TAB B: QUALIFICATIONS & EXPERIENCE Qualifications and Experience - Proposer's Qualification Statement, shall include the following information: 1. A brief discussion of the firm's understanding and approach to the work described herein. Gehring Group understands the scope of work to include all aspects of servicing the City of South Miami's employee /retiree benefits program including but not limited to bidding, evaluating, and making recommendations regarding all lines of the City's employee benefits insurance including health, dental, life, LTD, STD, Vision and all other supplemental insurance, as well as providing various additional services to City staff and employees throughout the year. From bidding, to implementation to open enrollment, your Gehring Group service team is truly an extension of, and valuable resource to the City's benefits administration and human resources staff. To further clarify, we have addressed each item in the Scope of Work outlined in the RFP as follows: ONGOING SERVICES A. Monitor the programs' operations throughout the year to ensure that benefit providers are meeting all customer service requirements and standards. Confirmed. Your Gehring Group team will remain in constant communication not only with City Staff, but also with each of the City's vendors to ensure that plans and contracts are being administered correctly. B. Provide ongoing administrative support, as requested, by acting as a liaison between the City and providers to assist with resolving claim disputes, contract administration and interpretations, and other issues. Confirmed. Your Gehring Group Account Manager will maintain continuous communication with City Staff throughout the plan year to provide support with administrative, legislative, enrollment and billing questions. They can also be a resource to your employees and are available to assist with the resolution of claim problems and other issues such as policy interpretation. C. Provide dedicated personnel as the primary contact for managing the account relationship with the City. Confirmed. The City has been assigned an expert team of Gehring Group staff which includes an Account Manager for day -to -day issues and an Analyst for any financial related plan issues. In addition, the overall level of service to the City will be overseen by Gehring Group's Managing Director, Anna Maria Studley. (Resumes included below.) -3- D. Meet with specific City staff throughout the yearns reasonably necessary (minimum is quarterly). Confirmed. We provide claims experience reports on a quarterly basis (if available) and monitor the plan's performance in order to be adequately prepared for the renewal. Your team is available to meet the City at least quarterly as well as on an as needed basis with more frequent meetings often occurring during renewal time. Please note that Gehring Group does not pass through any travel expenses to our clients and your team is available for on -site meetings with reasonable notice. E. Coordinate annual audits of City's benefits plans and associated vendors and prepare annual financial reports on the results of the completed plan year. Confirmed. Gehring Group's Analytical Services Department staff members are experts in the evaluation skills necessary to present complex insurance information in an easy to understand format. We monitor claims experience on a monthly basis, providing detailed reports, and alert you regarding large claims or cost trends impacting your experience. For many of our groups, we also prepare an annual presentation in which we present a summary of the benefit plan year. In this presentation we summarize the significant events relating to your employee benefits plan such as plan changes, carrier changes, large claims, claims trends in general and the implementation of any new cost saving strategies. F. Prepare and deliver any necessary reports to the City Employee Benefits Administrator, including, but not limited to, reports showing claims experience at intervals acceptable to the City. Confirmed. Gehring Group closely monitors the group's claims experience reports and provides them to the appropriate City Staff on a quarterly basis. Additional reports are also available, the frequency of which may be determined by the size of your group and can be discussed upon selection. Provide advice and assistance in the review of City employee health and medical benefits program on a continuing basis to ensure that those plans are in compliance with state /federal requirements and their adequacy of benefits with respect to other plans. Track, monitor and provide information on changes in, or any pending or new legislation in the applicable state and federal laws, as well as any funding trends that may affect the benefits program; to the City's Human Resources Director. In addition, we are available to assist with all regulatory and compliance duties including all required filings. Another major consideration is the Patient Protection and Affordable Care Act, also known as Health Care Reform. As health care reform continues to evolve, Gehring Group will diligently review all newly available product offerings to ensure that our clients are always presented with the best available options while complying with all mandates and requirements of the health care reform legislation. Additionally, we provide our clients with free access to a human resources research tool, HR Answers Now. This tool is one of the value added services provided to our clients at no additional cost via the Gehring Group client portal. HR Answers Now offers a one- stop resource for quick answers to thousands of HR questions covering such