Event Coordinator will check the application against this checklist to ensure all
information was submitted correctly.
APPLICATION: All information has been properly filled out on the application
with all required documents at least 21 days prior to the
proposed event.
APPLICATION FEE: A $60 application fee and $100 administrative fee must
accompany your submission in check or money order form.
SIGNATURES: Application must be signed and notarized.
EVENT NARATIVE: Briefly discuss your Organization/Business, the purpose of your
event, the benefits, etc.
NOTIFICATION FORM: The name, signature, address and phone number of each person
who is either a property owner or lessee within the requested
event area must appear on the sheet.
SITE PLAN: A site plan must be submitted with the application that must
show intersecting streets, parking areas, tents, stages, routes,
barricades, etc.
COPY: It is strongly recommended that you make a copy of your
application for your own records

Once the application has been approved processed and the date has been reserved the following must be
submitted prior to the event date.
INSURANCE: The applicant is required to submit a Certificate of Insurance
naming the event and the City of South Miami (6130 Sunset Drive
South Miami, FL 33143) as an additional insured, in the amount
of $1,000,000.
EVENT FEES: All other City service fees including but not limited to, Off-Duty
Police, Public Works, Parking Meters, etc. will be due ten (10)
days prior to the event date.

It is our pleasure to announce that the 25th annual South Miami Hospital Twilight 5K has become the
Twilight Trilogy 5K. We have added two (2) events to our Twilight 5K. Dates will be as follows:
Race #1: SUNSET 5K, January 15, 2023 /
Race #2: TWILIGHT 5K, May 14, 2023
This year’s Twilight 5K will also celebrate FOOTWORKS 50th anniversary (FootWorks running store
has been in the same location in South Miami since 1973).
Race #3: DUSK 5K, September 24, 2023
(Contingency date in the event of severe weather – October 15, 2023)
The races will begin at 6PM and we expect to have about 1,200 participants.
TeamFootWorks is a 501(c)(3) non-profit corporation dedicated to fitness education through the
production of events. We have been an institution in this community for almost 30 years, and supporters
of the City of South Miami via charitable contributions.
As active participants of this community, we would like to request the City of South Miami waive all fees
for City services that will be incurred by TeamFootWorks for this event. These to include, but not limited
to the following:

Public Works
Parking Meters

SK start time: 6:00PM • South Miami
Dear Neighbor:
The annual South Miami Hospital Twilight SK Run /
Walk will be held on June 5, 2022. This is a Sunday
evening run/walk that starts at SW 58 Ave and SW 74
St and finishes on SW 57 Ct (Hans Huseby Run)
between SW 73 and 7 4 St. Please see the course map on the right.
The SK run/walk event begins at 6:00pm and will take about one hour to
complete. We are expecting approximately 1,200 participants. The lead
runners will run just over/under 5-minute miles -meaning that they will
get to mile one at 6:05pm, mile two at 6:10pm and the finish (3.1 miles) at
6:16pm. Walkers will finish at about 6:40to 6:55pm.
Dotted line indicates racecourse and will be shut down from 6pm-7pm. If
you need to leave your home, please avoid these streets.
