24RESOLUTION NO. ___ _ A Resolution appointing , I 2 3 4 5 6 ______ and to serve on the Health Facilities Authority for a two-year term ending December 5, 2013. 7 WHEREAS, the City form the Health Facilities Authority in 1995 pursuant to Chapter 154, 8 Florida Statutes; and 9 10 WHEREAS, the Health Facilities Authority is required to have five (5) members who are II residents of the City and three (3) members are required for a quorum; and 12 13 WHEREAS, the Health Facilities Authority needs to provide information to the Internal 14 Revenue Service and to appoint a representative to communicate with them; and 15 16 17 18 19 20 21 22 23 24 25 26 27 28 29 30 31 32 WHEREAS, the term of office of the members of the Health Facilities Authority have expired. NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI, FLORIDA, THAT: Section 1. The City ------, Commission hereby and appoints to the Health Facilities Authority and they shall each serve for four (4) year term or until a successor is appointed and has qualified. Section 2. This resolution shall take effect immediately upon adoption.
