FIRM HISTORYCalvin, Giordano & Associates, Inc. (CGA) is a multi-disciplinary firm that began as a small two person-surveying firm in Florida over 76 years ago. From 1937 to 2013 we have grown to over 200 em-ployees, expanded geographically with offices in Fort Lauderdale, West Palm Beach, Port St. Lucie, Homestead, Clearwater, Jacksonville and Atlanta, Georgia. They are educators, community leaders, activists, volunteers, artists, designers, sculptors and LEED Accredited Professionals. This mix of talent allows us to provide our clients with holistic, well-round-ed design solutions that, beyond considering all the functional and implementation-related issues need-ed to be resolved, ensure a design product that, once built, will provide for longevity, experience, value and unique identities essential to establishing com-munity branding. This is supported by our qualified, award-winning staff, including Registered Land-scape Architects with more than 60 years of collec-tive professional experience and by a strong team of supporting experts. Streets/RoadwaysCGA’s roadway design experience ranges from small, two-lane rural roadways to large multi-lane highways and interstates. We work with the FDOT (Districts Iv and vI) on re-grading, striping, drainage improve-ments, implementation of ADA requirements, turn-lane additions, widening, and median modifications, and public information/participation. Facilities (Land Development)CGA has performed engineering services for a num-ber of parks throughout South Florida. This has involved the design of drainage and under-drain systems, grading of sports fields, parking lot/pedes-trian/sports field lighting, the design of parking lots, the extension of utilities to new or renovated struc-tures, and the overall storm water management sys-tem design and permitting. SurvEying anD MappingCGA’s team experience covers a broad spectrum from land surveying services such as boundary sur-veys, topographic surveys, tree surveys and hydro-graphic surveys to engineering design surveys such as route surveys, horizontal and vertical control sur-veys, construction layouts, utility surveys, as-built surveys and subsurface utility locations. Our team’s combined experience in working with state, munic-ipal and private-sector projects allows us the ability to integrate both agency and private-sector perspec-tive. CGA has four fully equipped survey crews with the potential of adding six more. Our firm is one of only a few survey companies to provide hydrograph-ic surveying and soft-dig subsurface utility locates. The Surveying and Mapping Division of CGA uses the latest in cutting edge technol-ogy when preparing surveys including GPS equipment, automat-ed data collection, laser scanners, un-derground utility de-tection technologies and a vacuum/slurry subsurface excavation system called the vaX-cavator that exposes existing underground utilities. Operated by one of the field survey crews, the VaXca-vator allows us to obtain subsurface horizontal and vertical locations required for design, while realiz-ing cost and time savings for our clients. Additional-ly, our state-of-the-art Hypack software with Odom sounder allows us to offer hydrographic surveying services. By integrating with our Trimble GPS units, the equipment creates a three-dimensional model of the water body being surveyed and significant sav-ings for our clients.
Diverse capabilities for a wide range of needsThe CGA Surveying and Mapping Division is equipped with staff who are best-in-class. Our staff consists of five professional surveyors and mappers, three CADD technicians and four survey crews that can perform the following:

Leica ScanStationSurvey reports require utmost detail in the least amount of time, which could typically take 1-2 weeks, possibly requiring several visits to a site. With the CGA Surveying and Mapping Division and the Leica ScanStation, survey scanning is significant-ly faster and can reduce the time by up to 50 percent.Recognizing the value to our clients, CGA was among the first firms in Florida to acquire the ScanStation. Our staff can now scan virtually anything in half the time of conventional methods, with a level of detail never before available. The ScanStation can take a virtual, detailed snapshot of a site including ceilings, undersides of bridges, elevated pipe racks, tall fa-cades, columns and towers — areas that were pre-viously difficult to access. It collects millions of data points of all items in a 360° horizontal and 270 ver-tical field of view, allowing a full field of view. This eliminates an additional visit to capture data not covered in the original scope of work or the question of where a shot was taken on an object. The ScanSta-tion captures all the items at once in a permanent record that can be filed, including hundreds of high quality photos, and graphics or animations, from 3D to virtual walk-throughs of sites - ideal for litiga-tions, client presentations or commission meetings. For added convenience, the survey data can be ex-ported to whatever format our clients need, whether CADD or other software.
It’s Safety First with the ScanStationIn addition to the highest quality survey data, the ScanStation has the surveyor’s best interest in mind. Because data is collected entirely by the scanner, it eliminates the need for a rodman to walk around a survey site. Surveying is now unobtrusive to motor-ists, construction workers, local employees and pe-destrian traffic.

