Section II: Qualifications and Experience
Understanding of Scope of Work and Approach
Bolton Partners has read and understands the Scope of Work for RFP No. SM- 2011 -08 -HR
hrsurance Brokerage Services. We are in the business of providing professional expertise and
assistance to clients, especially Public Sector clients. This proposal will outline and present
Bolton Partners' qualifications to provide Insurance Brokerage services to the City of South
Miami. We are very familiar with the unique challenges affecting Public Sector Employers and
have assisted our other Public Sector clients with the strategies necessary to combat budget cuts
while maintaining competitive and contemporary benefit plans. We will address each of the
tasks required in this proposal.
SCOPE OF SERVICES
The City of South Miami seeks the services of qualified firms to provide Insurance Brokerage
Services for the City's Employee Benefit Plans. The City of South Miami has approximately 142
employees and retirees participating in the benefits plan. All employees are located in Florida
and retirees may be elsewhere. The City's plan year is October 1 st through September 30 "i.
The benefit plans include the following, but are not limited to:
• Health Insurance
• Dental Insurance
• Life Insurance
• Long Term Disability
• Short Tenn Disability
• Group Life and AD &D
• Insurance Supplements
• Vision
Bolton Partners' consultants are experienced in all aspects of the above plans and we provide
health and welfare consulting services to over 35 public sector plans. All of our Health &
Welfare consultants are experts in the above fields. We have relationships with carriers /vendors
and industry leaders that provide all of the above services. Our experience and client base
enables us to negotiate competitive programs on behalf of our clients. Kay Moran, the lead

consultant, has been in the insurance and consulting industry for over 30 years and is a Certified
Employee Benefits Specialist.
This section outlines the minimum services that the City expects to receive fi-om a qualified
Consultant as a subject matter expert. Said Consultant shall assist in the design and
implementation of a comprehensive employee benefits program for fiscal year 2012 and beyond.
ON -GOING SERVICES:
Expected deliverables include, but are not limited to:
A. Monitor the programs' operations throughout the year to ensure that benefit providers are
meeting all customer service requirements and standards. '
Bolton Partners will work with the City and the City's vendors to make sure all the benefit
programs are meeting all of the service and performance standards established for the plan
year.
B. Provide on -going administrative support, as required, by acting as a liaison between the
City and providers to assist with resolving claim disputes contract administration and
interpretations, and other issues.
Bolton Partners will assist the City with all claim, contract and service issues. We will work
with the carriers to resolve disputes and make recommendations on how issues can be
resolved in the City's best interest.
C. Provide dedicated personnel as a primary contact for managing the account relationship
with the City.
Kay Moran will be the City's lead consultant. Kay has over 30 years of group insurance and
consulting experience. Kay is a Certified Employee Benefits Specialist (CEBS) and past
president of the CEBS Chesapeake Chapter. As noted in Kay's resume she has extensive
experience in the Employee Benefits field. Her tenure with several insurance carrier's (UHC
and CIGNA) is invaluable in understanding how costs are determined and how plans are
administered. She also has significant consulting experience with public and private sector
employers. Her past experience is what sets her apart from the competition. She has
expertise in all aspects of group insurance noted above including underwriting, claim
analysis, funding, legislative compliance and negotiating with vendors. She works tirelessly