issues as record- keeping, wages and withholding, workers' compensation, harassment, COBRA and FMLA. I. Advise and assist the City as requested with: • Writing employee benefits plan modifications and /or new benefits plans and any required amendment approval process; Confirmed. Your Gehring Group team of experts is available to assist the City in reviewing and /or modifying its employee benefits plan and reviewing all plan amendments for compliance and accuracy. • Submission of written reports and other documents as required by the state and /or federal government; Confirmed. Gehring Group will assist the City in completing all reports or documents required by the state and /or federal government. • Coordination of the annual employee wellness fair. Confirmed. Having coordinate numerous health and wellness fairs for our current clients, your Gehring Group Account Manager is well - versed in this process and can provide valuable assistance throughout this process. -5- • Development of an Employee Wellness Program. Confirmed. The key considerations for developing a wellness program are to obtain measurable results which are obtained through periodic health risk assessments and screenings; structured and non - structured activities that are provided by internal and external resources; incentive programs are needed to encourage and sustain participation; there must be support of management in order to effectively communicate and encourage participation; and budgeting of the program. The simplest of Wellness Programs can cost next to nothing; however, Wellness Programs in which health risk assessments and biometric screenings are conducted for the entire employee population and measured throughout several plan years can incur substantial cost to the group. For many of our smaller clients who may not have a "wellness budget ", Gehring Group has enlisted the client's EAP provider or health insurance carrier to provide more extensive disease management and educational wellness seminars. We have assisted some of our larger clients in implementing a full blown wellness plan that includes incentives for lifestyle changes such as weight loss, smoking cessation, lowering their cholesterol, exercise, etc. Having researched and implemented numerous wellness initiatives at our public sector clients, we have included the following examples for your review: • Tax Collector of Palm Beach County, whose program includes annual health risk assessments and biometric screenings, implementing a smoke -free campus, addition of fitness equipment at various worksites, and incentives for health risk improvements. • City of West Palm Beach, who also implemented a smoke -free workplace, offers monthly wellness meetings on various topics, biometric screenings and additional series of fitness focused activities on a regular basis. • City of Clearwater holds an annual health fair which includes biometric screenings, free fitness assessments with a health counselor, classes on healthy eating, smoking cessation programs and weekly competitions with incentives for increasing one's level of physical activity. • The City of Sarasota in implemented a customized wellness program that incorporated five measureable targets and alternative incentive rewards including the ability to buy down health plan deductibles or payroll deductions for health insurance premiums. We are available to assist in initiating new wellness programs and /or support wellness programs already established. I Perform special projects as requested by the City, including but not limited to: a. Development and assistance in the' implementation of new insurance plans; Confirmed. Gehring Group remains on the cutting edge when it comes to the implementation of new insurance concepts. We were one of the first in the state to -6- implement a Consumer Driven Health Plan in a public sector agency and also have significant experience with all types of funding arrangements and tax - advantaged accounts such as Health Savings Accounts and Health Reimbursement Accounts. Additionally, we are closing monitoring the health plan trends taking place as a result of last year's health care reform legislation. ln. Assistance with adjudication of specific claims as requested by City; Confirmed. Gehring Group staff is available to assist with claim issues that may require additional attention. In addition, we can deal directly with employees seeking resolution to specific claims problems. Gehring Group is HIPAA compliant; therefore, all confidential information will remain such. c. Recommendation of alternative benefit designs or delivery systems as dictated by emerging plan costs or benefits practices. Confirmed. With experience in all types of benefit designs, funding arrangements and new insurance concepts, Gehring Group will keep the City advised as to all benefit plan alternatives. K. Ensure personnel availability for meetings, phone calls, and e-mail correspondence as required. Confirmed. Gehring Group provides all staff members with the technological tools to service our clients in a timely manner. L. Maintain the confidentiality of City records and data where applicable under federal and state laws. Confirmed. Our organization has established several policies and procedures to ensure the confidentiality, integrity, and availability of all our information and information systems. We formed these policies and procedures to minimize any risks to the organization from internal and external events. These policies and procedures cover areas such as: hardware and software purchasing, anti -virus and anti -seam systems, data management including data encryption and confidentiality, network