RE: Report of Asbestos Abatement & Work -in- Progress /Clearance Air Testing for the
Retail Space Located at 5810 Sunset Drive, Miami, Florida ( "the Site ")
Dear Mr. Ng:
Enclosed please find the analytical results and certification for the asbestos abatement and
clearance air testing performed at the site.
The abatement project consisted of the removal of approximately 320 square feet of
asbestos containing vinyl floor tile and mastic within the south area of the retail space. The
abatement was conducted by affiliate Anders Environmental Group, Inc., on April 5, 2006,
under the supervision of Sitech Environmental Corporation ( "Sitech ") representative Mr.
Winston Radhay.
The work areas were regulated and enclosed with polythene sheeting to create a
containment. A HEPA machine was used to create negative pressure within the work area.
The materials were abated underwet conditions and caution was taken to not cause a fiber
release into the air. No visible emission of dust to the outside areas were observed during
the abatement activities. Prior to encapsulation of the work areas, a visual inspection of
the work areas was performed. All accessible previously identified asbestos - containing
vinyl floor the and mastic material within the south area of the space was removed and the
work area was clean and free of visible debris.
A total of five (5) area samples including two (2) Quality Control Blanks were collected
during the work -in- progress /clearance air testing performed within the site during the
removal of the asbestos - containing materials.
The results of the analysis of the samples utilizing the NIOSH 7400 Method did not reveal
fiber concentrations above the Environmental Protection Agency (EPA) clearance criteria
of s 0.01 fiber per centimeter (f /cc) signifying that the air in the work areas is acceptable
and ready for re- occupancy.
The results of the analysis can be found in Attachment A "Clearance Air Test Analysis
Report". The certifications for air testing can be found in Attachment B "Certifications ". A
copy of the DERM notification can be found in Attachment C "Notification to DERM"

1.0 INTRODUCTION
At the direction of Mr. Abe Ng of Canton Restaurants, a survey of potential asbestos
containing materials (ACM) was conducted within the interior of the Canton and Sushi
Maki Restaurant Office Space, located at 5810 Sunset Drive, South Miami, Florida. The
asbestos survey was performed by Mr. Ashmeed Mohammed, a Florida Licensed
Asbestos Inspector (see Appendix I). The purpose of the survey was to locate and
identify ACM within the accessible interior building materials.
The scope of this survey included the vacant space of the restaurant. The roof and the
exterior of the restaurant were not addressed in this scope. Any suspect material
encountered within wall cavities and beneath the floor covering should be treated as
ACM and should not be disturbed until identified through sampling and analysis.
More specifically, the scope of services for this project consisted of the following steps:
➢ Bulk sampling of suspect ACM
➢ Polarized Light Microscopy (PLM) analysis of bulk samples
➢ Final report preparation

2.0 SITE DESCRIPTION
The commercial property located at 5810 Sunset Drive, South Miami, Florida is a one (1)
story building which encompassed an area of 975 square feet. The ceilings consisted of
wood. The interior walls were constructed of interior wall plaster and wall board and
joint compound. The flooring consisted of vinyl floor tile and ceramic floor tile.

3.0 SURVEY METHODS AND LABORATORY ANALYSES
Sampling protocol used in this asbestos survey is listed in Title 40, Code of Federal
Regulations (CFR), Part 763.86 for friable surfacing materials and friable miscellaneous
materials (e.g. ceiling tiles). The U.S. Environmental Protection Agency (EPA)
regulations require that sample locations be randomly selected. Suspect ACM were
identified and samples of each different type of material were obtained.
The bulk sampling procedure utilized for the collection of samples suspected of being
ACM within the site required the establishment of homogeneous sampling areas. A
homogeneous sampling area is defined as an area of friable or non - friable material of the
same type that appeared to be applied or constructed during the same general period of
time. This is the most acceptable and cost effective method for sampling of suspect
ACM. Samples collected from these pre - determined homogeneous areas were labeled
and transported to EMSL Analytical, Incorporated for analysis using PLM of bulk
samples. PLM is a method of analysis that utilizes the unique optical crystallographic
properties of the various crystalline forms in the samples. These properties such as
refractive indices, birefringence, sign of elongation and extinction angle, are unique to
crystalline asbestos forms and therefore can be used to identify the type of asbestos
mineral as chrysotile, amosite, crocidolite, anthophyllite, tremolite or actinolite. Each
type of asbestos displays unique characteristics when subjected to these tests.
Percentages of the identified types of asbestos are determined by visual estimation.
Attempts are made to mix the sample thoroughly to provide a more accurate percentage.
Any material containing over one percent (1 0/6) of any type of asbestos is considered by
the USEPA and Occupational Safety and Health Administration (OSHA) to be an ACM
and, if disturbed, must be handled according to specific State and Federal regulations.

