Res No 139-13-139576. RESOLUTION NO. 139 -13 -13957 A Resolution appointing Sally B. Philips, EdD to the City of South Miami Health Facilities Authority to serve for a four -year term ending July 1, 2017. -*, WHEREAS, the City of South Miami Health Facilities Authority was created by ordinance no. 11 -95 -1584, dated August 15, 1995, and WHEREAS, the Health Facilities Authority is required to have five (5) members who are residents of the City and three (3) members are required for a quorum; and WHEREAS, The City Commission desires to appoint Sally B. Philips, EdD to serve on the City of South Miami Health Facilities Authority for a four -year term. Appointment shall expire July 1, 2017 or until a successor is duly appointed and qualified. NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND CITY COMMISSION OF THE CITY OF SOUTH MIAMI, FLORIDA, THAT: Section 1. The City Commission hereby appoints Sally B.
