McKinley Insurance Services is an independent, full- service, all -lines insurance agency that is locally headquartered in Fort Lauderdale, Florida, and maintains an office in Orlando, Florida. Jim McKinley founded McKinley Financial Services Inc. dba McKinley Insurance Services, Inc, in 1987 as a one - person operation. Since then, the firm has grown exponentially to become the largest minority -owned insurance agency in the United States. Since writing its first policy over 23 years ago, McKinley Insurance Services has evolved into an agency with a dedicated team of agents, brokers, customer service representatives, management, and office personnel. McKinley Insurance Services, Inc. is one of the largest insurance agencies in the South Florida Marketplace as determined and ranked by The South Florida Business Journal -, McKinley Insurance Services has a diverse customer base ranging from personal coverage lines (individual auto insurance, homeowner, and health coverage); employer groups (large and small); government agency benefits; as well as commercial property and casualty programs. This would include health risk assessments, wellness, health coaching, disease management, etc. McKinley Insurance Services offers a comprehensive wellness program in conjunction with the health care carrier to our clients which includes carrier administered Health Risk Assessments, Monthly Wellness Seminars, Annual Health Fairs and Quarterly Newsletters to educate, engage and motivate employees to become more aware of health related topics and services available to maintain a healthy lifestyle. McKinley Insurance Services will coordinate monthly and/or quarterly wellness seminars. The seminars feature specific health topics whereby a guest speaker will conduct educational session(s) for the employees. In addition to an educational session, a screening or complimentary service may be offered to the participants (i.e. Rand Eye Institute conducted onsite eye exams, a chiropractor offered a complementary consultation /evaluation). Other services (i.e. biometric screenings, health risk assessments) may also be offered working in conjunction with the health care carrier. 4. Describe an example of a City for whom you have coordinated or provided these services. McKinley Insurance Services currently provides informative Quarterly Newsletters to the employees of the City of Parkland. Additionally we coordinate and facilitate Annual Health Fairs to which health providers are present to conduct biometric screenings. All of the employees' test results are examined and recorded and a Registered Nurse is sent by McKinley to meet one -on -one with the City's employees to relay their personal results. 5. Have you ever failed to complete work awarded to you. If so, when, where, and why? No. McKinley Insurance Services, Inc. has never failed to complete awarded work. 6. How will you maintain confidentiality of the City's records and data (include in your discussion any security procedures for accessing, sending, and storing data that are currently in place)? McKinley Insurance Services understands the importance of confidentiality in that much of our business is with municipal and not - for - profit organizations that regularly handles sensitive information. In addition, our role with our clients often exposes us to very sensitive data such as health conditions and financial status. Each of the areas we are trusted to keep them confidential and discreet when disclosing any aspect of information in the course of resolving inquires. We use secure emails whenever necessary to communicate with carriers. We speak directly with the employee /member regarding health, financial and relationship matters. When handling sensitive documents we use the utmost discretion when receiving and storing. When handling confidential data within our facility, McKinley Insurance Services operates under the following protocol: Locked file cabinets with limited personnel access Locked offices are with the only two keys to access — Assigned person / Facilities management Onsite shredding is performed on a monthly basis. 7. It is our common practice to respond within the hour. In the case of an urgent request, our clients have several points of access to getting the service they need. 1) e-mail 2) call our office and 3) cell phone contact information for all members of the service team for 24 hour access. This ensures that our clients receive the level of required attention and response to assist them with inquiry resolution. When we receive a request, we assess the nature of the request and the impact of response time (i.e. if an employee is attempting to get service at a provider office, we will intervene immediately by contacting the carrier and getting someone to assist at that moment to resolve the matter; in the case there is a question regarding specific guidelines, we will determine if further research is necessary and then follow up with the answer the same day or request additional time if absolutely necessary. 8. How do you measure client satisfaction? Client satisfaction is measured in several ways; clients that do long term business with a service or product provider demonstrate their satisfaction through contract renewal. Another way a client demonstrate their satisfaction is that they recommend our services to others. And the third measurement, is when a client gives you an unsolicited reference. McKinley Insurance Services has and does experience these measures of satisfaction. Direct feedback is the best measure of client satisfaction, McKinley Insurance Services has incorporated an annual client survey to further assess client satisfaction and to determine more personalized client needs. 