BIDDERS shall use the Bid Form(s) furnished by the City. Failure to do so may cause the Bid to be rejected. Removal of any of the Bid Forms may invalidate the Bid. BIDDER shall deliver to the City, as Bid package, the following documents: a) The Invitation to Bid and Instructions to Bidders. b) A copy of all issued addenda. c) Four (4) copies of the Bid Form completely executed. d) Bid Bond is Waived e) Certificates of Competencies, State, County and City Licenses. f) Certificate of Insurance and /or Letter of Insurability. g) Performance and Payment Bond is required in accordance with Sec. 17 -6 of the City's Ordinance h) Proposal submittals shall include an electronic copy, either CD or flash drive, in addition to one original hard copy and three copies of the complete bid package. The entire Bid Package shall be placed in an opaque envelope and clearly marked with the BIDDER'S name and "SEALED BID PINECREST VILLAS NIEGHBORHOOD TRAFFIC CALMING IMPROVEMENTS PHASE I." 1 Bids having an erasure or correction must be initialed by the BIDDER in ink at the location of each and every erasure or correction. Bids shall be signed in ink; all quotations shall be typewritten or printed and filled in with ink. 7. Mistake: In the event that unit prices are part of the Bid and if there is a discrepancy between the unit price(s) and the extended price(s), the unit price(s) shall prevail and the extension(s) shall be adjusted to coincide. BIDDERS are responsible for checking their calculations. Failure to do so shall be at the BIDDER'S risk, and errors shall not release the BIDDER from his /her or its responsibility as noted herein. 8. Brand Names: If a brand name, make, manufacturer's trade name, or vendor catalog number is mentioned, whether or not followed by the words "approved equal ", it is for the purpose of establishing a grade or quality of material only. BIDDER may offer equals with appropriate identification, samples and /or specifications on such item(s). The City shall be the sole judge concerning the merits of items Bid as equals. 9. Pricing: Prices should be stated in units of quantity specified in the Bid Form. In case of a discrepancy, the City reserves the right to make the final determination at the lowest net cost to the City. 10. Safety Standards: The BIDDER warrants that the product(s) to be supplied to the City conform in all respects to the standards set forth in the Occupational Safety and Health Act (OSHA) and its amendments. Bids must be accompanied by a Materials Data Safety Sheet (M.S.D.S) when applicable. 11. Liability, Insurance, Licenses & Permits: Where BIDDERS are required to enter onto City of South Miami property to deliver materials or to perform work or services as a result of a Bid Award, the BIDDER shall assume the full duty, obligation, and expense of obtaining all necessary licenses, permits, inspections, and insurance required. Section 313.24 Chevron and Diagonal Crosshatch Markings Option: 01 Chevron and diagonal crosshatch markings may be used to discourage travel on certain paved areas, such as shoulders, gore areas, flush median areas between solid double yellow center line markings or between white channelizing lines approaching obstructions in the roadway (see Section 3B.10 and Figure 3B -15), between solid double yellow center line markings forming flush medians or channelized travel paths at intersections (see Figures 3B -2 and 313-5), buffer spaces between preferential lanes and general- purpose lanes (see Figures 3ll -2 and 3D -4), and at grade crossings (see Part 8). Standard: 02 When crosshatch markings are used in paved areas that separate traffic flows in the same general direction, they shall be white and they shall be shaped as chevron markings, with the point of each chevron facing toward approaching traffic, as shown in Figure 313-8, Drawing A of Figure 313-9, Figure 311-10, and Drawing C of Figure 3B -15. 03 When crosshatch markings are used in paved areas that separate opposing directions of traffic, they shall be yellow diagonal markings that slant away from traffic in the adjacent travel lanes, as shown in Figures 313-2 and 313-5 and Drawings A and B of Figure 313-15. 04 When crosshatch markings are used on paved shoulders, they shall be diagonal markings that slant away from traffic in the adjacent travel lane. The diagonal markings shall be yellow when used on the left -hand shoulders of the roadways of divided highways and on the left -hand shoulders of one -way streets or ramps. The diagonal markings shall be white when used on right -hand shoulders. Guidance: 05 The chevrons and diagonal lines used for crosshatch markings should be at least 12 inches wide for roadways leaving a posted or statutory speed limit of 45 mph or greater, and at least 8 inches wide for roadways leaving posted or statutory speed limit of less than 45 mph. The longitudinal spacing of the chevrons or diagonal lines should be determined by engineering judgment considering factors such as speeds and desired visual impacts. The chevrons and diagonal lines should, form an angle of approximately 30 to 45 degrees with the longitudinal lines that they intersect.