and telecommunication security, and access controls and password usage. Specific examples of the use of security techniques to protect information include the strict use and monitoring of VPNs between computer facilities, the use of secure FTP (FTP/ES) for transferring files between our organization and external parties, the use of PGP public key encryption to secure information stored on systems, and the use of encrypted email for transmitting sensitive information M. Perform other related services on all "as needed" basis. Confirmed. All insurance related services would be included under the Agent of Record arrangement. -7- RENEWAL YEAR SERVICES A. Using current health and medical benefit plans as benchmarks, research, design and propose employee benefit plans for the City, as appropriate. Confirmed. All analysis materials compiled by Gehring Group will include a comparison of any proposed option against the City's current benefit plans. Gehring Group's Analytical Services Department staff members are experts in the evaluation skills necessary to present complex insurance information in an easy to understand format. B. Meet with the City as necessary to discuss benefit plan options and establish goals and objectives for the City's Benefits program. Confirmed. Our first priority would be to meet with City staff to determine what they deem to be the positive aspects of their program as well as any areas of particular concern. Discussion of future goals will take place. In addition, Gehring Group staff would outline the various program options available to the City for all lines of coverage including self insurance, fully insured options, alternative health plan models, and other cost saving tax- advantaged funding vehicles such as HRA's, HSA's and Medicare Advantage plans. The next step would be to review the current claims utilization for all employee benefit insurance offerings. As a rule for all of our clients, we consistently track the available claims utilization data of your program throughout the plan year in order to more effectively prepare for the renewal process. We would review available claims utilization reports to determine whether your programs are running favorably in order to forecast renewal projections and negotiate with your current vendors. C. Provide analysis of renewal of current plans, including the review of past performance with regard to renewal. Confirmed. We monitor claims experience on a monthly basis, providing detailed reports, and alert you regarding large claims or cost trends impacting your experience. Exhibit 4 includes a sample Presentation to Executive Staff for your review. 1). Review additional available cost savings plan alternatives and creative funding options. Confirmed. Your Gehring Group team will consider all available funding and benefit plan options to present to the City for consideration. We will make our. recommendations based solely on the City's needs, goals and budget requirements. Due to our completely independent status, we base all recommendation on what is best for our clients. -8- E. Determine the appropriate employee and employer benefit contribution levels. Based on our focus on the public sector, Gehring Group has a significant amount of benchmark information in- house. In addition, we conduct additional benchmark surveys, the results of which can be used to measure the competitiveness of the City's employee benefits plan benefits as well as employer contribution levels. Upon selection, Gehring Group will discuss the City's current contribution structure in greater detail and determine the City's philosophy on contribution levels. From this point, we can recommend alternatives for consideration if deemed necessary. F. Review and recommend annual contribution strategy from active participants and retirees. Confirmed. As stated in bullet E above, Gehring Group will assist the City in determining its contribution strategy on an annual basis. We will take into consideration local benchmark data, renewal cost proposals, union agreements and all other factors relating to the implementation of a competitive contribution strategy and make recommendations accordingly. G. Provide City with information on 'what other municipalities of comparable size and location will be doing with their benefits in the upcoming. years. Confirmed. Gehring Group's large number of public sector clientele enables us to have significant comparison information in house. In addition, Gehring Group conducts local entity surveys for our clients on a regular basis. We realize the value in maintaining a competitive benefits package as it relates to employee hiring and retention. A sample of a local entity survey conducted for another Gehring Group client is included in Exhibit 8 for your review. 11. Conduct renewal negotiations and develop appropriate information for management purposes. Confirmed. Gehring Group will lead the negotiations whether at renewal or during the RFP process and will compile all necessary reports for management staff. These reports would include a detailed cost analysis and will clearly indicate all applicable plan changes and alternatives. We will provide City staff with a management report to summarize the negotiation activities and outline the resulting recommendations. 