9. Confirm that you serve as a consultant or broker, independently, and are not affiliated with any insurance company, third party administrative agency or provider network. McKinley Insurance Services is an independently owned and operated full service insurance agency. Our business license and insurance license is not tied to any other organization or entity. 10. Describe your experience in provider network development, recruitment and negotiation, and maintenance. McKinley Insurance Services role as agent of record does not include provider network development. However, we work with the insurance carriers to address provider network needs. 11. Outline your ability to provide expertise and experience in the areas of health benefit plan analysis and design. McKinley Insurance Services conducts a historical review of each client's benefits portfolio, unique needs, and contribution strategy. We will compare The City of South Miami's structure to other relevant market data taking into consideration your budget and other impactful factors; after which we make recommendations best suited for the City. 12. Explain in detail the types of analyses you have conducted relative to benefits analysis and design for a health plan with at least 100 employees. McKinley Insurance Services provides plan design, premium and contribution analysis to its clients (examples included). It is our desire for a client to review all aspects of their renewal, employer and employee costs. The analysis we provide reflects both areas. 13. (Examples, overrides, commission agreements, preferred contracts, pricing based on volume, etc.) McKinley Insurance Services does not have any service provider relationships that would prevent this firm from acting independently when providing service and consultative advice to our clients. 15. List any subcontractors who will provide services under this Contract and the services they will provide. The foregoing list of subcontractor(s) may not be amended afterward of the contract without the prior written approval of the Contract Administrator, to be designated by the City Manager, and whose approval shall not be unreasonably withheld. Benefits Outsource, Inc. (BOI) provides COBRA services; BrokerSuite provides online enrollment services. 16. List and describe all bankruptcy petitions (voluntary or involuntary) which have been filed by or against the Proposer, its parent or subsidiaries or predecessor organizations during the past five (5) years. Include in the description the disposition of each such petition. Provide specific examples of a significant savings in the cost of benefits to the client that can be directly attributed to your past services. McKinley Insurance Services has a proven track record of assisting clients in achieving significant cost savings as related to their employee benefits portfolio. Example 1: City of Parkland — by leveraging new legislative mandate; that a carrier has to provide customers (fewer than 100 employees) with their claims experience data within 21 days of the customer's written request. Obtaining this data revealed that the proposed renewal was excessive. We used the data to renegotiate the renewal from a double to single digit increase Example 2: City of Oakland Park — by using our experience and knowledge of products, McKinley Insurance Services assisted them in changing from a heavily burdened self- insured program to a traditional plan platform and then to their now consumer driven platform which is designed to engage both the employer and employee in cost saving practices. Example 3: City of Riviera Beach — by exploring the current market, McKinley Insurance Services was able to assist the City in reducing its health benefit expenditure by $755,000 with plan modification and tier re- structuring. Example 4: City of Miami Gardens — year over year, McKinley Insurance Services has successfully negotiated single -digit renewals using claims data knowledge and carrier negotiation experience. Describe your firm's ability to assist with Benefits Administration issues. McKinley Insurance Services works with its clients as an extension of its HR / Benefits Department to assist in administering the selected benefits. McKinley Insurance Services acts as liaison between the client and carrier to ensure that benefits and services are rendered in conjunction with issued certificate of coverage and service agreements. In addition to ensuring the clients receive proper service, we also provide data, reports and other documentation that assist the client. McKinley also is able to provide assistance using our current event approach due to our involvement as SHRM members. We are also constantly reviewing legislation, State and Federal to keep our client abreast of and compliant with required regulations as they may pertain to their employee benefits portfolio. 