• Development of an Employee Wellness Program.
Confirmed. The key considerations for developing a wellness program are to obtain
measurable results which are obtained through periodic health risk assessments and

communicate and encourage participation; and budgeting of the program. The
simplest of Wellness Programs can cost next to nothing; however, Wellness Programs
in which health risk assessments and biometric screenings are conducted for the entire
employee population and measured throughout several plan years can incur
substantial cost to the group.
For many of our smaller clients who may not have a "wellness budget ", Gehring
Group has enlisted the client's EAP provider or health insurance carrier to provide
more extensive disease management and educational wellness seminars. We have
assisted some of our larger clients in implementing a full blown wellness plan that
includes incentives for lifestyle changes such as weight loss, smoking cessation,
lowering their cholesterol, exercise, etc.
Having researched and implemented numerous wellness initiatives at our public
sector clients, we have included the following examples for your review:
• Tax Collector of Palm Beach County, whose program includes annual health
risk assessments and biometric screenings, implementing a smoke -free
campus, addition of fitness equipment at various worksites, and incentives for
health risk improvements.
• City of West Palm Beach, who also implemented a smoke -free workplace,
offers monthly wellness meetings on various topics, biometric screenings and
additional series of fitness focused activities on a regular basis.
• City of Clearwater holds an annual health fair which includes biometric
screenings, free fitness assessments with a health counselor, classes on healthy
eating, smoking cessation programs and weekly competitions with incentives
for increasing one's level of physical activity.
• The City of Sarasota in implemented a customized wellness program that
incorporated five measureable targets and alternative incentive rewards
including the ability to buy down health plan deductibles or payroll
deductions for health insurance premiums.

programs already established.

6. Provide resumes of staff that will be performing the work.
Gehring Group embodies an unparalleled commitment to providing a value proposition to its
clients, employing high caliber professionals and remaining on the cutting edge of industry
innovation. Gehring Group is not organized like a traditional insurance agency. Typical
agencies grow by employing a number of producers who sell and manage a "silo" of accounts.
Free of this business model, Gehring Group is able to maintain a supportive team environment
and culture whereby all employees feel an allegiance and commitment to all of our clients as a
whole. Additionally, the resources usually dedicated to commissions to these producers are
funneled into additional value added services for our clients such as professional level
communication materials, COBRA services, FSA administration services, on -line benefits
administration solutions, HR resource tools, etc. We provide an all inclusive service model so
that our clients never hesitate to tap into our expertise.
Expertise is in abundance at the Gehring Group. We have a corporate culture of only employing
high performing and talented professionals who add to our team. This employment model
ensures that the primary and back up account managers, and evaluators assigned to your account
will be proficient extensions of your human resources and benefits teams. All staff members
assigned to service our clients are well- versed in all lines of employee benefits insurance. And if
you need to speak to someone and your representative isn't available; any of our other highly
capable professionals will be willing (and have access to your information through our agency
management system), to assist you. And from an evaluations and analysis standpoint, it is
important to note that our evaluations team markets and bids all lines of coverage for all of our
clients out of one central location, ensuring in -depth experience, negotiating influence and timely
expertise is afforded to all of our clients.
From an account service standpoint, it is important to note that all of the team members servicing
the City's account have had significant experience in claim escalation and issue resolution. In
addition to your assigned account manager, Gehring Group also provides two in -house account
service personnel specifically for this purpose, so that even if your account manager is
temporarily unavailable, there will be someone accessible to you and your employees. These
staff members are available to help employees work through claims issues by analyzing the issue
and working with the carrier claims department or service representative as well as the provider's
office to seek resolution. The internal account managers are also intricately involved in helping
to resolve escalated claims issues by assisting with writing appeal letters in the event a claim has
been denied. They follow up with the applicable carrier claims department or service
representative and assist in gathering all required information and documentation and
continuously follow up through resolution. They exhaust all avenues in their efforts to bring
each employee issue to resolution.
Additionally, Gehring Group's expertise is well -known throughout the state as evidenced by the
repeated number of requests for our staff members to be featured speakers at various Florida
public sector associations and other organizations including:
FAC — Florida Association of Counties
■ FERMA — Florida Educational Risk Management Association
FGFOA — Florida Government Finance Officers Association
■ Florida League of Cities
_19"'
• FPELRA — Florida Public Employer Labor Relations Association
• FPPA — Florida Public Personnel Association
• PRIMA — Public Risk Management Association
• RIMS — Risk & Insurance Management Society
• SHRM — Society for Human Resource Management
Gehring Group is also a member of each of the above listed associations through which we are
able to stay abreast of all issues public sector entities are facing today.

