1. Approval granted for the interior renovation of an existing building at the above
referenced location to accommodate the operations of a dental office.
2. Operations at the aforementioned office will include a film processing unit,
associated to the X-ray equipment on premises. The chemical waste generated from
the operation of said unit will be stored and hauled off site by an authorized hauler,
as per plans and letter from Dr. Faine dated November 15, 2001 attached thereto.
3. As a condition of this approval, chemicals/wastes associated to the film processing
operations, must be stored on an impervious surface with secondary containmen,
and covered area away from floor drains or other drainage structures.
4. Receipts from all waste disposal must be maintained at the business and be
available for inspection by DERM personnel for a period to be established by the
ro required Industrial Waste Annual Operating Permit (IW5).
There shall be no discharges from the aforementioned film processing unit to the
sanitary sewer system. Installation of a silver recovery unit as well as a I ny