20. The information will result in a custom working strategy to achieve the City of South Miami goal of providing affordable, comprehensive coverage to employees while meeting your budgetary parameters. From the discussion, we will also determine if negotiating with the enforce carrier(s would yield the required results) or if any products need to be marketed for better leverage. If any of the products require marketing, McKinley Insurance Services would develop a timeline with specific dates according to your plan year anniversary. COMPENSATION: Compensation for all services provided shall be on a Nat fee basis from the City. Throughout the duration of the agreement, the successful Proposer shall provide an annual statement from each carrier giving full disclosure of compensation earned. Annual disclosure of all direct or indirect earnings shall be provided prior to March 15 of each year of the agreement TERM OF CONTRACT The initial Contract shall be for a period of three (3) years with an opportunity for the City to extend the Contract for one (1) additional two (2) year period at the same terms, conditions and prices upon mutual agreement of both parties. The Proposer agrees to this condition by signing its proposal. If Proposer cannot renew its initial terms, conditions and prices, Proposer must notify City on /or before 90 days prior to Contract expiration. ADDENDA CHANGES OR INTERPRETATIONS DURING BIDDING Only inquiries or requests for interpretation received five (5) or more days prior to the date fixed for the opening of the proposals will be given consideration. All such interpretations will be made in writing in the form of an addendum and posted on the City's website not later than the established proposal opening date. Each prospective proposer shall acknowledge receipt of such addenda by including its acknowledgment in its proposal. In case any proposer fails to include such addenda or addendum, his proposal will nevertheless be considered as though it had been received and acknowledged and the submission of his proposal will constitute acknowledgement of the receipt of same. Insurance Brokerage Services RFP No. SM- 2011 -09 -HR Page 5 of 25 RULES REGULATIONS LAWS ORDINANCES & LICENSES The Proposer whose bid is accepted shall observe and obey all laws, ordinances, rules, and regulations of the federal, state, and local municipality, which may be applicable to the service being provided. The awarded firm shall have or be responsible for obtaining all necessary permits or licenses required in order to provide this service. RESERVES THE RIGHT The City anticipates awarding one or more contract(s) for services as a result of this Request for Proposals. The City, however, reserves the right to reject any and all submitted Proposals and to further define or limit the scope of the award. The City reserves the right to request additional information from Firms as deemed necessary. Notice is also given of the possibility that an award may be made without discussion or after limited negotiations. It is, therefore, important that all the parts of the Request for Proposals be completed in all respects. The City reserves the right to negotiate modifications to Proposals that it deems acceptable, reject any and all RFP in its sole discretion, and to waive minor irregularities in the procedures. CONTRACT CANCELLATION The resulting Contract may be canceled at any time by the City of South Miami for any reason, upon a thirty (30) day written cancellation notice. OWNERSHIP OF PRELIMINARY & FINAL RECORDS All preliminary and final documentation and records shall become and remain the sole property of the City. The awarded firm shall maintain original documents thereof for its records and for its future professional endeavors and provide legible reproducible copies to the City. In the event of termination of the agreement, the proposing firm shall cease work and deliver to the City all documents (including reports and all other data and material prepared or obtained by the awarded firm in connection with the work being performed), as well as all documents bearing the professional seal of the firm. The City shall, upon delivery of the aforesaid documents, pay the firm and the firm shall accept as full payment for its services there under, a sum of money equal to the percentage of the unpaid work done by the firm and accepted as satisfactory to the City. All brokerage fees being received from carriers or insurers for insurance or benefit plans obtained by the Proposer on behalf of the City for current business and for renewal business shall terminate. INDEMNIFICATION To the extent permitted by law (F.S. 768.28) the proposing firm shall indemnify and hold harmless the City, its officers and employees, from liabilities, damages, losses and costs, including, but not limited to, reasonable attorney's fees, to the extent caused by the negligence, recklessness, or intentional wrongful misconduct of the proposing firm and any persons employed or utilized by the proposing firm in the performance of the work being proposed. EQUAL EMPLOYMENT In accordance with Federal, State and Local law, the proposing firm will not discriminate against any employee or applicant for employment because of race, color, ethnicity, religion, sex, sexual orientation, national origin or handicap. The proposing firm will be required to comply with all aspects of the Americans with Disabilities Act (ADA) during the performance of the work being proposed. Insurance Brokerage Services RFP No. SM -201 I -09 -HR Page 6 of 25 INSURANCE Without limiting its liability, the proposing firm shall be required to procure and maintain at its own expense during the life of the Contract, insurance of the types and in the minimum amounts stated below as will protect the proposing firm and the City, from claims which may arise out of or result from the proposing firm's performance of a Contract with the City, whether performed by itself or by any sub - contractor, or by anyone directly or indirectly employed by any of them or by anyone for whose acts the City may be liable. Workers' As required by law and