4.0 SUSPECTED ASBESTOS CONTAINING MATERIALS
Table 1 lists homogeneous areas identified during the site inspection and laboratory
analytical results from bulk sample analysis of the respective building materials. Of the
materials tested, two (2) homogeneous areas were identified as ACM by the laboratory.
Copies of the analytical results and chain of custody record are included in Appendix II.

5.0 DESCRIPTION OF ASBESTOS CONTAINING MATERIAL
Two (2) of the homogeneous areas were found to contain more than 1% asbestos. A
description of the materials is summarized below, as well as in Table 2, Asbestos
Containing Materials.
➢ A brown layer floor tile below the light beige 12" x 12" vinyl floor tile was found
in the kitchen area covering an area of approximately 220 square feet. The brown
layer vinyl floor tile contained 3% chrysotile asbestos. The material is non - friable
and in good condition.
➢ The black mastic below the light brown 12" x 12" vinyl floor tile was found in the
storage area covering an area of approximately 130 square feet. The black mastic
contained 3% chrysotile asbestos. The material is non - friable and in good
condition.

6.0 CONCLUSIONS AND RECOMMENDATIONS
An asbestos survey was performed within the commercial property located at 5810
Sunset Drive, South Miami, Florida. Results of the survey and laboratory analysis
revealed that two (2) of the homogeneous areas tested was ACM as defined in the
National Emission Standards for Hazardous Air Pollutants (NESHAP), Title 40 Code of
Federal Regulations (CFR) Section 61, Subpart M.
Under the National Emission Standards for Hazardous Air Pollutants (NESHAP), all
regulated asbestos containing materials (RACK must be abated prior to renovation or
demolition with notification to the Florida Department of Environmental Protection
(FDEP). RACM includes all friable asbestos containing materials, non - friable asbestos
containing materials that have or may become friable and non - friable asbestos containing
materials that have been, or will be, subjected to abrasion, exceeding 160 square feet or
260 linear feet. Work involving asbestos containing materials must satisfy all federal,
state and local regulations in addition to NESHAP.
Identified non - friable asbestos may be managed in place under an Operations and
Maintenance (O &M) Plan. In the event the structure is remodeled, refurbished, or
demolished, all identified friable asbestos must be abated. Any suspect asbestos
containing material discovered in inaccessible areas should be treated as ACM until
laboratory analyzed.

This document, issued under the provisions of Chapter 24, Miami -Dade County Code (Miami -Dade County
Pollution Control Ordinance), shall be valid from 15 -JAN -2006 through 14 -JAN -2008. The above named
permittee is hereby authorized to operate the pollution control facility at the above location which
consists of the following:
Eating establishment facility(ies) with 28 seats, one 950 gallon grease interceptor(s) and
appurtenances with effluent discharge to sanitary sewer.
This facility is subject to conditions listed below and in the following pages (if any) of this
permit.
SPECIFIC CONDITIONS
1. Grease interceptors and /or grease recovery units shall be of a type and capacity approved by the
Chief Plumbing Official (or State of Florida for Miami -Dade County Public Schools) that are
accessible for cleaning, maintenance, inspections and sampling. The installation of grease
pretreatment devices shall comply with requirements of DERM Plans Approval Process. No grease
intercetor or recovery unit shall be altered in any way that reduces, or eliminates the grease
retention capacity it was designed to operate by. During business hours the grease pretreatment
system shall be operating as designed except when the unit(s) is(are) temporarily out for repair.
2. Grease interceptors and /or grease recovery units shall be maintained in proper operating
condition by periodic removal of accumulated wastes. Total content of each interceptor shall be
removed by a certified liquid waste transporter and disposed of at an approved facility, at
minimum, 2 times per year. Total content of each grease recovery unit shall be properly disposed
of. No liquid waste shall be discharged from the interceptor to the sanitary sewer system in ways
other than that approved by designed means. Receipts of disposal shall be kept for one year and
made available to DERM upon request.
Each grease pretreatment system shall be accessible at normal operating hours, or at reasonable
times for compliance and /or sample inspections. Effluent discharge shall not exceed 100
milligrams per liter as a grab sample, or 50 milligrams per liter as a daily average for Oil &
Grease, nor be less than 5.5, nor - greater than 9.5 for pH. Effluent discharge to the sanitary
sewer system shall not exceed the Miami -Dade County Sewer Discharge Standard. An accessible
sample point shall be provided and maintained on the discharge side of the interceptor(s) and /or
recovery unit(s) where no further treatment has been determined to occur. The outlet tee of each
interceptor shall extend to within eight (8) inches of the tank bottom.
No grease pretreatment system shall be the cause of, or maintain a sanitary nuisance condition,
or groundwater contamination from leakage, overflows or line deficiencies. No pretreatment system
shall maintain a condition that allows groundwater infiltration or stormwater inflow. No inflow
of water, or wastewater shall be introduced into a pretreatment system by indirect disposal,
clean -outs, pumps and hoses, or by any other means other than the inlet pipe on plans approved by
Miami -Dade County