for its implementation has been established.
Technology assets are housed in an qMrppriqtely secure physical location. Technology assets
include servers, personal computers that contain systems with controlled access, and network
components (cabling, electronics etc.).
To control logical access, Gehring Group utilizes dual WatchQuard firewalls for perimeter
defense a intruders. The firewall devices ,provide robust user and -Vplicati
pplicy enforcement, multi-vector attack protection and secure connectivity services through
a wide range of security and networking services, including:
-33- GEA IRINGAGxROUI
• Advanced Application- Aware Firewall Services
• Robust Site-to-Site and Remote Access 1PSec VPN Connectivity
• Intelligent-Networking Services
Flexible Management Solutions
In the event of a failure of the primary firewall, the secondary firewall automatically and
immediately begins routing traffic. With this confi guration, a failure of a firewall device will
have virtually no impAct on system availability. The security posture of these firewalls allows
only specific traffic into the network, and restricts what traffic can leave.
In addition to the perimeter defenses, the GehrinZ_ Group employs a number of internal
logical security controls such as anti-virus, intrusion monitoring and prevention systems, and
network monitoring.

9. Confirm that you serve as a consultant or broker, independently, and are not affiliated'
with any insurance company, third party administrative agency or provider network..
Confirmed, Gehring Group serves as an independent broker /consultant and is not affiliated;
with an insurance company, TPA or provider network nor do we own any insurance trusts.
All recommendations are based on the needs of the client and are completely unbiased.
Thus, we can evaluate proposals from insurance carriers as well as entities such as the
Florida League of Cities Florida Municipal Insurance Trust and other health insurance
trusts, As previoqgy stated, our clients are our reatestm reference to our reputation and
integrity-in the public sector.
10 Describe your experience in provider network development, recruitment and negotiation,
and maintenance.
Gehring Group embodies significant technical experience in each of the areas mentioned
above. We have had experience in network development for many of our, clients includin
those located in areas with a history of independent doctors and more rural areas One
specific example of this is the City of Key West. Due to the City's location and lack of
emery medical facilities in the Key West area, Gehring Group acted , as the intermedia
between the City and provider of emergency medical transport services to develop a contract
to provide helicopter trans ort services for City employees ees in emer enc situations.
Gehring Group's analytical team performs network analysis for clients to ensure that the
selected network is providing sufficient doctor- and facility access to all emplo ees The
network review is evaluated in the following manner:
• Access to Care — Gehrin€ Group ensures that employees have adequate access to all
specialties of doctors and hospitals. This access is based on an industry model of the number
of doctors /hospitals within a certain mile radius of the employee zip code base
Subsequently, when transitioning to a new insurance provider, Gehring Group also evaluates
and compares current number of network providers and individual provider utilization over
the rior twelve month pefiod if available to determine the most frequently used providers
to ensure network adettuacy and mitigate member disruption. This analysis is performed
with all prospective carriers so that the City would have the knowledge potential
disruption and member :satisfaction issues that may arise due to a change in carriers.
• Discount Analysis — Knowing that the largest portion of a medical insurance Plogram is
the claims Maid, discount analysis is an integral part of the evaluation process. Since
discounts achieved are critical to the financial health of the plan Gehrana Group evaluates all
carrier /network discounts within our financial analysis to determine which carrier: yields the

highest discount in order to minimize claims impact. We also monitor and evaluate the
discounts expected versus those achieved. Additigadly-.- Gehring Group-D W otiates