covering all employees meeting Statutory Compensation Limits in compliance with all applicable state and federal laws. Insurance: With a minimum limit of $1,000,000.00 for each accident. Employer's Liability Comprehensive Coverage shall have minimum limits of $1,000,000.00 per General Liability: occurrence, combined single Limit for Bodily Injury Liability and Property Damage Liability. This shall include Premises and /or Operations; Independent Contractors and Products and /or Completed Operations; Broad Form Property Damage; and Contractual Liability Endorsement. Business Vehicular Coverage shall have minimum limits of $1,000,000.00 per Liability: occurrence. Combined Single Limit for Bodily Injury Liability, and Property Damage Liability: This shall include Owned Vehicles, Hired and Non -Owned Vehicles and Employees Non - Ownership. Professional Liability: Coverage shall have minimum limits of $1,000,000.00 per occurrence with respect to negligent acts, errors or omissions in connection with the professional services to be provided and any deductible not to exceed $5,000.00 each claim. A. The City shall be listed as an Additional Named Insured on the policies for Comprehensive General Liability, Employer's Liability, Workers' Compensation Insurance and Business Vehicular Liability. In the event the insurance coverage expires prior to completion of the term of the Contract with the City, a renewal certificate shall be issued 30 days prior to said expiration date. The policy shall provide for a 30 -day notification to the City in the event of cancellation or modification to the policy. The policy shall not be cancelled or modified without the written consent of the City. B. Unless otherwise specified, it shall be the responsibility of the proposing firm to insure that all subcontractors comply with the same insurance requirements as provided herein. All proposer certificates of insurance must be on file with and approved by the City before the commencement of work activities. Waivers of subrogation shall also be provided upon approval of the applicable insurers. C. The proposing firm shall "flow down" the requirements of this provision to all subcontractors. D. The limits of insurance required above must be retained throughout the term of the contract. The proposing firm must notify the City immediately if any of the required coverage limits are reduced due to claim activity or for any other reason. E. Policies should be written on an "occurrence" basis. Insurance Brokerage Services RFP No. SM- 2011 -09 -HR Page 7 of 25 OPENING OF REQUEST FOR PROPOSALS Request for Proposals will be opened and evaluated after the final date and time set for receipt. The City may request proposing firm(s) considered for award to make an oral presentation to a selection board or to submit additional data. REIECTION OF PROPOSALS The City reserves the right to reject any and all Request for Proposals. It also reserves the right to waive any minor irregularities in connection with response to the Request for Proposals. ACCEPTANCE OF PROPOSAL Within ninety (90) days after the final submission date for Request for Proposals, the City will act upon them. The Successful Firm will be requested to enter into negotiations to produce the Contract. The City reserves the right to terminate negotiations and cancel the bid award in the event it deems progress towards a Contract to be insufficient. Applicable Law T All applicable laws and regulations of the State of Florida and ordinances and regulations of the City of South Miami will apply to any resulting agreement and venue for any action arising out of the Contract shall be in Miami -Dade County, Florida, and such right shall remain solely with the City. Oualifiication of Proposing Firm Request for Proposals will be considered from firms normally engaged in providing the services requested. The proposing Firm must demonstrate adequate experience, organization, offices, equipment and personnel to ensure prompt and efficient service to the City of South Miami. The City reserves the right, before recommending any award, to inspect the offices and organization or to take any other action necessary to determine ability to perform in accordance with the specifications, terms and conditions. The City of South Miami will determine whether the evidence of ability to perform is satisfactory and reserves the right to reject Request for Proposals where evidence submitted, or investigation and evaluation, indicates inability of a firm to perform. The failure of the Bidder to make arrangements, satisfactory to the City, for the City's inspection and investigation shall be grounds for rejection of the Bid. Designated Contact The awarded firm shall appoint a person to act as the firm's primary contact with the City of South Miami. The contact person or person designated in writing by the firm, shall be readily available during normal work hours by phone, e-mail, facsimile transmission or in person, at the option of the City, and shall be knowledgeable of the terms of the Contract and the work being performed pursuant to the Contract terms. Deviations from Specifications The awarded Firm shall clearly indicate, as applicable, all areas in which the services proposed do not fully comply with the requirements of this Request for Proposal. The decision as to whether an item fully complies with the stated requirements rests solely with the City of South Miami. Insurance Brokerage Services RFP No. SM- 2011 -09 -HR Page 8 of 25 Precedence of Conditions The proposing Firm, by virtue of submitting a