In August 2012, Calvin, Giordano & Associates was selected by the City of Fort Lauderdale to implement a FDOT Broward Highway Beautification Grant to significantly enhance the landscaping in the medians on SR A1A/SE 17th Street Causeway. The project spans 1.6 miles along SE 17th Street from US1 to Mayan Drive – just east of the Intra-coastal Waterway. The emphasis of the planting design is on very large royal palms, “medjool” date palms, and coconut palms. These new palms will make a bold statement and have a huge impact on this busy thoroughfare. The design of the landscape shall also have an impact on the economic vitality of this important business corridor and enhance the area’s attractiveness to both tourists and residents alike. The CGA team provided site analysis, landscape design and irrigation design as well as maintenance of traffic plans for the project. Being that the landscape improvements are located on a state roadway. The design plans and construction drawings had to be prepared to meet the requirements of FDOT for permitting. CGA also assisted the City of Fort Lauderdale’s staff with preparation of the Maintenance agreements and the bid documents. Construction is set to begin in late spring/early summer of 2013. 8

The City of Weston received an Energy Efficiency & Conservation Block Grant and initiated a program that would result in the most effective results in energy conservation. CGA developed a request for proposals for energy reduction devices for outdoor lighting systems. Proposals were received and evaluated. The results of the responses indicated that LED luminaires would provide the best solution for the City and CGA prepared bidding documents to replace the existing metal halide street lights with LED units throughout the City. When the contracted work is completed, 1,247 units will have been installed. The City anticipates a saving up to $160,000 a year in energy costs, and approximately $100,000 in maintenance expenses annually when compared with the previous metal-halide installation.

Patrick Figurella, PE Calvin, Giordano & Associates, Inc. worked closely with Town Staff to devise a strategy for improving the qualities and character of the Town’s west island’s neighborhood, something the Town had been struggling with for over five years prior to contracting CGA.CGA was successful in providing the necessary services to get public support for the project, design the roadway and landscape improvements, and oversee all aspects of the construction. The project consisted of engineering and landscape improvements that upgraded the residential streets in the form of milling and resurfacing of the roadways, lighting, planted and irrigated bulb-outs and signage throughout. While the scope of the project is similar to others in nature that CGA has performed, this project stands out as one where CGA’s heavy involvement in marketing, graphics and conflict resolution took center stage. These skills aided CGA in facilitating approval of the final design strategy. CGA’s experience in negotiating discussions to arrive at compromises proved to be the winning strategy, coupled with their excellence in providing comprehensive services to deliver a complete project.

Jeffery Maxwell, PE, PTOE As the Town’s traffic engineering consultant, Calvin, Giordano & Associates, Inc. conducted a town-wide traffic analysis to determine appropriate neighborhood traffic mitigation measures. The analysis included conversion of two-way stop control intersections to four-way stop control, installation of speed tables and the conversion of two-way roadway segments to one-way operation. The analysis included the development of a comprehensive traffic flow model to determine the impacts of proposed traffic calming measures. CGA also provided extensive public outreach and coordination between citizen groups and Town staff.

CGA performs all aspects of signal design, signal timing and coordination, permitting and related activities on behalf of the City of Weston for all signalized intersections throughout the City. These services have included the performance of signal warrant studies, interim traffic control measures, signal design, signal timing, preparation of construction documents, bidding activities, agency coordination and inspections. Our staff has been instrumental in maintaining special aesthetic mast-arm signal designs throughout the City, and has adopted ordinances supporting aesthetic traffic control devices within the public right-of-way. The above photographs capture the final phase of a "turn-key" installation project, which was the joint effort between private developers, the City of Weston, and the Broward County Traffic Engineering Division.

Calvin, Giordano & Associates, Inc. prepared a comprehensive Bicycle Master Plan for the City of Weston that was adopted by the City Commission in January 2013. The Bicycle Master Plan included an extensive inventory of existing facilities and infrastructure as well as analysis of existing and future bicycling Levels of Service. CGA worked closely with Weston residents and stakeholders in developing the Bicycle Master Plan and assisted the City in forming the Weston Bike Alliance to facilitate ongoing communication between the bicycling community and City officials. In addition to preparing the Bicycle Master Plan, CGA assisted the City in applying to the League of American Bicyclists for designation as a Bicycle Friendly Community.