1. Upon City's request, assist in coordinating a comprehensive "Request for Proposal" (RFP) process to identify potential high quality Benefits vendors, according to established City guidelines. The scope of this RFP may include, but is not limited to: Medical, Dental, Vision, Basic Life, Voluntary Life, Accidental Death and Dismemberment, Short Term and Long Term Disability insurance providers. Confirmed. Having developed a strong niche in the public sector market, public entities represent a predominant portion of the company's current client base. Based on this level of experience in such a specialized market, our firm is uniquely qualified in its won understanding of public entity issues. We understand the bid process and public record laws while maintaining familiarity with the complex Statutes that apply to governmental organizations. Due to Gehring Group's large public sector client base and thus, significant premium volume with the insurance carriers, Gehring Group has been very successful in negotiating competitive renewal rates with carriers. In addition, we have significant experience in conducting a formal competitive bid process in conjunction with our clients' Purchasing /Procurement Divisions. We can be as involved as the group would like us to be in the formulation and release of the RFP to the market and will be very involved in finalist presentations and best and final negotiations. Gehring Group will consistently provide thorough examination of all proposals received during a bid process. We will compare all proposals to the in -force program and illustrate the program differences to include the advantages and disadvantages of each. This will include a detailed cost comparison which outlines the total cost of the program in addition to breaking down the costs related to employer and employee contributions in an easy to understand format. During this process, we will also compare provider networks to determine which proposers may be considered viable options. Finally, we will be intimately involved in finalist negotiations, making recommendations and overseeing the implementation process. This includes gathering all relevant information such as claims experience, rate history, contracts, and other information required for an RFP and ensuring receipt of all implemented contracts. J. Act as Lead Negotiator and Consultant to the City during benefit contract negotiations and renewals. Confirmed. Gehring Group staff has the capabilities, education and training to negotiate for the City effectively. Our large public sector client base will provide your group with the necessary negotiating strength when dealing with various insurance carriers during the renewal season and throughout the year. Gehring Group represents you, not the insurance company. And, we are well - versed in dealing with issues relating to public entities. Review plan documents (including employee booklets) and master contracts before adoption and printing. -ro- Confirmed. Your Gehring Group Account Manager will be responsible for reviewing each employee booklet, certificate and /or contract for accuracy, completeness and compliance. M. Assist with the planning and implementation of selected changes including transition from the current to new vendors, the renewal proposal, and other benefits changes. Gehring Group staff has extensive experience in implementing plans with all major insurance carriers in the state of Florida. In addition, our goal is to ensure a smooth transition whether the group is switching from one vendor to another or implementing an alternative renewal proposal with the same vendor. We are also very involved in communicating benefit changes to your employees and are available to conduct open enrollment and new -hire orientations as necessary. Our staff will help the City navigate the various health plan options and funding arrangements in order to implement a program that best meets the City's needs. N. Assist with developing City employee benefit program communication materials. Coordinate the design, printing and production of those materials, as edited and approved by the Human Resources Director. Confirmed. As part of Gehring Group's services, we provide an employee benefits handbook and various communication materials, customized for and subject to the approval of the City. At the beginning of each new plan year we compile all of the information regarding your insurance coverages and summarize it in an employee friendly benefit booklet. This booklet has proven to be a valuable resource to our client's employees and has reduced the number of inquiries received by our client's HR and Benefits staff. This service is offered at no additional cost. We will provide you with enough copies for open enrollment and as needed for new -hire orientations throughout the plan year. Please refer to Exhibit 5: Sample Employee Benefit Highlights for examples. In addition, we have the ability to produce customized communication pieces based on the specific needs of the City. Exhibit 6: Sample Employee Communications includes such examples for your review. O. Advise and assist the Hunan Resources Director or designee with the review of contracts, plan documents, insurance policies and other documents for applicability, accuracy, consistency, and legal compliance. Confirmed. Your Gehring Group Account Manager will be responsible for reviewing contracts, plan documents, insurance policies and other documents for applicability, accuracy, consistency, and legal compliance. P. Assist City with the development of performance guarantees relating to vendors' performance of services to the City, and evaluation of the performance of vendors. Confirmed. Gehring Group has assisted several of our clients in negotiating performance guarantees for claims administration. The availability and depth of such guarantees will depend on various factors such as group size, funding arrangement and plan design. "11" f� 2. A resume of the Proposer who will work on City matters and a statement