to get the best deal for her clients and she is creative in coming up with innovative and cost
effective solutions.
D. Meet with the City's Human Resources Director throughout the year as reasonably
necessary (minimum is quarterly).
We will meet with the City's Human Resource Director at least quarterly to discuss
employee benefit trends /norms and review all aspects of the benefit plans. In addition, we
will ask the carrier representatives to attend some of the strategic sessions where we will
review utilization reports and compare the City's utilization with peer groups. The strategy
sessions will include a list of short term as well as long -term goals of the programs. Our
quarterly running trend reports also provide the necessary data to forecast program costs and
identify trends.
E. Coordinate annual audits of City's benefit plans and associated vendors and prepare annual
financial reports on the results of the completed plan year.
Bolton Partners will support the City with regard to employee benefit claim audits. While
our firm does not conduct the audits, we have worked with other firms that are very
experienced in conducting audits. We can provide specifications for a Request for Proposal
for an auditing firm and can act as the project manager for the task.
F. Prepare and deliver any necessary reports to the City's Human Resources Director,
including but not limited to reports showing claims experience at intervals acceptable to the
City.
We provide all of our health and welfare clients with monthly or quarterly budget reports that
monitor costs on an on -going basis. We will provide a quarterly budget variance report that
will help monitor, for the current plan year, whether the rates set were appropriate as
compared to actual results. The quarterly trend report will track actual healthcare costs
(claims) per person on a rolling quarterly and annual basis. This is valuable in the annual
renewal process in terms of helping to decide which trend rate(s) to use for rate setting and /or
negotiating with the carrier(s). It is also valuable as an early indicator of changes in
utilization trends.

G. Provide advice and assistance in the 'review of the City employee health and medical
benefits program on a continuing basis to ensure that those plans are in compliance with
state /federal requirements and their adequacy of benefits with respect to other plans.
BP will provide a monthly summary of legislative updates. We provide this regularly to our
clients in a newsletter. Each week, BP conducts an internal technical research committee
meeting where we review weekly and monthly periodicals for any pending legislative
changes. We also have a client portal where updated Legislative Bulletins are posted weekly.
H. Track monitor and provide information on changes in, or any pending or new legislation
in the applicable state and federal laws, as well as any employee benefit and funding trends
that may affect the benefits program, to the City's Human Resources Director.
As noted in our response to item G above Bolton Partners will keep the City apprised of any
changes in legislation that will impact the City's plans. The City will also have access to our
client portal
L Advise and assist the City as requested with:
• Writing employee benefits plan modifications and /or new benefits plans and any required
amendment approval process;
• Submission of written reports and other documents as required by the state and /or federal,
government;
• Coordination of the annual employee wellness fair.
• Development of an Employee Wellness Program.
We assist our clients with communication pieces as part of our routine consulting services.
We also have used our partners to assist with customized communication pieces. For
example, we wrote the initial Employee and Retiree enrollment guides for Howard County
Public Schools and their vendor, Blue Cross Blue Shield, formatted and printed the guide as
part of their administration fee. The guides are updated annually with any benefit or
legislative changes.
We will assist the City with completing any annual reports or drafting plan documents as
required by state or federal laws. We will also work with the City and their vendors to
establish wellness programs, which will include an annual wellness fair.

J. Perform special projects as requested by the City, including but not limited to:
• Development and assistance in the implementation of new insurance plans;
• Assistance with adjudication of specific claims as requested by the City;
• Recommendation of alternative benefit designs or delivery systems as dictated by
emerging plan costs for benefit practices.
Bolton Partners will act as an extension to the City Benefits staff. We will continuously
monitor the financial aspects of the plans, negotiate with vendors, meet with the city to
develop long and short term strategies. We will ensure the City is in compliance with all
Federal and State regulations. We have made several presentations to our clients with regard
to Health Care Reform. We continuously attend meetings and seminars on new
developments and strategies in our industry so that we can share those with our clients. We
will assist with all claim problems as needed.
K. Ensure personnel availability for meetings, phone calls, and e -mail correspondence as
required.
We will provide the City with weekly status updates via email and monthly summary reports.
The City will receive updated status reports for all projects which will include changes and
additions to all tasks. Our personnel will be available for meetings and phone calls and will
respond timely to email.
L. Maintain confidentiality of City records and data in accordance applicable federal and
state laws.

