GENERAL NOTES: ENGINEER NOTES:

2. Elevations shown are based on N.G.V. Datum (1929). Public Works De artments and 0 E R M Standars & S ecifications
3. Underground utility information is taken from the best available sources,
but should be verified by contractor in the field with the appropriate
utility agency prior to commencement of any work. Existing utility lines
other than those indicated in this drawing may be on site. The contractor
is warned to proceed with caution with all work. He is to make all possible
investigation as to possible unmarked utility lines.
4. Contractor shall adjust any utility lids and /or covers in project area to
the finished grade.
5. Any apparent discrepancies in the plans and field condition shall be brought
to the attention of the Engineer before proceeding with the work.
6. Contractor shall notify the Owner's representative and Engineer if soil
condition encountered is unsuitable for construction.
7. The Contractor shall restore all areas disturbed by this construction to
a condition equal to, or better than, that now existing.
B. Pavement restoration as per City of South Miami Public Works Department
Standards and Specifications.
9. Erosion and Sedimentation Best Management Practices (BMP's) shall be
incorporated during construction. All runoff shall be contained on site.
If discharges must occur, hay bales shall be incorporated at all discharge
points. Silt screens shall be placed around all disturbed areas.
10. The sub -grade shall be stabilized to a minimum LBR of 40.
11. The sub -grade shall be processed in one course to a depth of 12" below
the lime -rock base material. The sub -grade shall be compacted to at least
98 percent of the modified proctor maximum dry density (ASTM 0- 1557).
12. The limerock base shall be compacted to a density of not less than 98 percent
of the maximum density obtainable under AASHTO Specification T180 -74, and shall
be a minimum depth of 6" after compaction.
13. Asphaltic concrete mix shall be central plant hot mix, and shall be FOOT Type de S -1.
14. The pavement shall be 1" thick minimum after compaction, constructed
in one course, compaction shall be to 98 % of design density as determined by
ASTM D1559 -78 test method for Resistance Plastic Flow of Bituminous Mixtures
using Marshall Apparatus.
15. Bottom of perforated pipe shall be at minimum elevation of 3.0' N.G.V.D. as per WC -2.2
16, Miami -Dade County Flood Criteria: 6.30 N.G.V.D,
UNDERGROUND CONTRACTOR:
A. Underground Contractor shall comply with the Trench Safety Act, HB 3183, Florida
Statutes, which incorporates, but is not limited to, the following:
1. OSHA Standard 29 C.F.R. Part 1926, Subpart P, Excavation and Trenches.
2. Contract bids for trench excavations (in excess of 5 feet deep) shall include
reference to the safety standards, written assurance of compliance, and a
separate item identifying the cost of compliance.
3. Adhere to any special shoring requirements of the state or other governmental
subdivision.
B. Engineer is not responsible for the safety of the Excavation or Design and
Construction of any shoring.

2) These plans has been prepared with the best information available
to the engineer. The Engineering Contractor MUST notify the "UNDERGROUND
UTILITIES NOTIFICATION CENTER OF FLORIDA", Phone No. 1- 800 -432 -4770, and
the Florida Power & Light Co., 48 hours prior to start of excavation and /
or construction. `If any discrepancies are encountered to the ex end that
a "Field Revision" is required, Engineer must be notified immediately.
3) Deviations from the approved plan shall not be permitted by th� Engineering
Contractor without the written consent of the Engineer of Recold, and the
City of South Miami and Miami -Dade County Public Works Department.
4) All elevations refer to the National Geodetic Vertical Datum.
5) All existing structures and utilities shall be protected at all times during
excavation and construction. Neither the Engineer or the Owner / Developer
shall be responsible for damages resulting from the Engineering Contractor's
work. All existing pavement, utilities (above and below ground), or surface
features disturbed during construction shall be restored to equol or better
condition by the contractor in accordance with the City of Miami -Dade County
Public Works Department Standards and Specifications or as directed by the
Engineer / Inspector.
