b. At the time of permitting, an application for Special Inspector will be submitted to the Building Officialfor
approval of employment as Special Inspector under the specific requirements of the 20]7FBC.
c. If the owner and/or contractor elect tm employ U.3. Structures, Inc. to perform the required Special
inspections, please contact this office at (305) 665-4555 prior to commencing the work. At that time, USSI will
issue on inspection plan with speoifioconditions that will ba required for this project.
d. In order to assure timely and appropriate inspections, the contractor shall notify the Special Inspector of
construction schedules and planned operations a minimum of24houn prior hmthe time ofinspection.
Actual date and time of site visit will be scheduled by the Special Inspector at that time.
e, At the time of inspection, the contractor shall assure that the approved set of permit drawings, including
approved revisions, approved shop drawings, and RF|'s are present ot the site. Inspections will not be
performed without the proper documentation.

a. The contractor shall submit to the architect complete shop drawlings as required by these notes.
b. Review and approval of shop drawings is for the design concept only. The contractor shall check and
approve the shop drawings for compliance and completeness before submittal. Approval by the engineer
or architect is not for the purpose of approving changes or substitutions. The contractor shall be responsible
for all dimensions, quantities, job conditions, and coordination wiith all documents and other trades.
c. Submit a minimum mf four (5) copies for review.
d. Allow a minimum of (7) business days for the review nyshop dnawiings,

b. All concrete shall be proportioned to attain a minimum compressive strength of 3000 psi at 28 days. Slump
shall be4"(+l^) and no water shall be added tm the concrete ot the site. Maximum water-cement ratio shall
be 0,40. Concrete for areas, which will remain exposed to weather after construction, shall have 3 to 5
percent entrained air.
o. The contractor shall contract onindependent testing laboratory h» perform the concrete cylinder tests oo
required by section \923.2of the F.B.C'
d. Contractor shall provide all forming and temporary shoring. Form chairs shall bo used throughout. Use of
debris or bricks is not acceptable. Plywood used for from work shall not be used for finish roof orfloor
sheathing.
e. Reinforcement: ASTMA6l5'grade 6U'f\STWAl8S for welded wire fabric. Splice all column bars tmdowels
minimum 3O bar diameters /u.o.n>, Splice #5 bars in grouted cells a minimum of36inches.
f. Concrete cover over reinforcement:
l) 3-inuhes where cast against earth.
2) 2-inches for columns and beams and 1-1 /2 inches for slabs where cast in forms or on polyethylene sheet
and permanently exposed tV weather orearth.
3) l-l/2 inches at all other locations (u.o.n.).
g. All concrete shall be finished as appropriate for the finishes indicated in the architectural drawings.
h. All concrete shall be cured for minimum of seven days. The curing shall entail maintenance cf the moisture in
the concrete. Generally this shall be accomplished by treating exposed concrete surfaces with a chemical
curing compound immediately after finishing and immediately after removal of forms. Forms shall be kept
moist by frequent water spraying prior toremoval. Verify compatibility of curing compound with proposed
finishes.

members, End joints shall occur at center of p6mary framing member w1ith both panels fastened to it. End
joints shall bestaggered.
d. Fasten plywood roof sheathing panels to all supporting members using 10d nails. Nail spacing shall be as

a. Prefab6cated wood trusses and girders shall be designed in accordance with ASCE 7-05 [building category 2;

c. Prefabricated wood trusses and girders shall be designed with a maximum steel plate increase = 1.00 and a
maximum lumber stress increase =l.33.

each other. Design of bracing for individual truss or girder members to resist lateral buckling under gravity or
