1. Call on plans type of lab being proposed i.e.: blood drawing and specimens collection to be sent out for test at a
third party company, complete medical tests, etc. Provide details on plans and modify accordingly.
If the Lab will be used to perform medical tests then (2 -4, 10 and 11):
2. List on plans all fresh products to be used in the Lab Room such as: reagents, neutralizers, etc. Indicate size and
amount of the containers and show on plans the storage location. Provide details and modify plans accordingly.
3. List on plans all waste generated for the Lab Room by the process /cleaning operations such as: waste chemicals,
spent solvents, etc. and indicate method of disposal and storage. Indicate size and amount of the containers and
show on plans the storage location. Provide details and modify plans accordingly.
4. Characterize the wastewater being discharged from the Lab Room shown on plans. Clarify whether or not any
waste chemical or wastewater in contact with chemicals will be discharged into the sanitary sewer system from Lab
Room sink. Wastewater shall meet Miami -Dade sanitary sewer discharge standards as per section 24 -42.4 of
Miami -Dade Code. Provide details.
5. Indicate on plans how the images taken by the x -rays equipment will be stored (electronic records, films, etc.),
processed and developed (films, computer monitors, printed out using laser printers, etc). Specify on plans whether
or not images will be wet processed and developed onto films. Provide details on plans and modify accordingly.
If images taken by any of the imaging equipment (dental and /or medical) will be wet processed and developed onto
films then (6 -11):
6. Show on plans the location of the film processors and call on the drawings the manufacturers name and model
number of it, if any. Provide details and modify plans accordingly.
7. Provide the manufacturerGs specifications for all proposed imaging film processor equipment being proposed.
8. Provide plumbing plans showing the interconnection of the film processors (wash, fixer and developer replenish
and waste tanks). How will the silver bearing waste be transferred, stored, treated, processed and /or disposed of.
Modify accordingly and provide details on plans.
9. Indicate on plans size of the containers of the fresh (unused) film processing chemicals (fixer, developer, etc.) and
the film processor waste: waste developer, waste wash and fixer waste. Show on plans storage location for the
unused processing chemicals and film processor waste. Modify accordingly and provide details on plans.
10. Make provision to provide the fresh (unused) chemicals (film processing chemicals, lab chemicals, lab reagents,
etc.) and waste from the film processors and lab with secondary containment, for containers 55- gallon or greater.
Secondary containment must specify containment dimensions, material of construction (high density polyethylene,
stainless steel, etc.), size of the storage containers and amount of the containers that will be held. Secondary
containment must be capable of containing the spillage of 110% of the largest storage container. Show volume
calculations and details on plan.
11. Provide an extra set of signed, dated and sealed plans to be retained by DERM for the operating permit file just
with the following pages: SP -1, A -2, MP -1 and all modified pages related to the above listed items.