In response to a need to perform traffic calming and provide greater safety in proximity to the new Joe DiMaggio Children’s Hospital, CGA designed all components for a road closure and roundabout circle at the intersection of Taft Street and 40th Avenue in the City of Hollywood. Among the services provided by CGA were Highway Design, traffic engineering, landscape architecture, electrical engineering, irrigation design, and construction administration. In addition, the signalized intersection at Johnson St. and N. Park Rd. was widened and realigned to accommodate future additional turn lanes. Johnson Street from N. 41st Ave. for about 850 ft to the east was widened and N. 40th Ave was widened for a distance of about 300 ft. to accommodate the addition of a left turn lane from Johnson St. to 40th Ave. The intersection of N. 38th Ave. at Johnson St. was closed with a street termination turnaround created 95 ft. to the north. Drainage modifications, utility accommodations and driveway connection retrofit were all part of these modifications.

CGA completed an intersection safety analysis in accordance with FDOT standards and we were successful in obtaining grant funding for construction of the intersection through the FDOT Local Agency Program (LAP). The City was awarded a grant in the amount of $467,000 as a result
of the CGA traffic safety analysis.

Street and Pedestrian Scale Lighting The CGA Team reviewed existing lighting and made recommendations for installation of lighting to fit the City of Miami Springs theme and provide pedestrian-scale light that enhances the safety of Miami Springs’ residents and business customers. The scope of services included performing a field investigation of the existing street lighting system and the lighting power distribution and control system, researching the roadway classification and the code requirements, and recommendations for the specific illumination levels, poles and luminaries.
Wayfinding and Signage Program The goal of the project was to design a clear and safe set of signs with a consistent and visual identity from beginning to end to enhance the visitor’s experience by easily directing them to places they would like to visit. The scope of services included an analysis of the current signs in the community directing visitors to locations, meeting with community representatives and recommendations about the best type of sign (by community/area or activity (shopping, cultural and civic, recreational), the number of signs required and sign locations.
Westward Drive Traffic Circulation and Parking Study CGA conducted an arterial qualitative assessment analysis in the business district of Westward Drive. The review included data collection and analysis of access management, high conflict locations, and turning movements Recommendations were based upon consideration of all relevant corridor elements and were directed at improving access, circulation, travel time delays, stops, motorist safety, and fuel consumption. The study’s ultimate goal was to improve mobility on Westward Drive in order to improve access to Downtown Miami Springs.

The City of Pompano Beach Community Redevelopment Agency (CRA) requested Calvin, Giordano & Associates, Inc. to develop a comprehensive traffic analysis for the Downtown Pompano Beach Area where a 250 room hotel, 120,000 square feet of office, 30,000 square feet of retail and a Civic Campus are proposed. Eighteen (18) intersections and Forty-one (41) roadway segments were evaluated in the Downtown area of Pompano Beach. This study developed a strategic parking plan along Martin Luther King Boulevard that included separate parking utilization techniques during the peak and off-peak periods to encourage business growth and increase pedestrian friendliness along the corridor. Also, this study made recommendations for the conversion of a segment of Flagler Avenue from a 2-way to 1-way roadway and analyzed pedestrian connectivity between the proposed FEC Railway Station, proposed retail, office, hotel and Civic Campus. In addition, a preliminary feasibility analysis was performed at the I-95/Atlantic Boulevard interchange to determine if the extension of NW 9th Avenue to Atlantic Boulevard could provide direct access from northbound I-95 to the redevelopment area, without placing traffic on Atlantic Boulevard. This interchange improvement would provide extensive relief to NW 6th Avenue, between Atlantic Boulevard and Martin Luther King Boulevard.

A beautification project at the downtown West Palm Beach Tri-Rail station was completed in July of 2012. The project’s goal was to improve the station on Tamarind Avenue, and make it safer and more “rider-friendly.” The project was paid for by a $750,000 grant from the Florida Department of Transportation and addressed many upgrades to the aesthetic additions to the beautiful 1920 historic station which was part of the Seaboard Railways which later became a Tri-Rail station in 1989. “One of the main goals of the city was to focus on sustainability by encouraging public transportation as much as possible. The project also improved the access for the Palm Tran buses, the downtown trolley and the “Kiss & Ride” carpool as it facilitated the pick-up and drop-off of passengers. New sidewalks with a shell texture finish and landscaping were the dominant improvements that were done with a native planting installation with 70 trees installed. Three “S-shaped” seating walls were also added to accommodate the increasing number of Tri-Rail riders and students using the train to commute. Calvin, Giordano provided the construction engineering inspection for the hardscape, landscape and irrigation for this prominent project.