performance guarantees with prospective carriers so that if the carrier does not meet the
guaranteed discount threshold, they would be subject pre-negotiated penalties.
11. Outline your ability to provide expertise and experience in the areas of health benefit
plan analysis and design.
We feel Gehring Grouts is best qualified to serve the needs of your group based not only on
our level of experience with provider relations, ,but also due to the level of Practical
experience acquired regarding all types of insurance arrangements. We have extensive
experience with clients that are fully insured and self-insured, clients who offer Health
Reimbursement Accounts (HRA) and Health Savings Accounts (HSAJ, clients insured under
a minimum aremium arrangement, covered under a multiple groug-dsk oql_or consortium,
etc. We were one of the first to successfully implement consumer driven healthcare options
in Florida public, sector entities, the majority of which remain in force jgdU._,-As.previously
stated we base all evaluations on the needs of the client and have the ertise,to review and
make recommendations on all funding arrangements and alternate plan designs. Due to our
focus on the public sector and the large and small employers that we relpresent, we can offer
significant insight regarding plan design and provide information on the outcomes of plan
designs implemented at other public sector entities. Our centralized team has
handled the negotiations with carriers for the various lines of.coverage, noting that we
negotiate the health coverage for all of our public sector clients ,.offering an unparalleled
depth of experience and-insight,-into the products and pricing for the public sector entities.
We have issued RFD's and evaluated proposals for fully insured options versus self-insured
and minimum premium in addition to pooled trust and consortium options.
12. Explain in detail the types of analyses you have conducted relative to benefits analysis
and design for a health plan with at least 100 employees.
Whether soliciting competitive proposals through a formal RFP process, or negotiating a
renewal with a current vendor, Gehring Group performs a detailed analvsis of each program
-37-
which outlines the total cost of, the -program in addition to breaking down the costs related to
employer and employee contributions.
If an REP Vrocess were performed, we would also compare provider networks to determine
which-proposers may be considered viable o Lions for the City in addition to peLfLorming a
network discount analysis. During this stnein the procurement process, Gehdu,,. Group will
schedule a meeting with the CiLy to review our initial findings. Once our analysis has
- ini i
determined which particular vendors are viable, we then attempt to clear up any details that
must be finalized prior to any changes being made. This process is a second level request for
clarification and is developed following the review of submitted proposals. The first draft
review process always produces questions that may not be anticipated and as insurance is
one of the few areas in public entity purchasing regulations where simultaneous negotiations
can take _place, it is always important for the RFPprocess to include best and final responses
within the RFP timeline.
Due to our unique process, providers with whom we work will spend time cgmplling their
best numbers to bring to the table. Due to the cost associated with the preparation of each
carrier proposal, we have found that competitive vendors appreciate the Gehring Group's
approach to this process and, as a result,, we tend to be extremely successful in obtaining the
maximum number of truly competitive quotations from an extensive array of carriers.
After such finalist negotiations and continuous communication with staff will provide
our formal evaluation and recommendation. The City can be confident that all
recommendations will be based on the needs of the entity.
13. Provide examples of communication materials developed and prepared by your
organization for use in City's health benefit communication campaigns.
-38-
14. Are there any existing service provider relationships that may prevent you from acting
independently and providing objective advice and guidance? (Examples include:
overrides, commission agreements, preferred contracts, pricing based on volume, etc.).
No — there are no existing relationships that EgqLd prevent Gehring Group acting-coinpletely
independent Ly and in the best interest of our clients.
15. List any subcontractors who will provide services under this Contract and the services
they will provide. The foregoing list of subcontractor(s) may not be amended after award
of the contract without the prior written approval of the Contact Administrator, to be
designated by the City Manager, and whose approval shall not be unreasonably withheld.
Although Gehring iGroup -anticipates all work related to the scope of services to be for ined
by Gehring Group staff, G
, ehring Group provides COBRA and FSA administration services
through Benefits WorkshW to those clients who do not have a direct contract with another
vendor for these services and wish to take advantave of this free service.
16. List and describe all bankruptcy petitions (voluntary or involuntary) which have been filed
by or against the Proposer, its parent or subsidiaries or predecessor organizations during
the past five (5) years. include in the description the disposition of each such petition.
Not applicable. There have been no bankruptcy petitions filed.
17. Provide specific examples of a significant savings in the cost of benefits to the client that
can be directly attributed to your consultant services.
-39-
Example 1: City of Stuart
Cost Reduction Strategy: Transition from Self Funded to Fully Insured
Gehring Group has also helped clients achieve cost savings by analyzing claims trends of the
specific group and recommending plan changes accordingly. Another related example of
Gehring Group's success in this area is the City of Stuart. Gehring Group was appointed
agent of record for this group in 2003, at which time we began the process of streamlining
the funding and cost tracking of their current self-insured health program. We were able to
pinpoint areas of their program where cost savings were achievable, and direct the RFP
process to ensure these areas were addressed, resulting in plan savings.
Due to the fact that the City had experienced several years of large losses due to high
utilization and catastrophic claims, a self-funded program was no longer proving to be a cost
efficient option for the City to continue to pursue. As a result of the RFP process, the City
took the Gehring Group's recommendation to transition from a self-insured program with a
rented network to a fully insured option with a reputable insurance carrier for the plan year
beginning October 1, 2004. This change was successful not only in simplifying the budget
process and plan administration, but also in reducing claims costs due to deeper discounts
associated with a carrier network versus a rented network as illustrated by the following data.
As of October 1, 2010, the City has been able to maintain a rich schedule of benefits, while
keeping claims costs below the levels of six years prior.
Example 2: Florida Keys Aqueduct Authority
Cost Reduction Strategy: Transition from Rented Network to Carrier Network
The Florida Keys Aqueduct Authority sought the expertise of Gehring Group in an attempt to
find a more cost effective alternative to the indemnity health insurance program in force at
the time. Due to their location in the Florida Keys, there were limited plan options available
to larger employer groups, therefore, they were participating in a pooled program with
several national labor unions in a health plan consisting only of an indemnity option. Based
-40-
on our review and analysis of their program, we were able to propose several
recommendations to accomplish their goal of cost savings while, at the same time, providing
an enhanced health insurance program to its employees.
The first step was to conduct a formal RFP process to ensure that the Authority received the
most competitive options available. After Gehring Group's evaluation of all viable options,
the Authority was able to transition to a fully insured PPO program with a reputable
insurance carrier. Employees benefited from an enhanced schedule of benefits while the
entity benefited from the< significant cost savings. The most notable accomplishment of this
transition is that the Authority was able to enter its fourth year under this program with no
rate increase. In addition, claims costs have stabilized significantly as indicated by the
following data and continue to be less than they were in 2006.

