Asyouhave been ableto see fromthe documentation I'veprovided,OrchidMania South Florida isa registered 501c3 organization thatwas established in October of1997.Wehave been providing fundstoProjectCradle,aprogramatthe University of Miami Leonard Miller Schoolof Medicinal Department of Pediatric Infectious Disease andImmunology that provides assistance to children affected and infected with HIV/AIDS,through our fundraising event duringallthis time. The event DOESNOTinvolvedrinking,music,orfood vendors.We are simplya group of volunteers that bringthe plants we've been caringforall year ata greenhouse nearby that is donated forusto use,byFairchildTropicalBotanic Garden,and set them upin the area of the park I showed youduring our meeting,tobesoldto the general public. I ask thatyou please allowusto continue hostingour event at Dante Fascell Park,as we have never caused anyproblems;thepolicehave never been calledtothepark because ofanykind of disturbance related to the event,once the event is finished,we make sure the area is clean and cleared ofany possible rubbish thus eliminatingthe need foradditionalworkon the partof the Sanitation Department.Furthermore,the Cityof South Miami Parks and Recreations Department does not loose any revenue,since you can rentout (and have always rented)the two existing pavilions during the time our event isheld. Thepaymentcanbemadeintheform of a personal check,money orderorcertifiedcheck.o Rental Application Form ©Failure to adequately clean,damage toorloss of park property orviolation of parkrules,willresultinthe loss of allorpart of the deposit. 2.Insurance ©Priortotheuse ofthefacilityorfacility,proof of insurancemayberequiredwiththepolicynamingthe City of SouthMiamiasanadditionalinsuredfortheduration oftheevent. ©ThecoveragefortheCityshallbe primary and non-contributory andwrittenon Florida approved forms byaninsurance company licensedbytheState of Floridatosellinsurance. ©Thecompanyshallnotbeasurpluslines carrier andtheinsuranceshallincludecontractliability coverage. 3.Activity ©Any unusual equipment and/oractivity,asmaybedefinedbytheCityinitssoleandabsolute discretion, mustbeapprovedbytheDirector of ParksandRecreationandnotedonthefacilityreservationform. ©All equipment must bedeliveredandremovedwithintherentalperiod-nostorageareasareavailable. ©All delivery vehicles must remainindesignatedparkingareas.Excessive amplified musicorotherloud noises,asdeterminedbytheCityinitssoleandabsolutediscretion,whichdisturbotherparkpatronsor adjacentpropertyownersareprohibited.Rides of anykindincludingponyrides,mechanicaland inflatablerides,pettingzoos,etc.arenotpermittedinany City park. ©Use of rentals,including pavilionrentals must be completed bysunset.Use of rentalsatPalmerParkand the Gibson-Bethel Community Centermustbe completed by 10:00PM. 4.Conduct ©Permitteemusttakeallnecessarystepstocontrolall of theirguests/participants/spectators.Any person(s) violating any of the Rules and Regulations containedherein,any City ordinancesorotherlaws,orwho are,intheopinion of theParksandRecreationDepartment,constituting apublicnuisance,potential hazardtothepublicorparkproperty,orexhibitingdisorderlyconduct,maybeexpelledfromthefacility by ParksandRecreationDepartment staff orPoliceDepartment without arefundfortheremainder of the permit period. ©All vehicles must remaininthedesignatedparkinglots. ©Theconsumption of alcoholic beverages andgambling of anyformisprohibitedinallCity parks. 5.Set up/Decoration ©Decorationsmaybeattachedwith MASKING TAPE ONLY.Theuse of staples,nails,tacks,etc.is prohibited.Therental period shouldincludesufficienttimeforset-up of facility,conductingtheevent, and cleanup ofthefacilitytothe satisfaction of the Parks andRecreation Department. ©All facilities/furniture/equipment mustbe returned toits original condition andbe placed inits original position if moved.Any cleanup performed bytheCitywillbedeductedfromthedeposit and/or billedto the Permitee. 6.Food Service ©Cooking of foodmustbedonein prescribed areas only.Nofoodordrinkswillbeallowedinthe GymnasiumattheCommunity Center.TheCity of SouthMiami,Miami-Dade CountyandtheState of Florida may require permits/licenses when food istobe sold.Itisthe responsibility of the permittee to contactthese agencies forthis information andto obtain all required permits. 7.Special Requirements ©Occupancy Limits-30 people per pavilion at Dante Fascell Park;30 people at Fuchs Park and 80 people intheMultipurpose Room. ©Additional trash receptacles,cleanup crews,portable toilets,off-duty police officers,or other special conditionsmayberequireddependingonthefacility,number of attendees,nature of eventorother Rental Application Form miscellaneousconditionsasdeterminedbytheCityinitssoleandabsolutediscretionand/orinthepublic interest.Thesale of merchandise of anykindisprohibitedunlessotherwisenotedontheapplicationform. 8.Pets ©Withtheexception of serviceanimals,animals/petsarenotpermittedintooruponCityparksand facilities unless otherwise required bylaw. 9.Cancellations and Refunds o Cancellations made72hoursinadvance of scheduled reservation dateareeligibleforarefund.No refunds thereafter. Restrictions/Revocation ©The Director of Parksand Recreation reservestherighttoapproveordenyany application onacase- by-case basis,including butnotlimitedto,limited capacity of parkorfacility,impact onsurrounding neighborhood,orsuch application isnot consistent withtheParksand Recreation Vision,Mission and/or Values. ©Permitsare revocable,without noticeatanytimebythe Parks and Recreation Director or their designee fora violation of the permit conditions,any park rule,ordinance,Federal or Statelawor if the Director of Parksand Recreation determines the permitted useisathreatto the health,safety,or welfare of the public. ©Should alapseinthegeneralliabilityinsurance policy occur,theCity reserves therightto revoke the user's permit.
