Dear Mr. Mumford:
As a supplement to the Environmental Clearance request delivered to you last week,
attached is a Thorough Property Inspection and Surrounding Property Review report
prepared for the referenced property. The report was prepared in October 2001 following
a Phase 1 Audit dated September 3, 1997.
The Inspection report indicated that "No evidence of negative environmental impact was
observed." And the report recommended no further assessment for the subject site. The
report was prepared by Environmental Site Assessments, Inc., we thought this report is an
important document for your consideration.

Review of Phase I Audit dated September 3, 1997 submitted to Miami-Dade County and City of

Dear Mr. Goodwin:
Environmental Site Assessments, Inc.,(ESA) has completed a property inspection and surrounding property
review at the request of the City of South Miami, in order to determine the current status of the above
referenced site. ESA had completed a Phase I Audit for the site on September 3, 1997, and the results of
the Phase I were that further assessment was warranted. The assessment recommended as to collect a
sample from the underground structure on site, and analyzed for disposal profile. Once the sediments have
been profiled,the contents should be property removed, and the structure properly abandoned.The structure
was not an underground tank. As there was documented contamination on-the adjacent south property, it
was recommended to install a soil boring and monitoring well to assess the soil and groundvrater as a result
of the adjacent south property activities.
ESA conducted a site inspection of the site on October 5, 2000. No evidence of the former underground
structure was visible on the surface of the site. The site consists of a vacant lot.The surface of the site was
throughly inspected, as the vegetation on site is kept mown. No evidence of distressed vegetation was
observed during the site inspection. No evidence of negative environmental impact was observed.
File review was completed for the adjacent south property, Independent Transmission Services
(IV11031UT2004!102) at the Miami-Dade County Department of Environmental Resources Management
(D=RDA). The site has had numerous violations for various infractions since 1986(please refer to Phase I
dated or further information). ESA personnel revie:ved the file on October 23, 2000, to determine the
environmental status of the property.Since the last file review in 1997, the site has been regularity inspected
by D=RDA at least once per year.All violations noted in the files prior to 1997 had beentcorrected, per DERM
correspondence 8/26/93. The 6/19/98 Facility Inspection Report (FIR) reported no violations for the site.
Based on the 7/20/99 FIR the site was issued a Pollution Prevention Field Notice (P2FN), as the inspector
note; improper storage of waste oil drums on the south side of the site, 'and the requirement of storm
dra:na:e plans to D_RDrI for submittal, as rainwater was observed discharging into the sewer system via the

oillwater separator. On 12/8/99, a Cease &Desist Order was issued to the site to clean out the soakage pit
located on the east side of the site, as sludge containing amounts of oil and grease and petroleum
hydrocarbons was discovered via a DERM sludge sample analysis.As of 1/31100,the site had not submitted
documentation of the soakage pit cleanout. No further information is available in the files. DERM has not
required any assessment of the soils and/or groundwater of this site as a result of the above.
Based upon the ESA 2000 site inspection, and the regulatory review of the adjacent south property, ESA
recommends no further assessment for the subject site at this time.All underground structures (septic tank,
underground structure) have been apparently removed, and are no longer located on site. No violations exist
for the subject site at DERM. Based upon information contained in the Phase I completed for the site,the
site did not generate/store and/or dispose of industrial waste.Although the south adjacent property has had
numerous violations on site,DERM is not requiring soil and/or groundwater assessment, as impact appears
limited to sludges contained in the soakage pit.This soakage pit is located on the east side of the property,
and not located in the hydraulic upgradient of the subject site.