• SR 5 / US 1 at Virginia Avenue The purpose of this study was to evaluate the conditions of intersections with respect to pedestrian safety and to identify opportunities for improvements, which integrate walking. This study was initiated as a follow-up to the FDOT programmed triple left-hand turn lane for the intersection of Port St. Lucie Boulevard and US-1 (#4231151), which did not include pedestrian improvements as part of the project. This study evaluated pedestrian levels of service by means of space and delay at the signalized intersections. The corridor of US 1 was also evaluated based on FDOT’s ARTPLAN, which evaluates pedestrian activity based on sidewalk existence, traffic volumes, free flow speed, sidewalk separation and roadway median type. CGA also provided extensive public outreach and presentations to the TAC and CAC.

Robert Roush, PE Calvin, Giordano & Associates, Inc. was contracted by the City of Miami Gardens to enhance the existing landscaping on N.W. 183rd Street from N.W. 27th Avenue to N.W. 47th Avenue. This 1.78 mile long corridor consisted only of some trees and grass in the medians and frontage road swales. The condition of the existing landscaping was poor due to the fact that no irrigation system had been installed with the original project. One of the main ideas for the design of this project was to work with the existing trees that were in good condition. The City wanted to keep the look of the landscaping the same as other parts of the roadway system within the City. The plans called for the removal of only those trees that posed a conflict with sight visibility, and also called for the relocation of a few specimen quality trees to new areas within the medians. In addition, the City requested that paver bricks be added to the ends of some the medians. This again was consistent with other roadway projects in the City. Perhaps, the most important element of the project is the installation of an automatic, underground irrigation system. This has made a huge difference in the establishment of the new plants and grass, and will allow the City to keep the landscaping in this roadway corridor looking good for many years to come. Recently, the City of Miami Gardens received “The Project of the Year” award for the NW 183rd Street – Phase II Beautification Project from the American Public Works Association – South Florida Chapter.

The intent of the project is to update the look and experience of the concourse, consistent with the vision outlines in their adopted master plan, without compromising the integrity of the corridor’s importance as one of the few remaining stretches to boast predominantly well-preserved MiMo buildings. The project’s scope includes paving design, landscape planting improvements, lighting and street furnishings. The project is presently in the permitting phase, and CGA will be coordinating the Town’s improvements with FDOT’s plans to provide improvements on the corridor’s roadway. The project was funded through the LAP process, for which CGA provided all administrative services.

Enhanced by native trees and shrubs and carpeted by pervious pavers, the new public parking lot between S.W. 5th and S.W. 4th Avenues is resolving parking woes in the West Neighborhood CRA district of downtown Delray Beach. This design is also reducing pollution, energy costs and emission of greenhouse gases. The 24-space lot just a few lots South of Atlantic Avenue is a “green design,” consisting of pervious pavers, decorative LED lighting, and low-volume irrigation, according to the Delray Beach Community Redevelopment Agency (CRA) which built the lot. Delray Beach is on the cutting edge of sustainable solutions with building green parking lots to reduce water runoff and protect waterways from dirt and petroleum that come from standard water-shedding, heat-reflecting pavement lots. The U.S. Environmental Protection Agency last year began investigating new ways to further reduce pollution runoff from paved surfaces. Pollution from those surfaces put undo stress on water infrastructure, especially in dense, populated urban areas, the agency said. The parking area covers two blocks which includes a “zig-zag” design for traffic calming and to discourage cut through traffic. The design includes trash receptacles and a bike rack and a vacant parcel on the East lot for future CRA facilities. On S.W. 5th Avenue the artful terrazzo paving design has been preserved and enhanced by the addition of the pervious pavers and the native planting layout.

(CGA) provided complete traffic analysis, construction documents, permitting and construction management for the conversion of the four-way stop controlled intersection of Saddle Club Road and Bonaventure Boulevard to a modern roundabout. Traffic analysis included an animated simulation of the roundabout operation in real time which was beneficial during public outreach meetings in allowing the public to visualize how the intersection would operate once converted to a roundabout. All aspects of the roundabout design was completed by CGA’s in-house staff and included complete permitting through Broward County Traffic Engineering Division. CGA developed final construction documents as well as the complete bid package and assisted the City in obtaining project bids. CGA’s in-house construction management team provided construction oversight through final sign-off by Broward County. CGA completed an intersection safety analysis in accordance with FDOT standards and we were successful in obtaining grant funding for construction of the intersection through the FDOT Local Agency Program (LAP).

