NOTICE OF ACCEPTANCE:.STANDARD CONDITIONS 1.Renewal ofthis Accqptance (approval)shall be considered aftera renewal application has been filed and the original submitted documents^including test supporting data,engineering documents,are noolderthaneight(8)years. 2.Anyand all approved products shallbe permanently labeled withthe manufacturer's name,city,state,and the following statement:"DadeCounty Product Control Approval",oras specifically stated inthespecific conditions of this Acceptance. 3.Renewals of Acceptance will notbe considered if: a)TherehasbeenachangeintheSouth Florida BuildingCodeaffectingtheevaluation ofthisproductandthe product isnotin compliance withthecode changes; b)Theproductisnolonger Che same product (identical)astheone originally approved; c)Ifthe Acceptance holderhasnot complied with alt the requirements ofthis acceptance,including the correct installation oftheproduct; d)The engineer who originally prepared,signed and sealed the required documentation initially submitted,isno longer practicing theengineering profession. 4.Any revision or change inthe materials,use,and/or manufacture of the product or process shall automatically be cause for termination ofthis Acceptance,unless prior written approval has been requested (through the filing ofa revision application with appropriate fee)and granted bythisoffice. 5.Any ofthefollowingshallalsobe grounds for removal ofthis Acceptance: a)Unsatisfactory performanceofthis product or process; b)Misuse ofthisAcceptanceasanendorsementof anyproduct,for sates,advertisingoranyother purposes. 6.The Notice of Acceptance number preceded bythe words Dade County.Florida,and followed bythe expiration date maybe displayed inadvertising literature.If any portion ofthe Notice of Acceptanceisdisplayed,thenit shall be donein its entirety. 7.Acopy ofthis Acceptance as well as approved drawings andotherdocuments,whereit applies,shallbe provided to theuserbythe manufacturer orits distributors and shall be available for inspection atthejobsiteatalltime.The copiesneednotbe reseated bythe engineer. 8.Failure tocomplywithanysection ofthis Acceptance shall be cause for termination andremoval of Acceptance. 9.This Acceptance contains pages 1,2,2(a)through 2(e)and this last page 3. Items 10.11 A 12 listedbelowonlyapply to plated nrodueft *nrf door? 10.Unless specifically indicated inthe Acceptance (approval),this unit h approved asa single unit installation.For multiple installation ofthis unit,a separate Acceptance for mullions is required from the Product Control Section. 11.The spacing of fasteners atwindowsills shall beas tested.The spacing of fasteners inallother parts ofthe frame, shallbeas tested,but innocaseshall exceed 24"oncenter.Thefirst fastener shallbelocatedata maximum of6" from each corner and mullion or stile.Fastener shall fully penetrate the buck,which shall bethesamesizeastheone tested withtheunit.Nowoodor plastic shields or pins shall be used for anchoring.Sec section four (4)ofthis approval forsizes of fasteners usedin testing. 12.Hardware for all windows and doors shall conform to Security and Forced Entry Prevention,Chapter 36 of Ihc South Florida Building Code.
