WHEREAS,East Greenway Alliancehas submitted a Special Event Application to holdits First-Ever Bike-ln South Miami Street Bash(Dorn Avenue from Sunset Drive to 73rd Street and 73rd Street from 59th Avenue to 58th Avenue)and History Tour Ride through South Miami presented by WHEELS November 14 and 15,2015;and WHEREAS,East Greenway Allianceis requesting a waiver for the cost of 63 parking meters in the amount of $1,575 ($25.00 fee per meter X63 total parking meters);and WHEREAS,the applicant is requesting a waiver for the cost of Police presence for the Street Bashon Saturday,November 14 and Police escorting of the History Tour Ridein the Cityof South Miamion Sunday,November 15,2015 totaling in the amount $7,260;and WHEREAS,the applicant is requesting a waiver for the street closure fee in the amount of $286.25;and WHEREAS,the applicant is requesting a waiver for Public Works presence,fees,and barricades totaling in the amount of $1,357.45;and WHEREAS,the applicant is requesting a waiver for the application and administrative fees in the amount of $160;and WHEREAS,the applicant is requesting a waiver for the street banner fees in the amount of $150. PERMIT PROCESS:Please complete the following formsin their entirety and submit them tothe Parks &Recreation Department Monday through Friday,8:00am until 5:00pm,to the attention ofthe Special Events Coordinator.The permitapplication process begins with the receipt ofacomplete application and payment of the nonrefundable SpecialEventApplication fee of $60.00.Delaysin providingailof the required items to the Special Event Committee may holdup the review process. Applications must be submitted a MINIMUM OF21 DAYS before the scheduled event.Any application submitted with less then 21 days priorto the event may not receive the necessary approvals from the required departments priorto the date ofyour event.An application with less than five(5)business days priorto an event WILL NOTBE ACCEPTED. SECURITY DEPOSIT:Upon applicationapproval,a $600 security deposit,as well as other possible chargers (such as:Street Closure,OffDuty-Officers,etc.)will be due and required before any permit canbe issued.If the event occurs withoutanycity damages and/or charges,$500 of the deposit may berefunded;$100will be retained by city for administrative cost HOURS:Event activities are prohibitedafter 11 p.m.in residential areas,after midnightincommercial areas,and before 8a.m.in any area.Loudspeakers,amplifiedmusic,bullhornorpublic address systems (PSA)during event hoursare strictly regulatedbytheCodeDepartment. EVENTS WITHINA PUBLIC RIGHT-OF-WAY:Events may be heldonpublicor private propertyas well as public rights-of-way such as:streets and sidewalks in commercial,residential,orindustrial zoned areas.Events on streets and sidewalks in commercial,residential,or industrial zoned areas may have special requirements and limitationson the use of certain streets to ensure safe and adequate trafficflowto areas near thespecialevent.Any special event that impacts traffic or circulation inthe area throughlaneclosures,orothermeans,will requireadditional planning and impactanalysis from ourPublicWorksandPoliceDepartments. m INSURANCE:All special events,especially events taking place on City property require the event holdertocarry liability insurance fortheirownprotection as well as insurance protection for the City. The event producer shall,priorto the event,submitaCertificateof Insurance showing the required insurance limitsof$1.000,000 and ineffect coverage;the Cityof South Miami must belisted as an additional insured in the amount of $1,000,000 are required prior to the event. SMOKING:Smokingisnot permitted inanypublic facility. EVENT NARRATIVE:A detailed written description/summary of your event is required.Please describe all activities,a schedule ofactivities,inventory of any sold and/or give-a-way items,listof vendor and any other important information. Background: There are two types of "event liquor liability,"one is "retail included"liability which assumes thattheevent organizers are directly involved in vending the alcohol.The other type is "host liability only"which assumes the event organizers are only convening theevent,butothers (who have their own insurance)are vending the alcohol. In our case: Only personnel from the restaurants and the beer company sponsor are directly vending the alcohol atthe WHEELS street bash,not ECGA.However,since WHEELS is providing wristbands foronefree drink,andtherestaurant owners are deeply involved members ofthe host committee,I decided to go with the more expansive coverage so that there are zero questions and WHEELS,the City and ECGA would have maximum protection.