The firm maintains a workload of dozens of projects at any given time. CGA’s system of “look aheads” tracks current, future and projected work-load on a weekly basis. Within the understanding of scope, a list of the additional personnel assigned to the proj-ect and his or her particular role will be included for City approval. The qualifications of the personnel to work on that particular project will be readily avail-able. The understanding of project scope and prelim-inary project time-line will be provided to the City for review and approval prior to the CGA internal project “Kick-off Meeting”. This “Kick-off Meeting” will occur after the initial project award meeting be-tween City staff and the CGA project manager. The Project Manager will also be responsible for assigning tasks to the various departments within CGA and also to any applicable sub-consultants. At the “Kick-off Meeting”, the team will be introduced to the scope, the budget and the time-line. The Project Manager will define the various elements of the proj-ects and describe the quality control/quality assur-ance requirements at this meeting. The team’s Proj-ect Manager will periodically review the progress of the project to assure meeting the project time-line and budget as initially approved by the City. The Project Manager will then update City Staff of the project progress.
Regular project meetings will be held with City staff for all phases of the project. The CGA attendees pres-ent at the meetings will be dependent on the com-plexity and the phase of the project. These meetings will be focused on adherence to the project timel-ine, quality control, budget, and on the resolution of any outstanding issues. Close attention to the budgetary constraints of available funding will be a primary focus of the CGA team. In addition, value engineering analyses will play a critical role in insuring the most efficient de-sign possible. No work will be added to the project without review and agreement by both the City and the CGA project manager; such review will include cost and time impacts in order to identify alternative solutions which can maintain the project’s budget. An initial review of the long range estimate will be conducted as one of the first work items under this contract. verifying the estimated work against the agreed upon project scope will establish the base-line cost for construction. Just as in the design con-trols stated above, no additions to the scope will be made without a review by the City and the CGA proj-ect manager.CGA’s project management approach makes quality control an integral part of the project. Thorough and diligent quality control has a direct effect on keep-ing the project on time and on budget. The quality control concept is a major agenda item at the kickoff meeting with the design team. All team meetings will address the quality by measuring progress against the City’s expectations. The team will meet all prod-uct requirements, technical, budget, and schedule or take immediate corrective action to bring the project back in line. Weekly progress meetings are supplemented by a peer review, where a design group not assigned to the particular assignment will review the plans with a fresh perspective. This quality review is performed as part of our quality assurance program. CGA has instituted the policy of a multi-disciplined review of the product before it is released to the client to en-sure adherence to the big picture and to counteract any potential lack of congruence in the overall prod-uct.

In addition, the CGA team will review the project from a “Constructability” of design perspective in order to meet the City’s goals and objectives and to maintain a firm grasp on the costs associated with each project. The “Constructability” of a design is an important facet to consider during each phase of a design’s development; however, it is most poi-gnant when a design is nearing substantial comple-tion (90% plans), where it undergoes an in-house constructability review by our CEI and Construction Management Departments. This review specifically ensures potential problems in the field during con-struction are evaded or minimized.WILLINGNESS TO MEET TIMELINES AND BUDGET REQUIREMENTSThe CGA Team constantly strives to deliver a high quality product to our client at or before the sched-uled deadline. CGA understands that with the cur-rent economic conditions it has become ever more important to deliver capital projects ahead of time and under budget. Carrying Capacity Traffic Study, Key West, Florida; Under Mr. Maxwell’s direction, CGA recently completed a city-wide carrying capacity traffic study for the City of Key West to determine the impacts of traditional and non-traditional modes of transportation on the overall roadway network. The project involved extensive public involvement and Mr. Maxwell was instrumental in coordinating numerous public outreach meetings. Public feedback was sought through several outlets including the use of hand-held voting devices and online surveys.
Downtown Pompano Corridor Study, Pompano Beach, Florida; Under Mr. Maxwell’s direction, CGA recently worked with the City of Pompano Beach Community Redevelopment Agency (CRA) to develop a comprehensive transportation analysis for the Downtown Pompano Beach Area redevelopment project. The City envisioned a complete revitalization of the economically challenged area that would include hotel, office and commercial uses as well as a Civic Campus. CGA developed a strategic parking plan along Martin Luther King Boulevard that included separate parking utilization techniques during the peak and off-peak periods to encourage business growth and increase pedestrian friendliness along the corridor. The study included recommendations for the conversion of a segment of Flagler Avenue from a 2-way roadway to a 1-way roadway and analyzed pedestrian connectivity between the proposed FEC Railway Station and proposed retail, office, hotel and Civic Campus areas.

