FOR AN AMOUNT OF $6,560.00 FOR THE PROVISION OF TRANSPORTATION FROM AREA SCHOOLS TO THE AFTER SCHOOL HOUSE PROGRAM AS DESCRIBED IN THE CHILDREN'S TRUST GRANT AGREEMENT AND TO BE CHARGED TO THE CHILDREN'S TRUST GRANT ACCOUNT NUMBER; PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, the City of South Miami has received a grant from the Children's Trust for the purpose of providing After School Tutoring Services to South Miami area residents; and WHEREAS, Del Aguila Bus Service Inc, is a Miami -Dade County Public Schools approved vendor which currently provides bus transportation services in the school system; and WHEREAS, Del Aguila Bus Service, Inc., will provide the daily after school transportation from area schools for an amount of $6,560.00 to be charged to account number 114- 2020 -519 -4075 with a current balance of $15,060.00. NOW, THEREFORE, BE IT RESOLVED /ORDAINED BY THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI, FLORIDA THAT: Section 1. That the City Manager is authorized to issue a work order to Del Aguila Bus Service, Inc., for the provision of daily after school transportation of After School House participants from area schools to the Gibson - Bethel Community Center in an amount of $6,560.00 be charged to the Children's Trust Grant - After School House Program account number 114- 2020 -519 -4075 with a balance of $15,060.00. One of the provisions in the grant allows the City to provide transportation of After School House Program participants from their area schools to the South Miami After School House Program. Services provided by the program can reach a broader audience with transportation provided for program In participants. order to provide our After School Tutoring Services to the entire community as efficiently and effectively as possible, it would be in the City's best interest to contract with an experienced bus transportation company in order to safely transport children to the City of South Miami After School House Program. The attached resolution would allow the City Manager to issue a work order to Del Aguila Bus Service Inc., a qualified Miami -Dade County Public Schools vendor. The vendor was selected after a competitive bidding process involving the following three qualified vendors approved and used by Miami -Dade County Public Schools: Del Aguila Bus Service, A I A Transportation, and Air B Bus Service. The work order would allow for Del Aguila Bus Service Inc. to provide transportation from Area Schools to the Gibson - Bethel Community Center after each school day for children participating in the After School Program. Cost: $6,560.00 Funding Source: Funding for this purchase is included in the Children's Trust Grant - After School House Program account number 114- 2020 -519 -4075; with a current balance of $15,060.00. Del Aguila Bus Service, Inc. agrees to provide: l) Provide safe transportation to children enrolled in the City of South Miami After School Program on a daily basis for the remainder of the 2008 -2009 school year on a timely basis from the area schools attached. 2) Provide to the City - Proof of insurance for the program listing the City of South Miami as an additional insured party. 3) Provide to the City - Background Check documentation for each driver / personnel involved with the program. 4) Provide the services listed above at the agreed rate below as listed in the quotes presented within the original proposal — Also attached. a) Bus service from approximately 1:30 to 4:30pm at a rate of $150 per day for 41 projected days left in the school year assuming this program begins on Monday, April 136i, 2009 for a total projected cost of = $6,560.00 TOTAL The City of South Miami Parks & Recreation Department agrees to contribute to Del Aguila Bus Service, Inc.: 1) Providing the list Area Schools with addresses and pick up times from which After School House Children will need transportation from. 2) Provide an updated daily list of which children need to be picked up from each school by no later than 1 Iam on the day of each pick -up. 3) Provide payment only when receiving the services listed above in their entirety. Invoices can be submitted in a weekly, bi- weekly, or monthly basis for processing. 4) Prompt payment service as it relates to payment to the vendor once invoices are submitted and services rendered. After execution of this memorandum by both parties, and the adoption of services by Resolution from the City Commission, both parties will incorporate the terms of this Memorandum of Understanding. . COMPANY PROFILE Our company was incorporated in the State of Florida in April 1996, and, has been operating routes and field trips within Miami -Dade and Broward Counties for the past eleven years. We are approved by both Miami -Dade and Broward School Boards. We operate approximately 29 daily routes and 600 field trips a month. DISPATCH OFFICE: Our dispatchers are capable of communicating instantly with our drivers. They have stand -by drivers and spare buses at their disposal for any emergencies. A dispatcher will be on duty any time you hire a bus from our company in order to give your supervisory staff the opportunity of communicating with us for any reasons, including but not limited to schedule changes, location of buses. Our drivers and buses receive certification by meeting and adhering to the following requirements: CRIMINAL BACKGROUND CHECK: Drivers are fingerprinted at Miami -Dade County Public Schools by the Florida Department of Law Enforcement and the FBI, with a Level II screening to adhere to the Jessica Lunsford Act. STATE OF FLORIDA MVR EVALUATION CRITERIA AND FREQUENCY: Each driver's DHSMV complete record is examined