Res No 130-17-14931RESOLUTION NO: 130-17 -1 4931 A Resolution authorizing the City Manager to negotiate and enter into a contract with H&R Paving, Inc., for road resurfacing work on selected streets within the City of South Miami. WHEREAS, the Mayor and City Commission wish to improve the roadway conditions throughout the City; and WHEREAS, a condition assessment was performed and resulted in the recommendation for seven locations throughout the City; and WHEREAS, pursuant to a competitive selection process, it was determined that H&R Paving, Inc., submitted a proposal in the amount of $78,940 that was the most comprehensive and cost effective in its construction approach; and WHEREAS, the City desires to provide a contingency of $7,800 over the proposal amount for unknown factors that may arise during the work; and WHEREAS, the total expenditure, including the contingency amount, is not to exceed $86,740; and WHEREAS, the Mayor and City Commission desire to authorize the City Manager to negotiate and enter into a contract with H&R Paving, Inc. for the citywide road resurfacing for a total amount not to exceed $86,740. NOW THEREFORE BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI, FLORIDA; Section 1. The City Manager is authorized to negotiate the price, terms and conditions and to execute a contract with H&R Paving, Inc. for citywide road resurfacing for an amount not to exceed $78,940 and he is authorized to expend up to $7,800 for unforeseen conditions. A copy of the approved form of contract is attached and the City Manager may negotiate a lower price and more advantageous terms and conditions if approved by the City Attorney. The expenditure shall be charged to the People's Transportation Plan Trust Fund account number 124-1730-541-6490, which has a balance of $1,346,682.88 before this request was made Section 2. Severability. If any section clause, sentence, or phrase of this resolution is for any reason held invalid or unconstitutional by a court of competent jurisdiction, the holding shall not affect the validity of the remaining portions of this resolution Section 3. In response to these concerns, a ro~dway assessment condition was performed. The results of this assessment resulted in the recommendation for Road Resurfacing at the locations listed below: Location #1: SW 61ST (62 PL to 62 AVE); Location #2: SW,59 PL (74 ST to 76 5T); Location #3: 5W 76 TR (69 AVE to 67 AVE); Location #4: 5W 68 AVE (74 5T to 76 TR); Location #5: 5W 57th CT (78 5Tto 80 ST); Location #6: 5W 78 5T (SW 57 CT to SW 58 AVE); Location #7: 5W 68th C1 (75 TR to 74 ST); Location #8: 5W 75th 5T (68 CT to 6870 5W 75 ST) The City received ten proposals in response to a solicitation. Pursuant to review, it was determined that H&R Paving Inc., is the most responsive and responsible bidder for this proposal. General Requirements: The work specified in this Request for Proposal (RFP) shall consists of furnishing all goods, materials, supplies and services to perform the work described in this RFP. The work is to be performed per specifications and the contract documents. This includes, but is not limited to the furnishing all labor of Contractor and the labor of all allowable subcontractors, and cost of disposal of materials as well as all necessary machinery, tools, transportation, equipment rental and permits, to perform all of the Work described in this RFP and which is necessary to provide a completed project that meets all of the needs described in this Scope of Services. Prior to any digging, Respondents must locate all underground utilities and other facilities as well as contacting Sunshine 81 I to coordinate the process between excavators and member utilities in Florida so that they can mark the approximate location of underground lines, pipes and cables on construction site. Permit fees are waived for permits required to be issued directly by the City of South Miami. Permit fees from other government entities, if required, shall be the responsibility of the Respondent/Contractor however, in all cases; it is the responsibility of Respondents/Contractors to secure any and all permits that may be required for this project. The work associated with this project should be reflected in the Respondents proposal and Exhibit 3, "Construction Bid Form, and the "Schedule of Values, Exhibit # I, "Scope of Services," Attachment B. II. Scope of Work: Scope of work involves performing road resurfacing work on selected streets within the City of South Miami and shall comply with applicable standards, including but not limited to the FDOT and the Miami Dade County Public Works standards. Work covered under this contract shall also include and is not limited to maintenance of traffic, off duty Police officer, milling, clearing & grubbing, asphalt placement, and striping. Scope of work involves performing Itt milling and Itt resurfacing with Type S-III asphaltic concrete at the locations described below. All striping shall be thermoplastic paint. Temporary paint shall be included within the thermoplastic paint item. Thermoplastic paint shall be installed 30 calendar days after substantial completion. Thomas F. ' Off duty Police officers will be required for lane closures. An allowance will be proVided for 160 hours of off duty Police officers. The off-duty Police officer hours item is an estimate. Selected contractor will be required, to apply for a right of way permit, during , which time Police will evaluate the MOT plan and assess if an off-duty police officer will be required. If the contractor believes that more than 160 hours are required for this project, the contractor shall adjust the quantities and total cost for this item in the proposal. If the number of hours required is less than the proposed number of hours, the difference shall be credited to the City.
