A Resolution authorizing the City Manager to permit Hope 4 L.Y.F.E foundation to host its
second annual Hope 4 L.Y.F.E 3K Breast Cancer Walk and to waive all special event fees and
insurance requirements.
WHEREAS, the Hope 4 L.Y.F.E foundation (lithe applicant") has requested to host its second
annual Hope 4 L.Y.F.E 3K Breast Cancer Walk from 6:00 a.m. to 3:00 p.m. on Saturday, October 6, 2018.
This event will begin and end at the Gibson-Bethel Community Center 5800 SW 66 th Street South Miami,
FL 33143; and
WHEREAS, the walk will take place along the sidewalk through the CRA neighborhood, starting at
68th Street and east of 58th Place and will begin heading west, making a right turn on 58th Place, down to
66th Street, turning right to 57th Avenue and ending on 68 th Street; and
WHEREAS, after the walk, participants will return to the Gibson-Bethel Community Center for
music, educational exhibits and live entertainment including local speakers, cancer survivors, doctors and
more; and
WHEREAS, funds raised will be donated to Florida International University ("F.I.U") and will be
used towards the screening of breast cancer in men and women in the City of South Miami only, and who
may need further testing beyond F.I.U's Mammogram Van Services; and
WHEREAS, the applicant is requesting a waiver for all Special Event fees in the amount of $4,833.27,
as illustrated in the table below; and

WHEREAS, the applicant has completed the Special Event Application and has submitted the
necessary Notification/Petition Form signed by homeowners and businesses that will be affected by the
requested sidewalk closure.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF THE CITY OF
SOUTH MIAMI, FLORIDA THAT:
Section 1. The City Manager is hereby authorized to permit the applicant to hold its second annual
South Miami Hope 4 L.Y.F.E 3K Breast Cancer Walk and to waive insurance requirements.
Section 2. The City Manager is hereby authorized to waive all special event fees in the amount of
$4,833.27.

Section 3. If any section clause, sentence, or phrase ofthis resolution is for any reason held invalid
or unconstitutional by a court of competent jurisdiction, the holding shall not affect the validity of the
remaining portions of this resolution.
Section 4. Effective Date. This resolution shall become effective immediately upon adoption by
vote of the City Commission.

The Hope 4 L.Y.F.E foundation (lithe applicant") has requested to host its
second annual Hope 4 L.Y.F.E 3K Breast Cancer Walk from 6:00 a.m. to 3:00
p.m. on Saturday, October 6, 2018. This event will begin and end at the
Gibson-Bethel Community Center 5800 SW 66 th Street, South Miami, FL
33143.
The walk will take place along the sidewalk through the South Miami CRA
neighborhood, beginning at 68 th Street and east of 58 th Place and will head
west, making a right turn on 58 th Place, down to 66 th Street, turning right to
57 th Avenue and ending on 68 th Street. After the walk participants will
return to the Gibson-Bethel Community Center for music, educational
exhibits and live entertainment from local speakers, cancer survivors,

To register, participants will be charged: $25 for individuals, $15 for military
members or spouse, $5 for Youth (up to 21 years old with Student 10) or
$100 for a team of 5 participants. Funds raised will be donated to Florida
International University ("F.I.U") and will be used towards the screening of
breast cancer in men and women in the City of South Miami only, and those
who may require further testing beyond F.I.U's Mammogram Van Services.
The applicant has completed the special event application and has
submitted the necessary notification/petition approval form signed by

Hope 4 L.Y.F.E is a 501 C3 not-far-profit corporation. Our mission is to provide awareness and
treatment for women and men affected by breast cancer.
Hope 4 L. Y.F .E. 3K walk fundraiser was started by Angela Roberts-Oliver, restaurant owner of Chicks N'
Wings, located in South Miami Dade County. Owner, Angela Roberts·Oliver was compelled to launch this
initiative in 201 I, when Uris horrific disease struck home. Angela Roberts-Oliver, started Uris cause of
making a positive difference in the lives of women and men, by supporting and providing free
mammograms, wellness and breast cancer programs to educate, empower and restore. TIlls effort provides
the opportunity for uninSured residents in the South Miami area 10 have access for annual mammogram
screenings.
The SOMI Hope 4 L. Y.F.E. 3K Breast Cancer walk attendees will enjoy educational exhibits, exciting
entertainment, food and a kidz zone, which promotes empowerment and breast cancer awareness while
participating audiences will experience the joy of giving to others in need through a cause they love.
To ensure the success of the SOMI Hope 4 L. Y. F.E, 3K Breast Cancer walk, we are seeking partnerships,
fmancial support, and critical stakeholder's involvement to support and promote Uris vital and unique cause.

naming the event and the City of South Miami (6130 Sunset
Drive South Miami, FL 33143) as an additional insured, in the
amount of $1,000,000.
A $500 refundable security deposit, and other applicable
chargers such as, but not limited to, street closures, off-duty
police officers, etc. will be due and required before any permit
can be issued.

Hope for L YFE Somi Cancer Event
Route confirmed on attached map walk will begin at the community center gate 66street side
and will go north to 57 Ave and make a right easterly turn to 68 street to 58 place where the
walkers will go to 66 Street again. The route will continue until the 3 K mark is reached for 2 Y2.
times around and will end at the entrance gate of the Marshall Williamson Building.
• Measure off ~ quarter mile for Tents & Water Spots I First Aid stations
• Police and explorers will be at the Mobil gas station and the entrance of Red Road Commons Apartment for the
hour of the run with cones. barricade & police will provide yellow tape.
• Public Works will provide cones, Barricades
• Notification will be done by the Police Department & James McCants

8:00am, Event Ends 1:00pm