The signal warrant analysis was prepared in accordance with the signal warrant criteria set forth in the Manual on Uniform Control Devices. The analysis consisted of a field review, traffic data collection, and the review of crash history. The analysis concluded that 4 of the 9 signal warrants were met, therefore recommendation of a traffic signal was to be installed.
Cypress Key Town Centre Royal Palm Beach FL; Signal warrant analysis and signalization plans involving the projection of anticipated traffic volumes based on the proposed plan of development. Construction plan included design for new sidewalks, curbing, median modifications, and turn lane modifications.[2]
Florida Department of Transportation District IV Traffic Operations Contract, FL; Project included five counties and involved work orders for traffic operation studies such as signal warrant studies, conflict analysis studies, speed studies, corridor studies, interchange studies, pedestrian studies, and signal design.[3]
Quillen Development of Regional Impact, Indiantown, FL; Detailed traffic analysis of the surrounding roadway network impacted by the proposed plan of development of 2250 residential dwelling units, 150,000 square feet of retail area, and a 15 acre private school facility. Project also included proportionate fair share cost calculations for all required roadway improvements. [2]
Florida’s Turnpike and Southern Boulevard (State Road 80) Interchange, FL; Interchange Modification Report that involved preparing multiple simulation models of possible improvements to Florida’s Turnpike and State Road 80 Interchange. Projected traffic volumes ranged from the existing year to the year 2030. [3]
US Highway 1, Monroe County FL; Travel Time Study involving two weeks of data collection including recording no passing zones, types of delays, and locations of delays. A final report was produced summarizing the levels of service based on the calculated vehicular travel speeds.[3]
City of Fort Myers City Wide Traffic Calming, FL; The project involved the development of a traffic calming plan for each of the five (5) wards in the City of Fort Myers. Data collection analysis included speed data, traffic volume data, and origin destination data; the data was then presented to City staff and residents with the recommended traffic calming improvements. State Road 80 Access Management Plan, Fort Myers FL: A traffic study was prepared to improve aesthetics and safety of pedestrians and motorists traveling the area. The project involved the planning process for the development of an access management plan to implement a raised median along a five-lane roadway. The raised median is replacing the two-way left turn lane. This was a locally administered project on a state owned and maintained highway that required close coordination with the Florida Department of Transportation District I representatives. The study included traffic data collection, pedestrian analysis, accident analysis, coordination with City of Fort Myers Staff, and community involvement with business owners and residents. [4]
Connector Road, Auburn NY: Locally Administered project involving the construction of a new two mile road connection from North Street to Grant Avenue. Responsibilities include traffic capacity analysis utilizing Synchro, traffic signal design, and maintenance & protection of traffic.

His primary areas of expertise are highway design, signalization design, work zone traffic, quality control and project management. Mr. Pippitt served in the capacity of Deputy District Design Engineer in the Florida Department of Transportation’s District 4 office for 10 years. His responsibilities included managing the District’s Quality Control section, acting as the District Pavement Design Engineer, District Estimates Engineer and District Specifications Engineer. He also served an additional 9 years as a project manager with FDOT. Prior to joining FDOT he served in the capacity of project manager for the Bureau of Local Federal Aid and Safety Programs for the New Jersey Department of Transportation for 10 years.