RENEWAL YEAR SERVICES:
A. Using current health and medical benefit plans as benchmarks, research, design, and
propose employee benefit plans for the City, as appropriate.
We constantly review and make recommendations for plan design changes for all of our
Health and Welfare Clients. We pride ourselves on staying ahead of the curve in offering
new benefits while capitalizing on the cost effective strategies that have worked well in the
past. We will provide an overall analysis of all the benefit programs offered at the City. We
will incorporate a comparison with other public sector entities in the analysis to see where
adjustments can be made to ensure the City benefit programs remain comprehensive and
competitive. We will include the clinical analysis of the utilization data to see where
enhancements to the disease management programs and employee health and wellness
programs are appropriate based on the disease states of the current population.
BP has a tenured staff of Benefit Sr. Analysts that stay informed of new benefit trends. We
will make recommendations and provide education regarding the trends. For example, we
have recently offered a High Deductible Health Plan with a Health Savings Account to
several of our Public Sector clients. It is our approach to offer this plan as an option to the
medical plans. It is important to first educate the benefit's staff on these types of consumer
programs and then communicate with and educate employees. The communication
campaign will include written materials with examples as well as in- person open enrollment
and educational meetings. In the interactive sessions, employees will be given specific
examples of how the benefit programs differ so that they can determine based on their needs,
which option fits them.
B. Meet with the City as necessary to discuss benefit plan options and establish goals and
objectives for the City's benefit programs.
Bolton Partners will conduct an overall analysis of the current benefit plans in place after our
initial kick off meeting. This will enable us to compare the goals of the programs to the
actual plans available in the market and the City's current financial position. We will be able
to provide a fresh look at the plans, make observations of the plans and recommendations for
alternatives. Our consultant's work with our technical analysts to review the current program
costs, benefits, administrative processes and communications materials. We then developed
a report that outlines recommended changes along with estimated costs for the proposed
program. Our recommendations and reports are peer reviewed by other BP consultants
before they presented to the client.

C. Provide analysis of current plans, including the review of past performance, with regard to
renewal.
Although there are specific issues that differ each year, in general we perform the following
functions for all our Health and Welfare clients as pant of our renewal services: projecting
costs for the upcoming plan year, including setting rate equivalents, COBRA rates, employee
contributions, and overall costs for budgeting purposes. We also review proposed renewal
administrative fees and stop -loss fees /limits for appropriateness and negotiate on behalf of
our clients as necessary. This will include an analysis of the historical large claims to
determine the appropriate reinsurance or pooling levels.
Bolton Partners understands the relationship with insurance companies and vendors. We
have strong partnerships with these organizations and do not take the "strong arm" approach
to negotiation. We use a fact -based position. This will include using competitive information
from other carriers /clients to negotiate fixed fees and costs. Our consultants have strong
underwriting and actuarial backgrounds that allow us to illustrate other approaches to
developing administrative fees and fixed costs and claim projections. We negotiate on behalf
of all of our clients. Recent negotiations include Frederick County Government, City of
Frederick and Town of Ocean City.
Each year as part of the planning process we establish target dates for all consulting tasks
including renewals. Our timeline ensures we have ample time to evaluate plans, negotiate
rates and work with our clients to meet budget and employee communication deadlines.
We will provide a comprehensive annual renewal report that includes all of the items
required above. This report will provide an Executive Summary of the recommendations and
supporting exhibits as documentation.
D. Review additional available cost savings plan alternatives and creative funding options.
As noted above, this is part of our annual renewal services.
E. Determine the appropriate employee and employer benefit contribution levels.
Bolton Partners will work with the City to develop an employee contribution strategy that
meets their benefit objectives. We will review the current contribution levels and make
recommendations for adjustments that will meet the City's long and short-term cost sharing
goals.