identifying the individual who will attend meetings and have primary responsibility for City matters and whether that individual represents other municipalities. At Gehring Group, all clients are assigned a team of consultants who work on their account including an account manager, a back -up account manager, an analyst, and a service representative. These staff members are under the direction of the Managing Director who has 30 years of industry experience and will be the City's Lead Consultant. This approach ensures that an entire team of experts are considering responses, resolutions, and recommendations being put forth to our clients and monitoring their success. Anna Maria Studley will be the Lead Consultant responsible overseeing the account management and analytical staff as well as meeting with the City as needed and making presentations as required. Provide specific examples of a significant savings in the cost of benefits to the client that can be directly attributed to your consultant services. -39- Example 1: City of Stuart Cost Reduction Strategy: Transition from Self Funded to Fully Insured Gehring Group has also helped clients achieve cost savings by analyzing claims trends of the specific group and recommending plan changes accordingly. Another related example of Gehring Group's success in this area is the City of Stuart. Gehring Group was appointed agent of record for this group in 2003, at which time we began the process of streamlining the funding and cost tracking of their current self-insured health program. We were able to pinpoint areas of their program where cost savings were achievable, and direct the RFP process to ensure these areas were addressed, resulting in plan savings. Due to the fact that the City had experienced several years of large losses due to high utilization and catastrophic claims, a self-funded program was no longer proving to be a cost efficient option for the City to continue to pursue. As a result of the RFP process, the City took the Gehring Group's recommendation to transition from a self-insured program with a rented network to a fully insured option with a reputable insurance carrier for the plan year beginning October 1, 2004. This change was successful not only in simplifying the budget process and plan administration, but also in reducing claims costs due to deeper discounts associated with a carrier network versus a rented network as illustrated by the following data. As of October 1, 2010, the City has been able to maintain a rich schedule of benefits, while keeping claims costs below the levels of six years prior. Example 2: Florida Keys Aqueduct Authority Cost Reduction Strategy: Transition from Rented Network to Carrier Network The Florida Keys Aqueduct Authority sought the expertise of Gehring Group in an attempt to find a more cost effective alternative to the indemnity health insurance program in force at the time. Due to their location in the Florida Keys, there were limited plan options available to larger employer groups, therefore, they were participating in a pooled program with several national labor unions in a health plan consisting only of an indemnity option. Based -40- on our review and analysis of their program, we were able to propose several recommendations to accomplish their goal of cost savings while, at the same time, providing an enhanced health insurance program to its employees. The first step was to conduct a formal RFP process to ensure that the Authority received the most competitive options available. After Gehring Group's evaluation of all viable options, the Authority was able to transition to a fully insured PPO program with a reputable insurance carrier. Employees benefited from an enhanced schedule of benefits while the entity benefited from the< significant cost savings. The most notable accomplishment of this transition is that the Authority was able to enter its fourth year under this program with no rate increase. This plan provided a more comprehensive schedule of benefits while reducing health insurance premiums by 11.4 %. Subsequently transitioning to a "participating" contract allowing the group to take advantage of profitable claims years, and receiving a refund over $150,000 for its 2007/2008 plan year. Example 3: City of Vero Beach Cost Reduction Strategy: Transition from Fully Insured to Self Funded The City of Vero Beach has successfully maintained a comprehensive health insurance program consisting solely of a PPO offering for over 30 years. Numerous factors have contributed to their ability to maintain such a rich plan including location, demographics, claims experience, group claim trends, etc. Funded under a fully insured arrangement for over 30 years, Gehring Group recommended that the City allow us to conduct an analysis of the group's claims history versus its annual renewal increases. The results of our analysis indicated that the group had the potential to save considerable money and better manage its renewal costs if they transitioned to a self funded arrangement. This strategy has paid off significantly for the City by enabling them to stabilize their renewal increases to a level that is tolerable year after year, and most importantly, predictable, under current budget conditions. The following chart illustrates how the City's health insurance renewal increases have stabilized under Gehring Group's recommendation to self insure, During the period of 1998 through 2001, renewal increases were erratic based on whether the group had a "bad claims year." By maintaining adequate plan reserves and funding the program at reasonable levels, we were able to better control renewal increases.