Having served as the Authority's consultant for the past five years, Gehring Group has
culminated numerous success stories including but not limited to
Transitioning from the United Furniture Workers Insurance Fund health plans to a more
cost effective health plan with Blue Cross & Blue Shield for the October 1, 2006 renewal.
This plan provided a more comprehensive schedule of benefits while reducing health
insurance premiums by 11.4 %.
Subsequently transitioning to a "participating" contract allowing the group to take
advantage of profitable claims years, and receiving a refund over $150,000 for its
2007/2008 plan year.
Successfully renewing the Authority's health insurance plan for two additional years with
no premium increase.
• Implementing a Flexible Spending Account program with debit cards and educating
employees on the benefits and tax savings associated, while generating FICA savings of
over $8,000 per year for the Authority.
-41- -1,.-,---,-,
® Educating employees on Medicare and Medicare D prescription drug benefits,
• Advocating for employees regarding various claim issues, helping to bring them to
resolution.
• Facilitating network development for specialized services in which the number of
Participating providers were limited in the Keys area (i.e. laboratory and surgical center
facilities, emergency air transport).
• Implementing BenTek@, a paperless online open enrollment system.
0 Providing an annual Employee Benefits Booklet summarizing all employee benefits
insurance coverages offered by FKAA to its employees at no additional cost.
Example 3: City of Vero Beach
Cost Reduction Strategy: Transition from Fully Insured to Self Funded
The City of Vero Beach has successfully maintained a comprehensive health insurance
program consisting solely of a PPO offering for over 30 years. Numerous factors have
contributed to their ability to maintain such a rich plan including location, demographics,
claims experience, group claim trends, etc.
Funded under a fully insured arrangement for over 30 years, Gehring Group recommended
that the City allow us to conduct an analysis of the group's claims history versus its annual
renewal increases. The results of our analysis indicated that the group had the potential to
save considerable money and better manage its renewal costs if they transitioned to a self
funded arrangement. This strategy has paid off significantly for the City by enabling them to
stabilize their renewal increases to a level that is tolerable year after year, and most
importantly, predictable, under current budget conditions. The following chart illustrates
how the City's health insurance renewal increases have stabilized under Gehring Group's
recommendation to self insure,
During the period of 1998 through 2001, renewal increases were erratic based on whether the
group had a "bad claims year." By maintaining adequate plan reserves and funding the
program at reasonable levels, we were able to better control renewal increases. Gehring
Group monitors the City's program and all factors involved on a continual basis to maintain
the rnost cost effective option for the City of Vero Beach.

conduct internet enrollments, and provides employees with internet access to an "Employee
Benefits Center" help site. BenTek has since become its own entity with its own President