The project included addition of bicycle lanes, widening resurfacing, improvements to the closed drainage system, stamped asphalt parking areas, new mast arm school flashers and the addition of a roundabout intersection.
Lauderdale Lakes Mast Arm Infrastructure: Acting as Project Manager and Engineer of Record was in responsible charge of all design elements of the signalization of two intersections along SR-7/US-441. The two intersections were upgraded from span wire signals to mast arm signals designed to meet the new hurricane wind loading criteria of 150 mph. City of West Park, LAP Certification: Acting in the capacity of Director of Highway Design supervised the application process for the city’s LAP certification by the Florida Department of Transportation. I-75 at Arvida Parkway LAP project FDOT/City of Weston, Florida: Acting as Engineer of Record was responsible for the design. These improvements were designed in a cooperative effort between the FDOT and the City of Weston, utilizing federal, state and local funding to facilitate needed roadway improvements in the City. Paloma Lakes Traffic Signal, Lennar Homes: Acting as Project Manager and Engineer of Record was responsible of all design elements of the signalization of the intersection of Lyons Rd. and Wiles Rd, in Coconut Creek, Florida. The intersection were upgraded from span wire signals to mast arm signals designed to meet the new hurricane wind loading criteria of 150 mph. SUMMARY OF QUALIFICATIONS Ms. Martinetti has nine years of engineering experience and has worked on projects from inception to project close out including supporting documents and coordination between all involved parties. The city needed design, permitting and construction completed in 13 months in order to meet the deadline for two separate grants funding the project. The project also included shoreline restoration using sheet pile wall, environmental and stormwater permitting, site grading, grant administration, and landscaping improvements. Roadway Design Projects:
Joe DiMaggio Children’s Hospital Offsite improvements: Design and management of offsite improvements at 10 different locations surrounding the hospital associated with the construction of the Joe DiMaggio Children’s Hospital. Improvements included street closures, 2 new roundabouts, widening, adding turn lanes, signalization, realignment of the roadway, bus bays, and pedestrian crosswalks. These projects were closely coordinated with onsite hospital expansion, and require considerable utility and local agency coordination. City of Weston, Florida. Serves as the FEMA Administrator as a function of a multi-service, multi-year contract that CGA has with the City. SUMMARY OF QUALIFICATIONS Mr. Perez has 17 years of experience in the field of technology with 14 of these years focused on GIS technology and its applications. Mr. Perez manages the developmental procedures for all projects within the Data Technologies and Development Department at CGA. His responsibilities fall within the disciplines of GIS, Application Development, INKforce and web development. He also manages all Information Technology related projects at a corporate-wide level in addition to client related IT implementations. • Project Manager / Asset Inventory, Cutler Bay, Florida. Completed a storm water utility infrastructure Inventory from the ground up for the Town. The Town contracted CGA to locate storm water utility structures using tradition surveying methodology. The information collected in the field was converted and imported into GIS data layers. CGA also coordinated with the [insert client here]’s contractor to have each structure cleaned. Additional data was collected as each structure was cleaned by the Town’s contractor and CGA included these attributes in the data as well. The data was delivered to the Town as usable electronic map exhibits and GIS data layers. This project was completed under budget and within the allowable time frame.
• Project Manager / GIS Implementation, Weston, Florida. Project involves developing, updating, and maintaining all GIS coverages for the City. The coverages include, but are not limited to, City boundary, parcel base map, streets, addresses, utilities (water, sewer, and storm drainage), residential commission districts, business locations, parks, school boundaries, refuse pick-up, etc. CGA implemented these coverages into an interactive GIS Map Service that is accessible through the City’s official web site. • GIS Coordinator / Developed Survey Quality Parcel Base Map for Broward County Property Appraiser’s
Office, Lauderhill, Florida. Developed parcel line information detailed to the lot level. All recorded plats, metes and bounds descriptions and condominium parcels were entered using coordinate geometry to create a base map that was accurate within +/- .5 feet. The final product was delivered in an ArcINFO format and was appended to the Broward County Property Appraiser’s GIS parcel base map.
• Project Manager / Lauderhill Storm Water – Project Manager responsible for aspects of this project. Project involved conversion of all existing AutoCAD storm water data into several GIS datasets. All data were attributed from the as-built hardcopy drawings. Global Positioning System (GPS) was used to locate outfalls. The final product includes attributed catch basins, drain pipes, manholes, retention areas, and outfalls.
• Project Director / Weston Sign Inventory – served Liaison and Project Manager accountable for entire project. Assisted in coordination of data collection with field personnel. Responsible meeting all time lines and budgetary constraints. Ensured all deliverables met client expectations.

As part of this project, GMT was responsible for generating a map of land use throughout Puerto Rico in 2003. As part of the project, I was in charge of IR image processing to generate NDVI outputs, and then use them within an automated classification. Along with the Project Manager, I was involved in the design of the classification methodology and interpretation process, as well as in the quality control procedure. Also, I developed a set of geoprocessing models to automate many of the processes involved in the methodology ofinterpretation and classification of images.• Digitization and Measurement of External Plant Easements Project for the Puerto Rico Telephone
(GMT, Corp.)The purpose of this project was the digitization of maps and plans of telephone external plant and calculates the linear feet of telephone cable easement. In 2008, Mr. Jackson was tasked by the City of Weston to have the City become participants of the Community Rating System. The City contains over 27 square miles with boundaries on the west at the Everglades L-33 levy, on the north and east at I-75, and on the south at the C-11 canal/Griffin Road. Calvin, Giordano & Associates provides engineering, landscaping, planning, and construction services for multitude of projects within the City. A new City Hall and Public Works building, along with extensive roadway improvements, such as traffic signalization, and under grounding the electric power on State Road 84 are just a few of the recent projects. CGA has provided analysis, recommendations, and Capital Plan input on water, sanitary, and drainage infrastructure. Sanitary and water systems were recently modeled with computer software to help determine improvements necessary to increase efficiencies to the systems. Calvin, Giordano & Associates has been the City Engineer for Weston for more than a decade, and can state that it was one of the few communities that had power before, during and after the hurricane Wilma. In addition, our emergency team responded immediately to restore the Infrastructure to the same level of service.