F. Review and recommend annual contribution strategies for active participants and retirees.
As noted in our response above, we will work with the City to develop an employee and
retiree cost sharing strategy to meet the City's benefit goals. We will review the
appropriateness of the cost sharing strategies each year during the renewal process.
G. Provide City with information on what other municipalities of comparable size and
location will be doing with their benefits in the upcoming.years.
Bolton Partners' public sector experience and client base is a good resource for
benchmarking comparisons. We have also conducted employee surveys on behalf of our
clients. We have the ability to conduct paper surveys or (our preferred method) on -line
surveys. We work with our clients and develop surveys that are quick and easy for
employees to respond to. We provide written and oral reports that provide survey results and
also outline our recommendations.
H. Conduct renewal negotiations and develop appropriate information for management
purposes.-

I. Upon City's request, coordinate a comprehensive "Request for Proposal' (RFP) process to
identify potential high quality Benefit vendors, according to established City guidelines. The
scope of the RFP may include but not limited to: Medical, Dental, Vision, Basic Life,
Voluntary Life, Accidental Death and Dismemberment, Short Term and Long Term
Disability insurance providers.
Bolton Partners has extensive experience in developing and evaluating RFP's on behalf of
our public sector clients. We represent our customers in the marketplace on a regular basis.
We will prepare detailed specifications for any line of coverage as appropriate. Over the
years we have developed a core set of specifications for each line of coverage. The core set
would be expanded to fit the needs and goals the City. We routinely work with Purchasing
Departments during the RFP process.
We have developed a 100 -point evaluation model that has served our clients well. It points
bidders to important areas and provides a documented and supportable selection process.

The evaluation criteria, including the relative point value assigned to each area of review, are
different for each of our clients and are tailored to their particular goals. After the evaluation
process, we will manage the finalist interview process by providing the score sheets and
finalist interview questions. We also obtain best and final pricing and make
recommendations to the City based on our RFP analysis and the finalist interviews.
We have completed recent RFP evaluations for Carroll County Government, Frederick
County Government and Frederick Community College.
J. Act as lead negotiator and consultant to the City during benefit contract negotiations and
renewals.

the current to new vendors, the renewal proposal, and other benefit changes.

routine consulting services. We also have used our partners to assist with customized
communication pieces. For example, we wrote the initial Employee and Retiree enrollment

printed the guide as part of their administration fee. We also have a graphic designer on staff
to assist with communication pieces. We have included a Bolton Partners' brochure in the
attachment section that was designed and written by our office staff. The guides are updated
annually with any benefit or legislative changes.

We will draft and review all open enrollment materials and we will conduct the information
sessions in conjunction with the City's staff and insurance carrier representatives.

P. Assist City with the development of performance guarantees relating to vendors'
performance of services 'to the City, and evaluation of the performance of vendors.
We will develop performance standards for each carrier and vendor to ensure the City is
receiving the services contracted. We will monitor the standards on an ongoing basis and
assist the City in documenting any unmet standards that will result in reduced costs to the
City.

anti -virus and a Cisco ASA 5505 firewall Our servers are locked in a Dell Server rack and
separated in a locked and environmentally managed data room.

Bolton Partners was hired to conduct a Medical Drug and Dental Marketing for Carroll County
to the County Commissioners. The County ecided to change carriers from CareFirst Blue Cross
and Blue Shield, carve out the Drug program and change carr iers for the dental PPO plan; it kept
the DHMO with CIGNA. The annual savings as a result of the changes were $570,000 on the
Medical $524,000 on the switch to Medco for the drug, $325,000 savings for plan design
changes to the drug plan and a $35,000 reduction to the Dental PPO costs. This project was
completed in October, 2009. BP conducted all the Marketing and assisted with the
imDlementation as part of a fixed fee arrangement. _

Kay takes the lead consulting role with Howard County Government and Schools, Frederick

In 2007, we helped implement Flexible Spending Accounts and worked with the payroll
department and finance to implement this tax savings vehicle. In 2010, the Town was faced with
a very large rate increase (30 %). Bolton Partners worked with CareFirst (their current carrier) to
negotiate a 5% reduction to the renewal (25% over current). BP conducted a bid and received
two competitive proposals. We asked all three providers to submit their best and final proposals
and CareFirst dropped their renewal to an 18% increase. This negotiation saved the Town of
Ocean city nearly $1,000,000. We also assist the Town with COBRA and all aspects of their
benefit programs. We recently completed a dependent eligibility verification audit and are
working with the City to finalize benefit changes for 2012.