proposal, agrees that City's General Provisions, Terms and Conditions stated herein shall be incorporated into the Contract and take precedence over any terms and conditions submitted with the proposal, either appearing separately or included, and shall not be modified without the written consent of the City. Proposal Withdrawal After Request for Proposals are opened, corrections or modifications to Proposals are not permitted, but the proposing firm may be permitted to withdraw an erroneous proposal prior to the adoption of the award by the City Commission, if the following is established: A. The proposing firm acted in good faith in submitting the proposal; B. The error was not the result of gross negligence or willful inattention on the part of the firm; C. The error was discovered and communicated to the City within twenty -four (24) hours of proposal opening, along with a written request for permission to withdraw the proposal; and D. The firm submits documentation and an explanation of how the proposal error was made. Public Entity Crimes A person or affiliate who has been placed on the Convicted Vendors List following a conviction for a public entity crime may not submit a proposal for a contract to provide any services to a public entity, may not submit a Request for Qualifications on leases of real property to a public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017, for a period of 36 months from the date of being placed on the Convicted Vendors List. Contingent Fees Prohibited The proposing firm must warrant that it has not employed or retained a company or person, other than a bona fide employee, contractor or subcontractor, working in its employ, to solicit or secure a contract with the CITY, and that it has not paid or agreed to pay any person, company, corporation, individual or firm other than a bona fide employee, contractor or subcontractor, working in its employ, any fee, commission, percentage, gift or other consideration contingent upon or resulting from the award or making of a contract with the City. Auditable Records The proposing firm shall maintain auditable records concerning compliance with the Contract. These records shall be kept in accordance with generally accepted accounting principles, and the City reserves the right to determine record - keeping method in the event of non - conformity. These records shall be maintained for five (5) years after the term of the Contract and shall be readily available to City personnel with reasonable notice, and to other persons in accordance with the Florida Public Disclosure Statutes. Unauthorized Aliens The employment of unauthorized aliens by any firm is considered a violation of Federal Law. If the Bidder /Contractor knowingly employs unauthorized aliens, such violation shall be cause for unilateral cancellation of any contract resulting from this RFP. This applies to any sub - contractors used by the firm as well. Insurance Brokerage Services RFP No. SM- 2011 -09 -HR Page 9 of 25 Waiver The failure to enforce Contract as to any particular breach or default shall not act as a waiver of any subsequent breach or default. No act or omission shall be construed as a waiver of any of the terms of the Contract. A waiver of any of the terms of Contract shall only be valid and enforceable if the waiver is in writing signed by both parties. Modifications and Amendments The Contract shall contains the entire agreement between the parties and no modification or amendment of any terms or provisions of the Contract shall be valid or binding unless it complies with this paragraph. The Contract, in general, and this paragraph, in particular, shall not be modified or amended except in writing signed by both parties to the Contract. McKinley Financial Services recognizes that employers must be contentious when designing their employee benefits portfolio, We understand how important it is to offer benefit options that are both affordable and valuable. We also believe that selecting the right benefit portfolio is critical to employee satisfaction and peace of mind. Below is a summary of our findings and recommendations for 2010 benefit year. Medical The medical insurance with MEDICAL CARRUER reflected an XXX% increase. The current benefit structure has low out -of- pocket costs. We recommend that the CLIENT remain with MEDICAL CARRIER as your medical carrier. Dental The dental insurance is provided through DENTAL CARRIER. The data provided by Aetna reflects the plan years based on claims paid for the periods of April 1, 2008 — March 31, 2009 and April 1, 2009 - March 31, 2010. Because of the group size more specific claims and utilization information is not available. As a result of the Patient Protection and Affordable Care Act, in the future, specifically due to the medical loss ratio and transparency requirements, we believe more detailed data will be available and we will be able to provide more in depth analytics. This report also contains rate history from 2007 to current, utilization information for 2009 and 2010, as well as demographic information on the client's employees. We are confident that this report will provide some insight as to the direction the City is headed and with our partnership we will be able to make any course changes that are necessary. Sincerely, Carmen Open Enrollment and the Implementation Since Open Enrollment and the implementation of the new plan year, October 1, 2010, we have held two (2) insurance committee meetings. The December meeting was cancelled. The next meeting is scheduled for January 13, 2011. We