13-185-036GENERAL NOTES- 1. CONSTRUCTION ACTIVITY TYPE 'D', POPULATION RISK GROUP 1, CLASS III/IV 2. OBTAIN INFECTION CONTROL PERMIT BEFORE CONSTRUCTION BEGINS. 3. ENSURE INFECTION CONTROL PRACTITIONER CONSULTATION HAS BEEN COMPLETED AND INFECTION CONTROL MEASURES HAVE BEEN APPROVED. 4. ERECT AN IMPERMEABLE DUST BARRIER FROM TRUE CEILING (INCLUDES AREA ABOVE FALSE CEILINGS) TO THE FLOOR CONSISTING OF 2 LAYERS OF 6 ML POLYETHYLENE OR SHEETROCK, THAT INCLUDES AN ANTEROOM. 5. CONSTRUCTION WORKERS SHALL LEAVE THE CONSTRUCTION ZONE THROUGH THE ANTEROOM SO THEY CAN BE VACUUMED WITH A HEPA FILTERED VACUUM CLEANER BEFORE LEAVING THE WORK SITE. 6. ENSURE WINDOWS, DOORS, PLUMBING PENETRATIONS, ELECTRICAL OUTLETS AND INTAKE AND EXHAUST VENTS ARE PROPERLY SEALED WITH PLASTIC AND DUCT TAPED WITHIN THE CONSTRUCTION/RENOVATION AREA. 7. CONSTRUCTION WORKERS SHALL WEAR PROTECTIVE CLOTHING THAT SHALL BE REMOVED EACH TIME THEY LEAVE THE CONSTRUCTION SITE BEFORE GOING INTO PATIENT CARE AREAS CONSTRUCTION WORKERS SHALL WEAR SHOE COVERS WHICH SHALL BE CHANGED EACH TIME THEY LEAVE THE WORK SITE. 8. DO NOT REMOVE DUST BARRIER UNTIL THE PROJECT IS COMPLETE AND THE AREA HAS BEEN CLEANED THOROUGHLY AND INSPECTED. 9. REMOVE DUST BARRIER CAREFULLY TO MINIMIZE SPREADING DUST AND OTHER DEBRIS PARTICLES ASSOCIATED WITH THE CONSTRUCTION PROJECT. 10. MAINTAIN NEGATIVE PRESSURE WITHIN THE CONSTRUCTION ZONE. 11. ENSURE AIR IS EXHAUSTED DIRECTLY OUTSIDE AND AWAY FROM INTAKE VENTS OR FILTERED THROUGH A HEPA FILTER BEFORE BEING RECIRCULATED. 12 ENSURE VENTILATION SYSTEM IS FUNCTIONING PROPERLY AND IS CLEANED IF CONTAMINATED BY SOIL OR DUST AFTER CONSTRUCTION OR RENOVATION PROJECT IS COMPLETE. 13. REMOVE DEBRIS AT THE END OF THE WORK DAY. 14. VACUUM WORK AREA WITH HEPA FILTERED VACUUMS DAILY OR MORE FREQUENTLY IF ADJACENT TO THE CONSTRUCTION AREA 15. IN COLLABORATION WITH ENVIRONMENTAL SERVICES, ENSURE THAT CONSTRUCTION ZONE IS THOROUGHLY CLEANED WHEN WORK IS COMPLETE. 16. COORDINATE WITH ENVIRONMENTAL SERVICES TO INCREASE THE FREQUENCY OF CLEANING IN AREAS ADJACENT TO THE CONSTRUCTION ZONE WHILE THE PROJECT IS UNDERWAY. 17. IN COLLABORATION WITH INFECTION CONTROL. ENSURE THAT CONSTRUCTION ZONE IS THOROUGHLY CLEANED WHEN WORK IS COMPLETE. 18. COORDINATE WITH INFECTION CONTROL TO MOVE HIGH -RISK PATIENTS WHO ARE IN OR ADJACENT TO THE CONSTRUCTION AREA 19. REPEATEDLY INSPECT DUST BARRIERS 20. IN COLLABORATION WITH THE FACILITY PROJECT MANAGER, DESIGNATE A TRAFFIC PATTERN FOR THE CONSTRUCTION WORKERS THAT AVOIDS PATIENT CARE AREAS AND A TRAFFIC PATTERN FOR CLEAN OR STERILE SUPPLIES THAT AVOIDS THE CONSTRUCTION AREA 21. PREVENT STAGNANT WATER. 22 COORDINATE WITH MEDICAL AND NURSING STAFF TO ENSURE THAT PATIENTS DO NOT GO NEAR THE CONSTUCTION AREA 23. HOLES IN WALLS SHALL BE REPAIRED OR TEMPORARILY SEALED. 24. ISOLATE HVAC SYSTEM IN AREA WHERE WORK IS BEING DONE TO PREVENT CONTAMINATION OF THE DUCT SYSTEM (EXISTING TO REMAIN). 25. ENSURE VENTILATION SYSTEMS ARE WORKING PROPERLY IN ADJACENT AREAS 26. ENSURE NEGATIVE PRESSURE IS MAINTAINED WITHIN THE ANTEROOM AND CONSTRUCTION ZONE. 27. REVIEW VENTILATION SYSM REQUIREMENTS IN THE CONSTRUCTION AREA WITH INFECTION CONTROL PRACTITIONER TO ENSURE SYSTEM IS APPROPRIATE AND IS FUNCTIONING PROPERLY. 28. REMOVE CONSTRUCTION DEBRIS DAILY. CONTAIN CONSTRUCTION WASTE BEFORE TRANSPORT IN COVERED CONTAINERS 29. REVIEW INFECTION CONTROL MEASURES WITH ENGINEERING STAFF, MAINTENANCE STAFF, ENVIRONMENTAL SERVICES STAFF AND MEDICAL/NURSING STAFF TO EVALUATE EFFECTIVENESS AND IDENTIFY PROBLEMS DURING AND AT THE END OF THE CONSTRUCTION PROJECT. 30. COORDINATE WITH INFECTION CONTROL PRACTITIONER TO REGULARLY VISIT THE CONSTRUCTION SITE TO ENSURE PREVENTATIVE MEASURES ARE BEING FOLLOWED. 31. IN AREAS REQUIRING SPRINKLER SHUTDOWN, PROVIDE A FIRE WATCH IN AREAS OF CONSTRUCTION DURING SHUTDOWN PERIODS. COMPLY WITH FBC 419.4.210.