BRIEF DESCRIPTION OF PROJECT AND RELEVANCE TO THIS CONTRACT (Include scope, size, and cost) CGA developed the Town of Surfside Storm water Management Master Plan to address water quality issues and reduce flooding within the Town. The Master Plan included a complete engineering analysis based on the ICPR computer models of the storm water management system. From a water quality perspective, CGA formulated the best approach to reduce or eliminate pollutant discharge loadings into Biscayne Bay from the Town’s system. In addition, the modeling targeted improvement in hydraulic performance of the Town’s drainage system. The Town’s storm water management system is divided into 9 drainage basins. Basins 1 thru 6 and Basin 8 were modeled with water quality improvements, as well as with basin interconnects for hydraulic improvement. The majority of the basin interconnects were modeled at the downstream end of the system to more evenly distribute the flow and to optimize the true discharge at each individual outfall. Control structures were installed upstream of all the outfalls to facilitate well discharge prior to overflow into Biscayne Bay through the outfalls. CGA also recommended that the Town consider the implementation of non-structural Best Management Practices, such as regularly scheduled street cleaning; inspections of commercial properties to ensure proper maintenance and storage of hazardous materials; and the functionality of wastewater and stormwater systems. Sediment removal devices and drainage wells require periodic inspections, and the drainage system requires proper maintenance for most efficient operation.

The projects included the design of two one-lane roundabouts and four left turn lanes, and are intended to improve traffic flow and safety along Bonaventure Boulevard; one of the roundabouts is a LAP safety project with FDOT District 4. The roundabout projects included intersection reconstruction and re-configuration of the approach roadways to reduce traffic to one through lane prior to the roundabout approaches. Maintenance of Traffic design was a critical component of these plans, due to the need to keep the intersections open to traffic throughout construction. Project components included roadway and drainage design, signing & pavement marking design, utility coordination, project permitting, landscape architecture & survey.

SBDD is bounded on the north by Sheridan Street and Griffin Road (SFWMD C-11 Canal), on the west by the Everglades Conservation Area, on the south by SFWMD C-9 Canal and the Florida Turnpike Extension and on the east by University Drive. As part of CGA’s ongoing contract with SBDD since 1996, CGA performs analysis, and updates storm water Master Plan, which includes the entire drainage district consisting of seven primary storm water pump stations, approximately 44 miles of freshwater canals, numerous lakes, culverts and control structures within its boundaries.

This phase involves the upgrade of roadway form SW 30th Street to Nova Drive from a two lane rural section to a four lane major collector with eight foot wide walks, bike lanes and planted medians in an educational campus environment. The proposed improvements will created a safer driving environment by providing safer turning opportunities as well as a safer environment for pedestrians and bicyclists by providing designated uses for each. The project involves extensive coordination, interaction and communication with the local community including Broward College, Nova Southeastern University, Florida Atlantic University, and the University of Florida as well as local residents and the student population. This addendum submission is issued to clarify, supplement and/or modify the previously issued
Request for Qualifications (RFQ) Documents, and is hereby made part of the Documents. All
requirements of the Documents not modified herein shall remain in full force and effect as
originally set forth. It shall be the sole responsibility of the bidder to secure Addendums that
may be issued for a specific solicitation.
Question #1:
Have you issued an addendum regarding that RFQ yet?
Answer to Question #1:
No, this is the first Addendum.
Question #2:
Can we use Standard Form 330’s in lieu of Standard Forms 255?
Answer to Question #2:
Proposer may submit SF330, Part 1 & II with their proposal. A SF255 is not required. Form
SF 330 should be included under “Submission of Responses to RFQ; Section III
Supportive Information.”
Question #3:
Attachment #4 “Acknowledgement and Conformance with OSHA Standards references the
Janitorial RFP. Please confirm that this form is not required for this submittal.

Answer to Question #3:
Proposers are to complete Attachment #4 and submit with their proposal. However, there is
a typo in Attachment #4 included with the RFQ which identifies the solicitation as “ Janitorial
Services RFP.” Enclosed with Addendum #1, is a corrected version of “Attachment #4” with a
revision date of 4/18/2013. Proposer may submit the revised copy or Attachment #4 from the
RFQ. If using the RFQ version, cross out Janitorial Services RFP and replace with “RFQ
#PW-S2013-10” and, initial the change.
Question #4:
Can the most recent two (2) years of unaudited financials be provided in a separate sealed
envelope?
Answer to Question #4:
Yes, financials may be submitted in a separate sealed envelope.
IT SHALL BE THE SOLE RESPONSIBILITY OF THE BIDDER TO SECURE
ADDENDUMS THAT MAY BE ISSUED FOR A SPECIFIC SOLICITATION.