have discussed the 2011 health fair, which will be held on May 18, 2011 at the YMCA. We have already sent out "Save the Date" notifications to vendors and we are still seeking more. Other insurance committee discussions have included the online enrollment process, the new carriers and also the RFP process for 2011. Customer Service Days We have also held two (2) customer service days. The first meeting in October was for Florida Dental Benefits, who came in and delivered ID Cards for members and answered any questions. The next meeting included Colonial, Benefits Outsource Inc., Solstice Benefits and Florida Dental Benefits. It is our expectation that each month there will be different carriers that will attend. This will give members the opportunity to ask questions directly of the carriers. We will continue to hold monthly Customer Service Days to address benefit 'issues. Although currently, there have been very few issues with these carriers, it is best to catch any issues before they get out of hand. New — Benefits on a Budget New for 2011 will be the quarterly sessions for "Benefits on a Budget ". These programs will be designed to support employees in utilizing their benefits in a cost effective manner. These 30 minute sessions will be held in conjunction with the birthday cake (sponsored by Florida Dental Benefits), since the committee feels that food will get people to attend. The first session will be held on January 26, 2011 at 2:30pm and will talk about Prescription Drugs, Generic vs. Brand. 2011 -2012 Strategy As discussed, we will go out to bid in April for Medical insurance, but we would also suggest that we include the legal product. The current plan costs approximately $20 for employees and their families to enroll. We feel there are several companies that will be more competitively priced with equal or better benefits. There are only 4 people currently enrolled in this product. Current Plans Status Medical: Employee has issues with the Physical Therapy benefit, which contains a lifetime benefit. Although Health Care Reform has removed such limits for "essential benefits" this is not considered an "essential benefit ". Dental: No outstanding issues with either dental carrier. One employee had a challenge with getting payment for additional cleaning but I have been assured that the payment has been sent to the provider. Another employee went for services earlier than the plan allowed and received minimum payment to the dentist, but that too has been corrected by the carrier. Both of these were issues with Solstice Benefits. Vision: This is a new carrier with benefits that went into effect 101112010. 1 have had no issues to date from the members or insurance committee. Currently there are no other benefits to be recommended, as the City currently offers a full selection of benefit options. Municipal Benchmarking McKinley Insurance Services is currently conducting our annual Municipal Benchmarking Survey to better advise if there are other innovations that cities are offering to keep the client competitive with other cities in the County and surrounding areas. Once the survey is complete, we will share that information with you. Closing Looking ahead, we are currently gathering claims and other information to request bids for self - insured options. All on -site clinic offerings are with self- insured programs. We cannot begin to look at the onsite clinic option until we know if there will even be an available carrier that will provide self insurance options. We expect to have some preliminary answers by April, in time for the RFP to be released. ANNUAL COMPARISON YEAR TO YEAR 2009-2010 IX I KlNfQ -E DEMOGRAPHICS The 2009 -2010 Demographics are essentially unchanged with 58 employees in 2009 and 57 employees in 2010. Total enrollment in 2009 was 95 and the total for 2010 is 98. The male to female ratio in 2009 was 58% to 42% and in 2010 the ratio is 57.5% to 42.5 %. The average age in the group went down slightly. In 2009 the average age was 38.3 and in 2010 it is 37.4. However the number of days of care increased by 44.9% and the average length of stay was up 125% with the average length of stay of 2.0 days in 2009 to 4.5 days in 2010. Inpatient vs. outpatient surgeries also changed from 2009 to 2010 with inpatient surgeries down 3.4% and outpatient surgeries up 16.8 %. Office visits also increased in 2010, the number of visits was up 10,6% and Emergency Room visits increased by 8.7 %. The charts below illustrate cost sharing 2009 vs. 2010 with the employer portion increasing in 2010 by 3 %. There was no coordination of benefits in 2010 and nothing attributed to a deductible. Copays were also down 1% in 2010. The average copay per claim was down in 2010 from $25.29 to $24.11 a 4.7% decrease. This is attributable to the increased generic utilization and generic substitution which where both up 4.5% and2.5% respectively. Formulary utilization was also increased in 2010 from 72.2% to 77.5 %. The chart below illustrates utilization of generic vs. brand for 2009 and 2010. According to Pricewaterhouse Coopers the medical trend nationally starting in 2008 was 9.9 %, 2009 — 9.2% and 2010 was 9.5 %. Florida has historically been 2 -3% higher than the national average. Pricewaterhouse Coopers has also estimated the medical trend for 2011 to be at 9 %. The Council for Insurance Agents and Brokers completed a survey in December 2010. The survey queried brokers nationwide concerning rate increases for 2010. For group sizes 51 -500 in the southeast 65% had rate increases between 11. and 20 %.