prior to initial certification. Points will be assigned for each entry on DHSMV driver record. As a general rule, individuals will not usually be approved to drive a school bus if they have: A. More than three points during the past year. B. More than six points during the past three years. C. More than nine points during the past five years. D. More than twenty -five points total on the entire driving record. E. Individuals that have ever been cited for driving while impaired due to unlawful use of alcohol or drugs. A subsequent record check is made every September, January and July. PHYSICAL AND DRUG TEST FREQUENCY: Each driver must pass an annual physical examination using an ESE 479 form, and an annual drug test. Both exams are completed by Miami -Dade School Board approved doctor and facility. DRIVERS: All drivers are certified by Miami -Dade County Public School. They receive certification by meeting and completing the following requirements:. A. Have five years of licensed driving experience. B. Hold a valid commercial driver's license (class A or B) with a passenger and school bus endorsement. C. Successfully complete forty hours of pre- service training. D. Submit a set of fingerprints for background investigation. E. Comply with pre - employment, annual, post accident, reasonable suspicion and random drug tests. F. Annual physical examination. G. Maintain a clean and safe driving record. CLASSROOM TRAINING OF DRIVERS: Prior to initial certification, each driver successfully completes the State of Florida Basic School Bus Driver Training Course. Drivers receive forty hours of training, at least twenty hours of classroom instruction and eight hours of on the road training. TESTING — ROAD AND /OR WRITTEN: Each driver receives a written exam and road test in order to complete the State of Florida Basic School Bus Driver Training Course. COMMERCIAL DRIVER LICENSE, DRUG AND ALCOHOL TESTING: AlA Transportation, Inc. adheres to the Omnibus Transportation Employee testing Act per Federal Regulations. 49 CFR Parts 40 and 382. Drug and/or alcohol testing procedures are performed with regard to: • Pre- Employment Drug Testing • Random Drug/Alcohol Testing • Return to Duty Drug Testing • Annual Drug Testing • Post Accident Drug/Alcohol Testing • Follow -up Drug/Alcohol Testing • Reasonable Suspicion Drug/Alcohol Testing AIA Transportation, Inc. is a Drug Free Workplace under contract with American Drug Free Workplace, LLC, which manages our drug program. SCHEDULED SAFETY MEETINGS: Since our company consists of approximately 35 employees, safety meetings occur very frequently. There is daily contact between drivers and supervisory staff. IN- SERVICE SAFETY TRAINING: In- Service Safety Training is held once a year as part of re- certification in order for drivers to renew their ESE 479 license. The last In- Service Safety Training was held on Saturday, August 18, 2007. ON ROAD SAFETY CHECKS: Safety checks are done periodically to insure that all students are safely transported. This includes checking railroad crossing procedures, loading/unloading students, and proper student management and driver evaluations. INSPECTION CRITERIA: A monthly safety inspection is performed on each bus by our maintenance department and by Miami -Dade County Public Schools Maintenance Facility. The inspection is done in accordance with Florida Statutes Chapter 1006.21 and the Rules of the State Board of Education Chapter 6A- 3.017. FREQUENCY AND LEVELS OF INSPECTION: Our maintenance department and Miami -Dade County Public Schools Maintenance Facility performs monthly inspections. Miami -Dade County Passenger Regulatory Division performs annual inspection on all buses. PREVENTIVE MAINTENANCE: A monthly preventive maintenance service is performed on each bus prior to taking it to Miami -Dade County Public Schools Maintenance Garage for inspection. By performing this preventive maintenance service, we have minimized the number of road calls dramatically. ASE AND CERTIFIED MECHANICS: We have one certified mechanic and an assistant in our company; however, some repairs are contracted out to certified mechanical agencies. BUS REQUIREMENTS: All of our buses are post 1978 models and meet all Federal Guidelines, Florida Statutes Chapter 1006.25 and Florida State Board of Education Administrative Rules 6A -3. COMMUNICATION: All buses are equipped with two -way radios for instant communication, either through our dispatch office or bus to bus. 65 PASSENGER SCHOOL BUSES: All buses are mechanically maintained at the highest level. They are inspected every twenty -eight days at a Miami Dade County Public School's Maintenance Garage for adequate certification. Our mechanic performs a Preventive Maintenance Service on each bus before it is inspected. Drivers perform daily pre - .inspections before departing our bus compound. By performing these inspections and operating a Preventive Maintenance Program we have minimized the number of mechanical breakdowns. You can rest assure that your children will be riding safe dependable buses. AUTOMOBILE LIABILITY INSURANCE: All buses are insured at $1,000,000,00 combined single limit. GENERAL LIABILITY INSURANCE: We carry Commercial General Liability in the amount of $1,000,000.00. WORKERS COMPENSATION INSURANCE: We carry Workers Compensation Insurance in the amount of $500,000.00 per accident. Your institution will be listed as Additional Insured and Certificate Holder on all of our insurance policies. FIELD TRIP FEE: The cost per bus is as follows: $45.00 per hour / minimum 4 hours, billing in half hour increments. Time starts when we arrive at your facility and the time stops when we return to your facility.
